Employee was working down by ball mill 6 and 7 in the cyclone feed pump build up area. Employee was using the bolt for a hammer and smashed a finger.
IP was changing a light on a haul truck and lost control of wrench, which flew back and lacerated their lip. Supervisor called over the emergency line at 12:38pm and arrived at Lark Gate to see EMT at 13:00. EMT cleaned the laceration and sent them to a clinic to get the laceration stitched up.
Craft A was turning a half inch ratchet while Craft B was holding the torque multiplier using an extended handle and wrench. Craft B's hand slipped, and the craft's middle finger was pinched between the wrench and the wall of the tank.

Employee cut two fingers while cutting a zip tie off of a hydraulic motor on a drill with a utility knife. Employee received 14 sutures in a medical facility.
Tightening a fitting in a small space under truck, only could turn fitting 1/4 turn at time, didn't get wrench all the way on and it slipped hitting EE in the tooth chipping the tooth.
Employee removed cut resistant gloves to apply a clean label to a wire. Employee states they forgot to put gloves back on before moving to next task. While using a linemen's tool to score the outer jacket of the new cable the linemen's tool slipped and caught the tip of the left middle finger by the fingernail causing a laceration.
Employee was stripping a large cable with a razor blade. While stripping the cable, the razor slipped and struck EE's left hand resulting in a minor injury requiring two sutures. A commercial stripper was onsite but was missing a blade so the employee decided to use the razor blade instead and didn't exercise EE's stop work authority.
A mechanic was using a Porta Power to push a bearing on a drive pulley to achieve the final alignment. A bolt was used to extend the length of the porta power cylinder. The bolt ejected during the process and struck the mechanic in the chin.
Employee eating lunch and cutting avocado slipped and cut palm of hand requiring 10 sutures returned to work.
While removing a tooth from an excavator bucket craftsperson used a 3lb hammer to hit the tooth after the third strike EE felt a sting to EE's left forearm and a blood spot appeared. at the hospital a metal shard was removed and EE's forearm received 5 stitches
Employee smashed pinky finger of right hand between pry bar and steel structure holding the donut inlet tab above ball mill feed.
Employee was tightening bussing from transformer to gear. Wrench slipped off striking #8 tooth. Employee went to see dentist. Dentist wanted to wait to see if pain subsided. No prescription meds, root & cap placed on Friday Dec. 5th became reportable.
Craftsman was working to replace a wear plate on a shovel dipper door when the cable supporting the plate caught on the door creating stored energy in the system. The craftsman tried to free the plate with a pry bar and as the cable suddenly released; the craftsman sustained a crushing injury to his middle and ring finger(s), by contact with the wear plate and dipper door.
AN EMPLOYEE WAS REPOSITIONING A GET (GROUND ENGAGING TOOL) TOOTH IN A SERVICE TRUCK WHEN A SECOND TOOTH SHIFTED AND PINCHED THE EMPLOYEES FINGER BETWEEN THE TEETH.
A worker was struck on his lower lip by the pry bar tool he was using to release a lever binder. The impact caused a 1/2" laceration to his lower lip and the loss of one of his lower incisor teeth. He reported that the release of energy was more powerful than he had anticipated and he could not keep the bar from striking his face.
Employee was helping to strip coating from electrical cable when his index finger contacted the blade of the stripping tool resulting in a laceration injury.
Employee was driving a stake to support concrete forms. The hammer glanced off from the stake and struck the employee on the arch of his left foot. Employee was transported to a clinic for evaluation. X-rays indicate a non-displaced fracture if the first metatarsal bone. Employee was placed in a walking immobilizer/support boot and released to return to work with restrictions.
Improper tool was used for the task at hand while stripping a cable to be installed.
Employee was pulling down on a pry bar. The pry bar slipped and hit him in the right cheek resulting in a laceration that required three sutures to close.

Employee was breaking pipe joint. He was holding the hook end of a 36 inch steel pipe wrench. The pipe wrench slipped out of employee's hand causing a laceration to palm of left hand.
Employee was using a personal pocket knife to cut nylon rope, when the knife slipped contacting his left middle finger resulting in a laceration injury requiring four(4) sutures.
Employee was attempting to remove a lip shroud from a front-end loader using a sledge hammer. The hammer, upon contact with the metal shroud, generated a small metal shard which came off striking and imbedding itself in his left leg. The shard was sugically removed from the leg.
Employee was helping to replace a metal shroud (on a shovel dipper) when he was struck on his left arm by a metal shard. He was standing approximately 15ft from a co-worker who had hit the shroud with a metal sledge hammer in the attempt to loosen dirt/debris from the shroud.
Wrench struck employee in mouth and fractured tooth.
Employee was working to replace a tempature sensor with a screwdriver when the tool slipped causing his hand to strike a sharp edge resulting in a laceration injury to his left index finger requiring sutures.
Worker was pounding a ground rod into the ground for electrical distribution grounding. A ground rod slide hammer was being used. Worker lifted the outside sleeve further than anticipated and it became detached. With driving force he pushed down hard to drive the rod - when the two pieces came together it resulted in a laceration of the thumb and outer middle finger.
The EE was in the process of removing a tail pulley bearing race. He was unsuccessful in removing the race by grinding it down, so he struck it with a hammer. A fragment shot off and struck him in the left leg. The EE and supervisor inspected his leg and found no injury. On 1/28/07 EE felt discomfort in his leg, went to instacare, X-rays showed metal fragment and it was removed.
Stripping the end of large wire with a uyility knife
No impairment. Employee placed wrench on lower ring pin bolt of a safety valve, applied pressure when bolt lossened, slipped forward smashing the tip of the right ring finger against valve body.
Employee was attempting to cut a rubber hose on a brake cooling line (using a utility knife) when he inadvertently contacted his left thumb with the knife blade, lacerating his thumb. Laceration required three(3) sutures.

Employee a laborer with 31\2 yrs experience, 5 mos. on this project was trying to drive a 1" diameter steel form stake into the ground by hitting it with a 21/2lb hammer. As he swung the hammer, he missed, striking his finger against the stake. This caused a laceration to his right index finger resulting in 15 stitches.
Employee was changing liners in a ball mill feed cart using a pry bar. The panel slipped into position and the employee smashed the end of his left ring finger between the pry bar and another liner panel.
Incident occurred at the Utah Power Plant where the employee was slagging the #2 Boiler. Employee had improper hand placement on the slagging hook and pinched his left ring fingertip between bar and frame.
Employee was loosening nut on foot clamp with pipe wrench when left middle finger was caught between wrench and wrench holder.

The stake slipped causing the form to strike and slide down front of employee's left leg (shin) causing contusion with no broken skin. On 5/13/06 the wound became infected and treatment for infection began.
Employee was attempting to cut barricade tape using a fixed blade knife when he contacted the distal region of his left index finger causing a laceration injury. Laceration injury required seven(7) sutures.
EE WAS USING A PUNCH BAR & PINCHED HIS LEFT RING FINGER BETWEEN PUNCH BAR & THE TIRE RIM. SUTURES WERE REQUIRED FOR CLOSURE
THE EMPLOYEE WAS USING A SHORT PUNCH OUT BAR & HAMMER TO COMPLETE KNOCKING OUT A BALL MILL LINER BOLT. WHEN HE STRUCK THE PUNCH OUT BAR, A SMALL PIECE OF METAL FRAGMENT LODGED IN HIS RT. FOREARM.
EE WAS USING A RAZOR KNIFE TO CUT ROPE FROM CABLE. THE ROPE WAS FROZEN SO THE EE APPLIED FORCE. AS THE KNIFE CUT THROUGH THE ROPE, IT STRUCK HIM ON THE RIGHT SHIN. THIS RESULTED IN A SMALL LACERATION TO HIS RIGHT SHIN. SUTURES (2) WERE REQUIRED FOR CLOSURE.
EMPLOYEE WAS USING A WRENCH TO REMOVE A NUT FROM A BOLT. THE WRENCH SLIPPED OFF THE NUT & CAUGHT HIS FINGER BETWEEN WRENCH & METAL PLATE.
EE WAS CUTTING 4" PLASTIC HOSE WITH A POCKET KNIFE. THE KNIFE PROTRUDED THROUGH THE HOSE WALL AND EMBEDDED INT HE EE'S LEFT ARM.
EMPLOYEE WAS USING A WRENCH TO REMOVE AN ENGINE OIL FILTER. THE WRENCH SLIPPED OFF OF BOLT HEAD AND THUMB STRUCK SHARP EDGE OF FILTER.
EE WAS STRUCK IN THE WRIST BY A SLING THAT HE MOVED OUT OF THE WAY.
EE WAS USING A WRENCH TO REMOVE DRAIN PLUG, HIS HAND SLIPPED FROM THE WRENCH AND STRUCK A BRAKE ASSEMBLY STUD.
