Employee and a Contract employee was removing a sheave from the motor shaft by hitting the sheave with a rawhide hammer, Employee went to rotate the sheave and the Contract employee hit the sheave at the same time hitting Employee's right middle finger.

Was cutting up a Tie down strap that was frayed. The knife cut through his shirt and cut his right upper abdomen. Took him to the hospital and he needed stitches.

EE WAS ADJUSTING A CONE CRUSHER WHEN THE HEAD OF A RAM WAS BROKEN OFF AND IT FELL DOWN BETWEEN THE CONE AND GRATING. IT STRUCK A DRIVE BELT AND CAME BACK THROUGH THE HOLE AND STRUCK EE IN THE FOREHEAD.

EE WAS LIGHTLY TAPPING A PIN INTO A LARGER CHAIN LINK. APPARENTLY A PIECE OF METAL CAME OFF OF THE PIN ON HAMMER AND STRUCK HIM IN THE NOSE, VERY SMALL OBJECT.
