EE was knocking build up out off the screen discharge hopper with a 3' pry bar. EE went to hit a piece of buildup on the hopper, and the bar hit a crossmember causing it to be knocked out of EE's hands. The bar fell and hit the inside of EE's left ankle causing a scratch that became infected.

Employee was chiseling frozen dirt from the frozen screen rail & the hammer slipped out of hand and hand continued into the frozen dirt. Employee did not think it was broken only bruised. Notified the company when happened and was going to ice and otc pain meds if pain continued. Over the weekend swelling went down & employee noticed bone displacement and went in for x-ray.

Employee was driving the stakes for the stacker with an 8 lb slug hammer and pinched sciatic nerve.

Employee was trying to clean over the 5th wheel plate as material was piling up. Attempted to clear material with a metal bar and the metal bar caught in the roller and pulled hand up into the belt. Employee called on radio and called for emergency shut off.

EE was helping change teeth on loader, another EE hit the tooth with a hammer and part of the tooth broke off and hit EE in the left cheek below EE's safety glasses. This was a first aid until EE was seen 6/20/18.

Employee was preparing to replace some belting on the stacker. The conveyor needed to be rotated, so employee used a pry bar to rotate the pulley to get the belt seam off of a roller. The bar slipped off the tail pulley and jammed wrists from the shock load of slipping. This caused a contusion to the left wrist resulting in suspected fracture of capitate bone.

Standing on catwalk of jaw. He was using a pry bar cleaning a clay chunk out of chute. He felt pain in his right knee. He didn't report pain until next day to his supervisor.

Cutting belting on conveyor. Razor knife slipped cutting his left thumb, requiring 5 stitches to close wound.

Struck left hand thumb with hammer while changing out tooth on loader. Caused contusion on thumb.

Using razor blade utility knife to cut conveyor belt, knife slipped lacerating index finger on left hand.

Removing a bolt with a crescent wrench. Wrench slipped off the nut a struck EE in the nose. He had just had surgery for a dievated septum and was healing when he hit his nose with wrench. He had to have his nose re-set again.

Cutting a piece of rubber on the bobcat skid steer with a razor knife. EE slipped with knife cutting his left hand requiring stitches in little finger.

Changing a tooth on a loader, using a hammer to drive it on when a piece of metal flaked off and punctured his skin on his chest. He went to a doctor, they removed the metal and put two stitches and allowed employee to return to work full duty.

EE WAS CLEANING MUD OUT OF THE SCREEN WITH A HAMMER. HE STRUCK HIS ELBOW ON SIDE OF PLANT CAUSING A CONTUSION. HE WAS STILL HURTING ON 6/02/05 SO HE WENT TO DOCTOR TO GET IT CHECKED OUT. IT WAS JUST A CONTUSION, THE DOCTOR PLACED HIM ON RESTRICTION UNTIL 6/09/05

EE WAS CUTTING A BELT USING A KNIFE WHEN IT SLIPPED AND CUT HIS THUMB. REQUIRED STITCHES TO CLOSE WOUND.

PULLING ON A CHAIN BEING USED TO LIFT A CONVEYOR. THE CHAIN BECAME STUCK. THE EMPLOYEE PULLED HARDER ON THE CHAIN WHEN IT FREED UP IT STRUCK EMPLOYEE IN MOUTH. TWO TEETH (MIDDLE BOTTOM) WERE K NOCKED LOOSE. EE WILL BE CHECKED IN ABOUT A WEEK TO SEE IF ANY TREATMENT WILL BE NEEDED.

EMPLOYEE WAS PULLING WITH A WRENCH ON A STUCK FITTING WHEN THE WRENCH SLIPPED CAUSING HAND TO CONTACT A SHARP EDGE. EMPLOYEE LACERATED HIS LEFT THUMB REQUIRING STITCHES.

EMPLOYEE WAS CUTTING A CONVEYER BELT. HE SLIPPED AND CUT HIS RIGHT THIGH.

EE WAS HITTING A CHISEL WITH A HAMMER WHEN THE HEAD FELL OFF AND STRUCK EE ON HIS PINKY FINGER OF L. HAND CAUSING HIM U LOOSE NAIL AND REQUIRED STITCHES TO REPAIR WOUND.

CHANGING TIP ON BARMAC CRUSHER, LOOSENING AND TIGHTENING BOLTS, AND CHANGED SCREEN, DID NOT NOTICE UNTIL HE RETURNED TO DOG HOUSE AND SAT DOWN HE FELT AN INTENSE PAIN IN STOMACH.

EE WAS CUTTING BELTING WITH A RAZOR KNIFE AND MADE A 5 CM LACERATION IN HIS LEG.

EMPLOYEE WAS ATTEMTPING TO SMOOTH ROUGH EDGE ON NEWLY CONSTRUCTED STORAGE SHELVING USING A CHIPPING HAMMER. HAMMER MISSED TARGET AND STRUCK EMPLOYEE'S RIGHT LEG (SHIN).

ROCK FROM CONVEYOR FELL & STRUCK EMPLOYEE ON THE HEAD EMPLYOEE WAS HELPING FREE MATERIAL THAT HAD JAMMED THE BELT INJURY REQUIRED STITCHES TO THE HEAD
