Employee was swinging a sledge hammer to free a steel component. As the hammer made the upswing it hit a piece of steel causing the hammer to deflect as it traveled back to the ground. The employee was struck by the handle of the sledge hammer in his shin. The result was a bruise that became infected over the weekend.

Crusher repairs: The supervisor welded a pad eye on the jaw plate and the employee attached a come-along to the pad eye. During the work activity the employee increased the tension on the chain and the weld holding the pad eye in place failed. The chain from the come-along hit the employee's elbow.

GREASING THE CRUSHER, HURT HIS SHOULDER.

WHILE DRIVING PINS OUT OF A BUCKET USING A BAR AND A HAMMER THE BAR CAME OUT AND LANDED ON TOP OF EMPLOYEES LEFT FOOT
