Pry bar fell and pushed EE's hand against the chute
Employee was using utility knife to cut fitting off of hose when cut palm of hand.
A frozen chunk had plugged up a conveyor on the crusher. The employee used a pinch bar in an attempt to reposition the chunk. The hook at the end of the pinch bar slipped off of the frozen chunk which caused the straight end of the bar to hit the employee in the mouth, chipping two teeth.
EE was cutting a piece of hose with a utility knife when he cut his gloved thumb. Three stitches were needed.
Employee was changing screens using hand tools, ratchet and socket.
Employee was in an awkward position, twisting and reaching between tires of portable screen plant to remove guard with a wrench in order to adjust belt tracking. He suffered a lumbar strain from this activity.
Sunny, mid 60's, EE was cutting a 5/5" belt, right hand holding a straight edge and left hand using a utility knife. The knife slipped, cutting the top side of the right hand which required 2 stitches to close. No medications or restrictions given and EE returned back to work that day.
Employee & supervisor were attempting to remove nipple of water valve on water truck. Employee using a pipe wrench on nipple while supervisor was heating 3" 90% cast iron pipe elbow. The wrench slipped and employee fell backwards while twisting to break fall. Twisting of left lower leg resulting in fracture. The ground was level with no slip/trip/fall hazards present.
Employee was using a rachet extension to drive wedges out of screen rails and a piece of steel chipped off and struck employee in the neck creating a puncture wound.
Employee was removing jack from under equipment when it shot out and struck forearm

WHILE REACHING IN DESK, A THUMB TACK STRUCK HIS FINGER CAUSING HIM TO PULL BACK, PUNCTURING HIS LEFT THUMB WITH A KNIFE THAT WAS IN HIS RIGHT HAND.
WHILE CHANGING THE TEETH ON THE 980 LOADER BUCKET, THE HAMMER BEING USED SLIPPED AND HIT THE EE'S FINGER.
WHILE TRYING TO BREAK A FLAT ROCK UNDER THE JAW WITH A HAMMER, EE'S FINGER HIT THE TENSION ARM OF THE CRUSHER.
