A miner was pulling on a screen in a screener with a metal hook to get the screen over the tension bar, when the hook slipped from the screen. The miner fell backwards and landed on their right elbow causing pain. The miner was seen at an Occupational Health Clinic and received job restrictions.

Employee experienced pain in right shoulder while pounding down a chute.

While ratcheting the come-along used to close a chute the pad-eye/anchor failed and contacted the employee on the head (hardhat), face, and back.

A miner was using a wrench to remove a tachometer bolt when they injured their shoulder. They notified their supervisor and received first aid. They continued to self-manage the injury with first aid. On 01/16/2025 they notified their supervisor that pain was persistent and they would like to go to a clinic. The miner received prescription medication and work restrictions.

While prepping a conveyor belt Splice employee cut left thumb with a skiving tool. 9 stitches.

Employee smashed a finger using a hammer to remove a bag house bag from the metal cage.

Employee was shoveling rock on MCC roof under 15 and 17 conveyors. Shovel hit a metal lip on roof and jarred shoulder. Employee went to ER with pain. Shoulder is sore. Right shoulder

An employee was using a single bladed knife to cut through and remove a fiber material support strap from a pallet with a secured bucket elevator head pulley. The force applied to cut the strap and direction of the cut angle allowed the blade to make contact with the employee's left forearm and created an ~ 1.5" long cut.

Striking chisel with a Hammer (head of chisel was NOT mushroomed), to turn bearing take-up nut tapered adaptor sleeve. A very small piece of chisel broke off and penetrated the leg of the individual using the hammer.

A miner experienced left forearm discomfort after clearing excess rock on a conveyor with a 2' x 6' x 3' wooden board. The following day the miner reported pain in their forearm. On 1/4/2022 the miner contacted the nurse and was directed to schedule a clinic visit since their forearm had not improved. On 1/5/2022 the miner received work restrictions.

Employee was attempting to unclog 3 bin chute on the 2nd floor with a sledgehammer. Shortly after, the employee felt a pain EE's right wrist whenever EE flexed or put pressure on it. Employee also noticed a small bump formed when flexing the wrist in a downward position. Employee immediately went to see the on sight nurse who recommended further evaluation from a medical provider.

Employee complained of discomfort in right shoulder after cleaning production tank with chain and mallet.

EE AND FELLOW CREW MEMBER WERE HANDLING A 250 LB HAMMER TO KNOCK THE NUT LOOSE OFF OF CONE. THEY HIT THE NUT, AND THEN THE HAMMER SLIPPED TO THE SIDE, AND EE'S RIGHT HAND GOT CAUGHT BETWEEN THE HANDLE OF HAMMER & THE CONE STEEL CRUSHING RIGHT THUMB.

Employee was attempting to extract a bolt to remove a paddle from a pug mill shaft. The employee was driving out the bolt with a 3lb sledge hammer and drift pin when the employee struck the edge of the paddle causing a section to chip away and consequently flew up striking the employee in the mouth.

Employee was aligning a hole with an alignment pin while work partner was tightening a 1/2" bolt with an adjustable wrench. During the tightening process, the adjustable wrench slipped off the bolt and hit the work partner in the lip and lower nose area causing a laceration to the lip and lower nose area.

EE was performing maintenance work on aerial lift. EE was working in a tight area underneath vehicle using a hammer to tighten hydraulic hoses. Hammer slipped & slid about 18" down the hose, hitting EE in mouth. EE reported damage to crown on tooth. EE was sent to dentist for evaluation & damage to tooth reported. Employer did not receive knowledge of damage to tooth until 3/5/19.

Pinched Finger in tool requiring 7 stitches.

Maintenance employee was trying to break loose a bearing from the shaft by pounding with a hammer. A piece of the metal chipped off and imbedded in arm. Employee was taken to ER for medical attention. Object removed and 3 stitches.

Employee felt popping in right shoulder when using sledge hammer head to clean silo tanks

Employee was laying on stomach in an awkward position, using two spud wrenches trying to free a steel frame from a wedged position. The employee felt a sharp pain in right shoulder.

Mechanic went home after shift and woke up with a sore right shoulder. Employee came to work the next day and reported injury. The day before the Mechanic was using a 3 pound hammer to pound a wedge in McCawber dome valve to loosen seal. Supv called plant nurse to talk to mechanic Employee is following up with Doctor today

Employee was performing screen change on #8 Rotex in Coloring Building and using wrench to tighten jackscrew. The wrench slipped off the jackscrew causing strain in left shoulder.

Employee was pulling on a pipe wrench and strained muscles in his right shoulder.

Employee was emptying out 2 QT sly bin and using 3 lb sledge hammer. As he was pounding on bin he felt a pain in his right shoulder and back. Went to doctor on 05/28/2015 and has slight tear in shoulder. He believes it is from this incident. Now he is on light duty and therapy.

Employees were attempting to open a bottom rail car door that was jammed. The injured employee was using a bar to open the door and the door unjammed suddenly as the employee was pushing down on the bar. The bar made contact with the track, pinning the employee's left pinkie finger between the track and the bar. The employee experienced a fractured finger tip and required stitches.

Employee was cutting a metal strap off of a pallet holding a set of new screen frames. Employee was reaching across the pallet to cut the strap using a hand cutting tool and it snapped back and cut the underside of the forearm.

Maintenance employee was removing an asset tag from a piece of equipment with a razor knife. The knife slipped and the individual stabbed his left hand. The cut required a total of nine sutures to start the healing process.

Hit left hand little finger with a 4lb sledge hammer handle. Catching the finger between a crescent wrench and hammer handle.

SIC employee was installing sound insulation blankets on line 1 at the crushing bag house. The blanket had metal ID tags to show where they needed to be installed. As he was swinging a hammer to pack the blanket in place, his right hand hit one of the metal ID tags causing a cut. He did have on the Mechanic's gloves.

Wrench slipped off the hex nut. When the wrench slipped, the box end of the wrench made contact with the base of the separator. The employee's hand slipped, from the orignal grip position, down the shaft of the wrench at an angle. This resulted in a hyper extension of the thumb on the Employee's left hand.

During normal work tasks employee strained low right back. The 2 tasks that contributed to the injury were; 1)Scraping snow balls off of stacker hopper bars while reaching out from catwalk. 2 )While cleaning out catwalk around 10 conv. tightner employee was shoveling and was prying a frozen chunk of mud and rock and felt a significant sharp pain in right side of low back.

Employee was using utility knife to cut strap. Knife slipped and employee cut his arm.

Employee was shoveling up and hit a metal plate that was under a pile. Strained right wrist and arm. Was fine at time but months later employeee went to doctor and got prescription meds.

The employee was opening the doors on the 35 ton dust bin. After knocking the bottom bolt out of the door slot with a hammer, employee raised his hand to remove the top bolt. The door unexpectedly sprang open and struck the little finger of the right hand.

Employee had a sore back for about one week prior to injury. Not sure how it got hurt. Then on 5/12/2009, Employee was pounding down a spout with a sledge hammer for about 45 minutes. Afterwards he felt alot of low back pain from pounding. Reported to nurse and iced went to doctor afterwards. Employee recieved 10 lbs lifting restrictions.

Miner was sharpening a hook knife away from his body, knife slipped off the sharpener and cut his finger through the glove.

Employee was trying to loosen a fitting on a water valve, and could not loosen it. Another employee was helping him and was using a second pipe wrench. The wrench slipped and pinched the middle finger on the left had of the employee, between the 2 wrenches.

Employee refused medical treatment @ the time of the incident he didn't feel it was sever enough to warrant medical attention, 11/13/07 returned to nurse requesting medical attention He was tightening a bolt on an HP300 Head whe the wrench slipped hitting the employee's little finger on right hand on the inside of the head.

Mechanic was removing transfer case from 992 weld truck. When transfer case was supported by jack and ready to be lowered, the transfer case shifted slightly. He pulled his right hand back towards his body and caught a sharp edge or burr on the underside of the front part of jack. The employee lacerated the right thumb palm of hand requiring 10 stitches.

Employee was using a fulcrum to raise a cage up so a co-worker could cut the wires of the cage. The empolyee had to maintain pressure on the fulcrum. As the employee was applying pressure, the co-worker cut the last wire. The handle hit the tube sheet and the employee's finger was pinned between the handle and the tube sheet.

While straightening a flange with a hammer, the hammer bounced back and hit employee's lower lip. Cut lip. 2 stitches.

Employee was attempting to cut end off of air hose that goes into tank to clean tank. Employee had air hose placed on the top of the tank to cut the hose when he cut through the hose the knife blade cut through his leather glove and into his left index finger.

The conveyor #46 had plugged up. It was decided that the cover had to be removed. The bolt in the slip stick had been put in before with an impact wrench. The bolt head needed to be sheared off. A chisel and hammer was used. While swinging the hammer at the chisel, it slipped off and struck the left index finger of the employee causing a contusion.

EE was using a metal bar to force the bottom door of the rail car closed. While pushing the bar he felt his left shoulder pain and some arm numbness and pain in the left scapula. he was taken to urgent care.

EMPLOYEE OPERATING COME-A-LONG, LIFTING C-CHANNEL. C-CHANNEL WAS NOT CENTERED OR LEVEL AND TIPPED AND TOP END STRUCK HIS LEFT FOREARM CAUSING LACERATION

Employee was changing Z-screen in Deister and was using a crowbar, the end of the crowbar was under the corner of the Z-screen and as the employee pushed down on the crowbar it slipped out of the corner and smashed the finger of the opposite hand. The left index finger required two stitches.

ROCKS BECAME JAMMED IN #31 ROTEX. EE USING A PRY BAR & WAS HITTING A ROCK IN ORDER TO FREE IT. AT ONE POINT HE MISSED THE ROCK & STRUCK HIS RT.RING FINGER. THIS PRODUCED AN ABRASION, BRUISING OF NAILBED, SLIGHT SWELLING. SEEN CVIC FOR EVALUATION, SUSPECT FRACTURE, SPLINTED UNTIL FILM READ BY RADIOLOGIST. OFF UNTIL REPORT BACK ON 8/5.

USING 8 LB SLEDGE HAMMER, TRYING TO REMOVE NAIL STAKE HIT STAKE LOW, NAIL STAKE KICKED BACK AND HIT PINKY FINGER POSSIBLE FRACTURE.

EE WAS POUNDING ON A BEARING HOUSING WITH AN 8 LB SLEDGE HAMMER TO REMOVE THE BEARING FROM THE SHAFT, AFTER ABOUT THE FIFTH TIME HE FELT A SHARP PAIN IN HIS LOWER BACK.

EMPLOYEE WAS USING A PRY BAR ON A FLATBED TRUCK WHEN THE PRY BAR SLIPPED CAUSING HIM TO HIT HIS ELBOW ON METAL TRUCK FRAME.
