Maintenance employee was pounding on a bearing assembly with a 16 pound sledge hammer. After the job was completed they noticed pain in right wrist. The next morning their wrist was stiff and sore. The employee did report that their wrist had been sore with pre-existing pain for sometime.

When the employee was cutting a plugged hose the knife slipped and cut his hand. The employee went to the ER to have his hand looked at. The ER opened his hand up to see if he had nicked the tendon. The doctor used 3 stitches to close the cut
Employee loaded shooter with cap - In an attempt to hook lead line employee accidentally shot the shooter injuring his left index finger.
Employee was tightening segment bolts on a dozer sprocket and the socket adapter broke causing the employee to strike his arm on the dozer frame. The employee reported the incident and finally asked to see a doctor in August. He finally got a doctor's appointment in September and the MRI showed a rotator cuff tear and was reported to the mine on September 15.
While stripping wire to connect to a new pump the employee cut his own hand requiring 9 sutures
Emplyee was removing a bucket tooth, hit the tooth with a hammer to free it and a small piece piece of metal flew off and struck his upper leg requiring three sutures
Employee was adjusting a bar on the shovel bucket while replacing/adding shims to the trip bucket. The shovel operator thought he was signaled for him to trip the bar. Employees finger was between the trip arm and the shovel door resulting in a broken, lacerated right index finger.
While using a masking tape paper cutter employee lacerated left index finger requiring five sutures.
EE WAS WORKING ON A CLEAN-UP CREW AFTER A MILL RE-LINE. HE WAS USING A LONG BAR TO LIFT ROLLER ASSEMBLIES FOR RIGGING WHEN HE EXPERIENCED DISCOMFORT IN HIS LOWER BACK. *FIRST DAY MISSED 5/3/00 .
EE WAS REPLACING A VALUE IN THR REFINERY. WRENCH SLIPPED OFF OF THE BOLT AND STRUCK EE IN THE FACE AND CAUSED A LACERATION ABOVE THEN UPPER LIP. LACERATION REQUIRED 3 THRU STITCHES.
THE EE WAS ATTEMPTING TO REMOVE THE DRAIN PLUG FROM THE FINAL DRIVE OF A CATERPILLER 773B HAUL TRUCK. THE WRENCH SLIPPED CAUSING THE EE FINGER TO COME IN CONTACT WITH THE EDGE OF THE DRAIN TRA YTHE EE WAS USING. THE EDGE CUT THE BACK OF THISLEFT MIDDLE FINGER. THE EE DID NOT SEEK MED ATTENTION UNTIL 9PM THAT EVENING.
EE WAS REMOVING A PIN FROM A BUSHING WITH A PUNCH. THE HAMMER STRUCK THE REAR OF THE PUNCH. THE PUNCH STRUCK THE EE'S SAFETY GLASES, A PIECE OF GLASS LACERATED THE CORNEA OF THE LEFT EYE. THE EE DID NOT STRIKE THE PUNCH WORK WAS BEING CONDUCTED AT EYE LEVEL. SAFETY GLASSES DID NOT SHATTER BUT A PIECE SIZE OF A DIME BROKE OFF THE INSIDE.
EMPLOYEE WAS USING A HAND POST DRIVER TO DRIVE A COPPER GROUNDING ROD INTO GROUND. POST DRIVER WAS LIFTED ABOVE THE ROD DURING THE UPSTROKE AND CAUGHT THE EMPLOYEE'S RIGHT THUMB BETWEEN THE TO P OF THE GROUND ROD AND THE HANDLE OF THE POST DRIVER ON THE DOWN STROKE.
EMPLOYEE WAS CUTTING A 2" U BOLT WITH A HAND OPERATED HACKSAW WHEN THE BLADE BROKE CAUSING A LACERATION OF THE EMPLOYEES LEFT THUMB REQUIRING 3 STITCHES.
EMPL WAS SETTING 4'X5'X3/4" PLATE AS RETAINING WALL WHEN PLATE CLAMP SLIPPED ALLOWING SHEET TO STRIKE EMPL IN UPPER THIGH CAUSING STRESS FRACTURE TO LOWE LEG FIBULA.
EMPLOYEE WAS WORKING ON 10 INCH 35 LB KNIFE VALVE ON 3 FOOT BENCH HE TURNED TO VALVE OVER AND THE KNIFE FELL STRIKING HIM ON THE RT FOOT BEHIND HIS STEEL TOE.
EMPLOYEE WAS REMOVING A SHOVEL TOOTH FROM A P & H 2800 POWER SHOVEL. THE TOOTH WAS BOUND, THE EMPLOYEE TRIED TO FREE IT WITH A SLEDGE HAMMER RESULTING IN A PIECE COMMING OFF THE TOOTH AND STRI KING HIM IN THE KNEE.
