Employee was clearing out a blockage on the fines chute underneath the jaw of the primary plant. The employee was using a metal bar to loosen the stuck material. When the last rock came free instead of falling down the chute it came out of the access hatch hitting the miner in the face and hand.

EMPLOYEE WAS REMOVING ANVILS FROM A CRUSHER UNIT. A PULLING TOOL COULD NOT BE FOUND, SO EMPLOYEE WAS USING A CHIPPING HAMMER TO EXTRACT THE ANVILS. THE HAMMER SLIPPED, PIERCING HIS LIP AND CHIPPING A TOOTH.
WORKER WAS LOOSENING A NUT ON A CONE CRUSHER UNIT. A WRENCH WAS ATTACHED TO THE NUT, AND THE WORKER WAS HITTING THE WRENCH WITH A 4" SHAFT. WORKER MISSED HIS TARGET AND SLAMMED HIS FINGER AGAI NST THE WRENCH.
EE ATTEMPTED TO CLEAR A BRIDGE OF MATERIAL IN THE PRIMARY JAW CRUSHER WHILE RUNNING WITHOUT AUTHORIZATION WITH A PRY BAR. THE BAR GOT CAUGHT BETWEEN A PIECE OF METAL & THE MOVING SIDE OF THE J AW WHICH CAUSED THE PRY BAR TO STRIKE EE
