EE was adjusting the headlamp on EE's hard. When doing this EE was cutting a zip tie that held the headlamp strap on EE's hat. EE punctured EE's left index finger.

Employee was tightening up a fitting on an air starter. When EE was finishing tightening it EE felt a pop in EE's right shoulder. Employee was taken to OMP and has a strained right shoulder with work restrictions.
Employee was pulling a chain hoist in from landing a screen. EE thought EE just pulled a muscle in EE's right arm. EE was taken to OMP for further evaluation on 9/15/23 and given restricted duty.
EE was cutting a piece of center rubber for a screen deck. When making the last cut on the rubber the box cutting knife slipped and struck EE's left leg causing a laceration. EE was taken to the Dr. and evaluated and received 6 stitches on the cut. Accident under investigation.
Employee was cutting a piece of belting with a utility knife, when the blade slipped from the belt cutting their left upper arm. Employee was taken to local ER and received 3 staples.
EE was in the adjustment area on the jaw crusher trying to slide the block behind the toggle plate cover to center it up. The cylinder on the porta-power was at an angle and when pressure was applied, it kicked out and hit EE in the face and left arm. This resulted in 3 stitches between the nose and mouth.
EE was preparing to replace a toggle bolt in the jaw. EE was putting a 3-ton come-along down and EE's thumb got caught in the come-along handle. This caused a laceration on this thumb and EE received 4 stitches
EE was loosening the compressor fill cap of a Sandvick T35 with an adjustable wrench. When the cap let loose, the wrench pinned EE's middle finger against the site glass busting the finger. EE went to the doctor and received six stitches.
Employee was tightening a bolt on the shaker screen wear rubber with a ratchet and socket. The socket was not seated well on the bolt and it slipped and hit EE in the cheek chipping tooth and caused EE to bite tongue. EE was sent to see the dentist for the tooth.
EE was using a 5 foot pry bar to line up hole on crane boom to install a pin and he felt something in his foot making a loud pop.
EE was working on Water Truck MQ0373 and he smashed his thumb between the rear leaf spring and pry bar. He was sent for medical treatment and was placed on light duty status, and prescribe a pain medication that was NEVER filled.
EE was working on transmission housing filter when the socket slipped off hitting his left hand on a piece of angle iron.

Lubrication technician was turning wrench when the wrench slipped allowing his hand to strike frame causing immediate pain and discomfort. Employee failed to report it until 4-21-10. He was sent to doctor, and diagnosed with FRACTURE of 5th metacarpal of left hand. He was able to return to his normal job with a follow up appt. with doctor. No restrictions.

EMPLOYEE WAS USING A BRAKE PRESS TO BEND A PIECE OF PLATE STEEL. THE STEEL WAS NOT POSITIONED STRAIGHT WITH THE PRESS. WHEN HE ATTEMPTED TO BEND THE STEEL, IT FLIPPED UP STRIKING THE EMPLOYEE ON THE CHIN CAUSING A CHIPPED TOOTH.

VICTIM WAS REMOVING A VERTICAL SHAFT FROM A CRANE USING A PORT A POWER. HE DROPPED THE PORT A POWER ON HIS LEFT INDEX FINGER CAUSING A FRACTURE TO HIS FINGER.
VICTIM WAS PERFORMING MAINTENANCE WORK LOOSENING A BOLT. THE BOLT BROKE AND HE FELL AGAINST HIS ARM CAUSING A BRUISE TO HIS ARM.
