Mostrando los 19Two employees were removing a pivot shaft seal using a pry bar and a utility knife. While one employee was prying on the seal the other employee was cutting the seal when the pry bar slipped off and struck the employee that was cutting the seal. Employee received three stitches in nose.
Employee was using a Hilti caulking gun to dispense epoxy into loose bolts. Employee failed to puncture the seal on end of tube and pumped pressure into the tube. When nothing came out, employee turned tube to look inside the end, the seal burst shooting epoxy onto safety glasses and into employees eye.
Employee was cutting a zip tie off of an extension cord. The blade of the pocket knife slipped and struck the employee in the left hand. The employee received 6 stitches on left hand.
Employee was using a pocket knife to cut a hole in and vent the bottom of a 1 gallon RV antifreeze bottle used to fill the water system of a blast hole drill. The bottle slipped from his hands, employee tried to catch it and in the process the knife cut his left thumb requiring seven stitches.
Person had is arm and hands between a fusion machine carriage using a crescent wrench. He was using this tool to wrench out the liner to help the liner pipe line up. staff member closed the carriage on his arm. This was caused from miscommunication between the two staff members. Tyrone was checked out a medical facility and had no breaks or cuts. He did have a bruise.
Employee was using a pry bar to dismantle and repair a light duty vehicle drive shaft when he sustained right shoulder pain. Was reported as a minor strain and employee would monitor improvement. Smoky Canyon received notification 12/12/11 from workmans comp that this injury was work related after an investigation was completed.
Employee was removing roller pin with a porta power from the under carriage of a D10 dozer. Pin became wedged,employee was tapping it with a 3lb. hammer. When the pin gave free it hit employee in the thigh bruising it. The next day employee's leg hurt, we sent him to the doctor. Examination showed possible blood clot, employee was admitted into the hospital for observation.
Employee was using a porta power to push a pin out of track frame, the pin slipped and porta power fell on employees right shoulder. There was damage to a tendon and surgery was done on 11/22/2010, first lost day was 11/23/2010.
Employee was removing a track roller off a 3600 Hitachi shovel, it was hung up and employee was prying on it with a pry bar. He felt a burning sensation in his groin area.
Employee was installing a rear gear on the end of a camshaft. He missed the punch with a hammer and hit his hand on stud between the middle and ring finger. He was taken to local medical facility. Hand was soaked in antibiotic liquid, ex-ray taken, and given a tetnus shot. Employee was given prescription for antibiotics and released to full time non restricted work.
EE was using a rubber mallet to remove wet product from a bucket. Felt a strain in the middle of the back.
EE WAS PRYING THE CAP OF AN AEROSOL PAINT CAN WHEN THE SCREWDRIVER SLIPPED & STABBED HIS LEFT HAND.
BEATING DENTS OUT OF BUCKET ON SMALL BACKHOE. SMASHED LITTLE FINGER BETWEEN HAMMER HANDLE & CONCRETE FLOOR.
USING A PRY BAR TO UNPLUG FEED CHUTE. BAR SLIPPED CAUSING EMPLOYEE TO TWIST BACK.
OPENING A CAN OF COFFEE. PULL TAB BROKE OFF OF LID. CONTINUED TO REMOVE LID WITH MULTI-TOOL & IT SLIPPED AND CUT HIS THUMB. HE RECEIVED 4 STITCHES.
WORKING ON TRACK DRILL. 2 EMPLOYEES WERE PULLING ON PIPE WRENCH WHEN THE WRENCH BROKE. ONE EE FELL ON THE OTHER'S KNEES. WENT TO DR. ON 6-11-02. BECAME REPORTABLE 6-17-02.
WHILE CHANGING OUT A SHOVEL BUCKET, EE WAS KNOCKING WEDGE WITH A 12LB HAMMER. THE EE LOST HIS FOOTING CAUSING THE EE TO FALL ON THE HAMMER HANDLE & HIT HIS SAFETY GLASSES SMASHING THE BRIDGE INTO HIS NOSE. THE NOSE CUT REQURIED 5 STITCHES
EE WAS REMOVING ROCKS FROM SCRUBBER WITH A PRYBAR. HE FELT A PAIN IN THE LEFT SIDE OF HIS NECK. THIS LEFT SIDE WENT NUMB DOWN TO HIS WAIST.
