Employee and a Contract employee was removing a sheave from the motor shaft by hitting the sheave with a rawhide hammer, Employee went to rotate the sheave and the Contract employee hit the sheave at the same time hitting Employee's right middle finger.

The injured miner moved to get a better position. The second miner, using the hammer, the hammer glanced off of the frame and stuck the second miner on the right knee cap.

EE was positioning steel with a spud wrench to weld steel into place. He pulled down on wrench abnormally, causing shoulder pain.

Was cutting up a Tie down strap that was frayed. The knife cut through his shirt and cut his right upper abdomen. Took him to the hospital and he needed stitches.

Prying on grating when he hyperextended his left knee. Employee was placed on restricted duty.

EE WAS ADJUSTING A CONE CRUSHER WHEN THE HEAD OF A RAM WAS BROKEN OFF AND IT FELL DOWN BETWEEN THE CONE AND GRATING. IT STRUCK A DRIVE BELT AND CAME BACK THROUGH THE HOLE AND STRUCK EE IN THE FOREHEAD.

Employee was prying on a hose clamp with a spud wrench. It slipped and struck him in the mouth.

EE WAS LIGHTLY TAPPING A PIN INTO A LARGER CHAIN LINK. APPARENTLY A PIECE OF METAL CAME OFF OF THE PIN ON HAMMER AND STRUCK HIM IN THE NOSE, VERY SMALL OBJECT.

TIGHTENING WATER FITTING IN VICE WHEN SOMETHING IN CHEST POPPED. NO INSTANT PAIN, GOT WORSE OVRNIGHT.
