EE bent over to fit the chain underneath a piece of granite that was on the conveyor and felt a sharp pain in EE's lower back.

EE was trimming a piece of radius curbing and EE missed the chisel and hit the top of EE's left hand with the hammer and forced EE's hand on the stone, the knuckles took the hit.

EMPLOYEE WAS SWEEPING OFF THE GRANITE SLAB BEFORE SPLITTING IT. THE BROOM HANDLE BROKE AND SLID ALONG HIS HAND/THUMB.

Ee had one foot on the block that he was "breaking". When it broke, ee fell and hit his side on the block.

EE WAS CHAINING A GRANITE BLOCK INT HE QUARRY WHEN THE CHAIN SLIPPED AND HIS ARM DRAGGED ALONG THE STONE.

EMPLOYEE WAS TRYING TO FREE A STUCK METAL WEDGE FROM THE GRANITE USING A SLEDGEHAMMER. HIS FINGER GOT IN THE WAY.

EE HAD BEEN USING A HAMMER TO FINISH THE CURBING AND WAS PUSHING THE CURBING ON ROLLERS. HE STARTED TO HAVE PAIN IN JUNE 2002, AND TRIED TO WORK AND THOUGHT IT WOULD GET BETTER. ON 7/22/02 HE MADE AN APPOINTMENT W/HIS DR. AS IT WAS WORSE.

EE WAS TURNING OVER A PIECE OF CURB ON THE BANKERS WHEN IT TIPPED AND HE WENT TO GRAB IT AND IT CUT HIS HAND WHEN IT SLID DOWN.

EMPLOYEE WAS SWINGING A SLEDGEHAMMER ONTO A WEDGE. THE HAND HOLDING THE WEDGE MOVED AND WAS CUT BY THE END OF A PIECE OF GRANITE.

EE WAS HITTING A WEDGE WITH A SLEDGEHAMMER AND THE HAMMER MISSED THE WEDGE & HIT HIS THUMB

EE WAS KNOCKING BITS OFF THE DRILL STEEL WITH A HAMMER WHEN A PIECE OF STEEL BROKE OFF THE HAMMER AND HIT HIS LEG.

EMPLOYEE WAS MOVING THE SLOT DRILL, PRYING ON THE DRILL TO GET IT ON LINE.

EE WAS USING A BAR TO REPOSITION THE LIFT DRILL WHEN THE BAR SLIPPED AND HIS HAND HIT UP AGAINST THE DRILL.

EE WAS USING WEDGES AND A SLEDGEHAMMER TO CUT OFF A SMALL PIECE OF GRANITE WHEN THE PIECE BROKE OFF, BOUNCED ON THE GROUND AND THEN HIS HIS FOOT.

EE WAS HOLDING A STEEL ROD ONTO THE DRILL RIG AS THEY WERE TRYING TO REMOVE A BOLT. ANOTHER EE WAS HITTING THE ROD WITH A SLEDGE HAMMER AND MISSED.

EE WAS HITTING A STEEL WEDGE WITH A SLEDGEHAMMER WHEN IT "POPPED" BACK INTO HIS FACE.

EE WAS TRYING TO GET A COUPLING OFF A STEEL DRILL ROD WITH A HAMMER. A PIECE OF THE HAMMER BROKE OFF AND HIT HIM IN THE CHEST.

EMP WAS SWINGING A SLEDGE HAMMER TO PUT SHIMS AND WEDGES INTO THE BLOCKS. WAS DOING THIS FOR QUITE A FEW HOURS EACH DAY. EMP NOTICED SOME PAIN AROUND THE 11-4-96 DATE. THOUGHT IT WOULD JUST GO AWAY SO CONTINUED WORKING. FINALLY COULD NOT STAND THE PAIN ANYMORE ON 11-18-96 AND ASKED TO GO SEE THE DR AT THAT TIME.

EMPLOYEE WAS HAMMERING A STEEL WEDGE WHEN A PIECE OF THE WEDGE BROKE OFF AND FLEW UP AND HIT HIM IN THE NOSE.

EMPLOYEE WAS USING WEDGES & SHIMS ON A RIFT LINE - WHEN HE HIT THE WEDGE, IT BOUNCED OUT OF THE HOLE AND HIT HIS NOSE.

REPETITIVELY SCOOPING PLASTER ONTO A BOARD.

EMPLOYEE WAS TRYING TO REMOVE A WEDGE FROM A HOLE IN THE STONE WITH A METAL BAR, WHEN A PIECE OF STEEL BROKE OFF THE BAR WHEN IT STRUCK THE WEDGE.

EE WAS TRYING TO PRY A PIECE OF GRANITE OVER IN THE KEYWAY WHEN HE LOST HIS GRIP AND THE BAR SNAPPED BACK.
