Mostrando los 25Employee was working with a "PRY BAR" while replacing Screen Cloth at the Secondary Plant. The Pry Bar slipped and employee's L/Pinky finger in the nail area got caught.
The bucket on the skid steer loader was stuck in the up position. EE was trying to get the bucket to release and rest on the ground. EE was hitting the machine with a hammer. EE got the bucket to release but was standing a little too close to the machine bucket. When it released, the teeth on the bucket scraped EE's left leg in the shin area.
EE was using a "pry bar" to loosen screen cloth EE was changing, when EE began to feel pain in lower back.
EE was holding onto an area where a "Nut" was attached with a wrench while another employee was using another tool a "Power Wrench" to loosen the nut. The vibration from the "Power Wrench" caused ee to loose grip, the wrench ee was holding onto "slipped" and the result was a "Fractured finger".
Employee was using a utility knife and did not position self correctly when using knife. Employee did not use proper procedure when cutting with keeping EE's wrist and arms tight to reduce being struck by.
Employee was helping to set a crusher bowl in place, one of the cables which they were using to support the crusher bowl broke and hit him in the back of the head and neck area.
EE and co worker were replacing a "Jaw Crusher Maganese Bolt". EE was using a "Sledge Hammer" and bar to loosen the bolt, he missed hitting the bar and the hammer motion took him, and the hammer into a steel plate which is part of the jaw. He was still holding on to the hammer handle and his fingers got caught between the plate and handle.
Employee was cleaning the shop area with a broom when he began to feel pain in the muscles of his upper back.
Knocking pins out of a bucket which was attached to a front end loader. A piece of metal broke off and cut his left arm
Employee was using a hammer to remove "build-up" from #7 screen. A chip from a rock flew upwards and hit him in the left eye. Employee was wearing his safety glasses.

Employee was trying to remove a nut from C-9 conveyor using a spanner wrench. He realized he was tightening the nut rather than loosening it, so he stopped applying pressure on the handle. When he stopped, the handle flung back and hit him on the right thumb knucle and broke it.
Ee was holding a "striker wrench" while another employee was hitting the wrench with a hammer. Ee moved his hand and the other employee missed the target and hit ee's third finger left/hand.
EMPLOYEE WAS USING A PICK AX WHILE DOING HIS JOB HE SWUNG THE AX BACK AND IN DOING SO, HE HIT HIS HAND ON A PIECE OF METAL AND CUT HIS RING FINGER IN THE JOINT AREA. THE CUT REQUIRED (5) STI TCHES.
EMPLOYEE WAS USING A STEEL WEDGE TO REMOVE A TAPER LOCK BUSHING, AFTER GETTING THE DESIRED LOOSENESS WITH THE WEDGE, HE BEGAN TO KNOCK IT OUT OF POSITION, AS HE DID THIS, THE WEDGE SHOT OUT OF POSITION, AND HIT HIM IN THE LEFT LEG CAUSING A DEEP LACERATION.
EE WAS USING A HAMMER TO NAIL THE PINS THAT HOLD THE BUCKET IN PLACE ON THE SHANKS, HE MISSED HITTING THE PIN AND HIT HIS L THUMB INSTEAD.
EE WAS USING A SLEDGE HAMMER TO GET A BUCKET LOOSE FROM THE MACHINE AND HE TWISTED HIS BACK IN THE PROCESS.
EMPLOYEE WAS SHOVELING UNDER THE CONVEYOR, HE JAMMED HIS THUMB BETWEEN THE HANDLE OF THE SHOVEL AND THE FRAME HE WAS WORKING UNDER.

EE WAS WORKING IN THE SHOP AREA CUTTING OR SAWING A PART IN HALF, THE SAW SLIPPED FROM THE AREA HE WAS SAWING ON AND CUT HIS LEFT THUMB. THE CUT WAS DEEP AND IT REQUIRED 8 STITCHES TO CLOSE TH E WOUND.
EMPLOYEE WAS CL9ANING UNDER AND AROUND THE CONVEYORS WHEN THE STEEL BAR HE WAS USING GOT CAUGHT. HE PULLED ON IT, IT FLEXED AND HIT HIS LITTLE FINGER AND BROKE IT.
