EE was removing "bolts" from the Meva forms and EE was using a hammer to tighten down a pancake nut. When EE wrenched the nut, EE felt a sharp pain in EE's shoulder. EE treated it with OTC medicine and ice at the time of the incident. As of December 11, EE has now seeked medical attention.

Employee was removing the lip shrouds off the BE spare bucket in Etterville using a 12 pound soft blow hammer. While striking the rear of the lip shroud with the hammer, a piece of shrapnel came off either the hammer of the lip shroud and struck the employee in the right shin area causing a laceration.

On April 17, 2018, employee utilized a sledge hammer to drive out pins on drill side frame. On May 2, 2018, the employee reported pain in left shoulder. On May 11, 2018, the incident review attributed the shoulder pain to the repetitive motion of repeatedly swinging (~75 times) the 8lb sledge hammer.

EE was torquing sideframe roller bolts on the backhoe and reported soreness in his left shoulder. Under further investigation and physical examiniation that it was tendinosis. The incident was November 18, 2008 and was working full duty until his scheduled surgery for 04/28/2009
Employee was assigned to work with the mechanics to remove the trommel rock box in order to remove and replace the conical rock deflector. Employee was cutting the welds and was using a pry bar to remove the structure. Using a 5 ft bar and prying up and the bar slipped and hit himself in the jar. The skin was not broken or bruised.
Employee was doing his routine shop clean up and while sweeping his area with a hand broom, his left pinky finger came in contact with a piece of metal in the steel cutoff rack.

Hammering on the clamp, pipe broke free, hitting ee in the face. Went to clinic, cut received on upper lip did not require stitches, ee was told stiches could be administered for cosmetic purposes (not leave scar)

Replacing pipe unions & nipples on a HAV unit, taking out the gas unit and tightening an elbow using a 3" x 3' cheater pipe over a 36" aluminum pipe wrench. The cheater pipe slipped off the pipe wrench, bounced off the bridge of his nose and then sliced into the left nasal alae area. He had a 3.5 cm curvilinear laceration that was tangential and down to the deep cartilage.
