EE was removing a loader bucket pin when it slipped and fell on their finger.
EE was removing right boot using a boot jack. As they pulled their leg up, they felt low back and hip pain.
While pulling an HQ 5' Innertube with a rope sling, the rope broke allowing the tube to fall and catch IE's hand, wedging IE's right thumb between the Innertube and an HQ drill rod crushing and cutting the right thumb.

On 6/2/2022 an underground miner was separating two air hose wing nut fittings using a rap wrench. EE struck left ring finger resulting a laceration requiring sutures.
EE was trying to close a compressed air valve with a 12' scaling bar. The employee lost grip on the bar and it slipped out of employee's hands, striking employee in the face causing a fractured tooth.
THE CREW HAD COMPLETED A CORE TUBE RETREIVAL AND WAS PREPARING TO REMOVE THE STUFFING BOX AND OVERSHOT. THE DRILLER'S ASSISTANT BEGAN UNTHREADING THE STUFFING BOX WITH A PIPEWRENCH AT WHICH TIME THE STUFFING BOX, OVERSHOT AND TUBE FELL ON TO THE ASSISTANT'S RIGHT FOOT FROM A HEIGHT APPROXIMATED AT 4 FEET.
Pre-Engineered Building was being erected. The employee was performing a rafter to column connection. Employee was twisting to bolt up using a spud wrench. Later ee noticed a pain in right shoulder.
Employee cut left thumb while cutting crab leg prior to going to work.
Employee was cutting HDPE pipe with a hacksaw and cut their finger.
EE was using a 4 foot long pry bar to pry a ring from a rim when the bar came free resulting in the pry bar striking the EE left palm.
Employee's hand contacted frame of equipment while swinging a hammer in an attempt to drive a hydraulic cylinder pin.
Employee was tightening a fitting when the wrench slipped off causing pain in left arm and shoulder.
Employee was using a 20# sledge hammer to straighten a step on mobile equipment and smashed finger between handle and track of equipment
Employee was using a hammer to remove a jaw block from a bolter. The hammer chipped off a small piece of steel from the jaw, hitting the employee in the tooth.
Employee received a self inflicted laceration to the left forearm as he was using a utility knife to trim away old gasket material from a window on mobile equipment.
EE was using a hummer to break legs of the old table. Hummer bounced back and hit EE's right shin causing a deep i inch wide laceration.
Miner was attempting to trim plastic cone with personal utility knife held in his right hand when the knife slipped. This resulted in a laceration to the first finger of the right hand.
Miner was attempting to dislodge a jammed split set from the bolter using a scaling bar. When the split set popped up and out, it struck the miner on the left side of his chest resulting in a contusion.
Employee felt pain in his right arm after lifting man hole covers and using pipe wrenches. No specific mechanism of injury is identified. Employee completed his work rotation and on his days off, saw his personal physician. Contacted employer on 7/25/2011 with a release from work note from physician secondary to right shoulder strain.
Employee was transporting HDPE pipe on a Boom Truck. He stopped to re-secure the load which was tied down with a chain binder. As he was retightening the load the cheater bar slipped off the chain binder handle and he was struck in the face resulting in a fractured mandible (rt side)
Mill Mechanic was lying down on a concrete floor and reaching through an opening while pulling on an 18" pipe wrench with his right arm. He was trying to break loose a rusted pipe fitting on the bottom of the air receiver tank. Between the awkward position and the force required to break loose the fitting he suffered an injury to his shoulder.
Intern was cutting small roots in the ground for soil sampling at a remote exploration site using a small 4-inch handsaw with both hands. The saw blade broke on a forward stroke and his momentum caused his left hand to strike the broken blade. He was wearing gloves at the time and received 4 sutures to the cut.
Employee was helping install guard rail posts when another employee was using a 4 lb hammer. The hammer slipped from his hand and hit the employee in the mouth. Note: The employee did not report this incident when it happened. He finally reported the incident to the company on 12/21/2010. He is to be seen by a dentist and has lost two teeth and two others are cracked.
Employee was removing the bands from a bundle of bolts, as he cut the bands the band sprung striking him on the left hand causing a laceration to the palm. The laceration required sutures.
While loading 18' lengths of 1 rebar for snow fence posts. A length of rebar slipped off the rack pinching the meaty palm side end of the ring finger between the rebar and the bed of the pickup. Resulting injury was deep 15mm laceration to tip of (R) ring finger on the palmer side.
In the process of tightening up a bolt, the wrench slipped causing employee to jam his little finger resulting in a fracture to the tip. The bolts were metric and the mechanic was using a standard combination wrench causing the wrench to slip
Individual was cleaning, repairing and demobilizing equipment on the 860 Helipad due to 2008 Exploration season. Individual was assisting in the repair of a horn button located on the dash of tractor. Using a non-locking blade knife proceeded to restrain the outer case of the button while it was being repaired. Knife slipped and cut right index finger.
Employee was using hammer to knock a pin out of drip pan cylinder while laying down on his back. Upon impact, right hand was struck between frame and hammer handle injuring the index finger. Employee was wearing gloves at the time.
Mounting tire and had left hand on the hand tool to keep it from slipping. As it came around his hand was between the tool and the tires valve stem. This caused a cut on his hand requiring 11 sutures.
Geologist using machete to clear brush about 40? uphill on a steep slope, in order to step forward up the hill, Geo stuck the machete in the ground freeing his hands to grasp onto trees and assist him up the hill. He began to step forward and up the hill; right knee contacted machete, resulting in a ~1" long laceration on the outside of right knee.
Underground mechanic was closing the left step assembly on a Bolter using hammer to get assembly closed to put closure pin in place. He hit his right thumb and was taken to the medical department, and then to Juneau Urgent Care, where he received seven sutures.
Miner was repairing a 2? discharge hose using a box cutter to cut off a damaged end prior to installing a new fitting and clamps. While cutting the hose with his right hand by drawing the box cutter towards him it slipped and sliced the knuckle of his left index finder creating an approximate one inch crescent slice requiring 6 stitches.
Employee was prying on a victualic coupling with a wrench in order to line it up so it could be tightened on the pipe. The wrench slipped, rotated around and struck the employee in the mouth.
HE WAS PRYING UP ON SKID WITH A BAR SO HE COULD GET A BLOCK UNDER THE SKID. HE FELT AND HEARD A POP IN HIS RIGHT SHOULDER. UNABLE TO WORK (PER DOCTOR) ON 6-15-05.
Employee was changing out a hydraulic hose on a loader. Hose was held in place with zip ties. Employee started cutting ties with box cutter knife, pulling blade towards him. Knife blade contacted back of other hand resulting in deep cut.
WORKER WAS SHARPENING CHAIN SAW WITH HAND HELD ROUND FILE. THE FILE SLIPPED OFF TOOTH WHILE WORKER WAS PUSHING ON FILE, CAUSING PALM OF HAND TO CONTACT SHARP TOOTH ON CHAIN.
WORKER WAS CONNECTING A HOSE TO A JUMBO DRILL USING A SNAPLOCK VICTAULIC COUPLING. THE COUPLING WAS DIFFICULT TO SNAP CLOSED SO WORKER STRUCK THE SNAP HANDLE WITH A WRENCH. THE COUPLER CLOSE D PINCHING HIS LEFT MIDDLE FINGER.
WORKER WAS REMOVING LARGE ROOF BOLTS FROM A CAT TRACK USING A LARGE WRENCH & BAR. AT THE END OF SHIFT HE FELT SHARP PAIN IN HIS BACK.
EE PLACED UTILITY KNIFE IN POCKET. BLADE WAS EXTENDED 3/4" BLADE POKED THRU BUCKET MATERIAL. LATER EE WIPED HAND ON CONVERALLS AND HAND CONTACTED KNIFE BLADE.
WHILE LIGHTENING A BOLT ON THE WIRELINER STINGER THE WRENCH SLIPPED OFF THE BOLT HITTING EE IN THE LIP.
WORKING ON REMOVING FEED VALVE FROM FILTER PRESS. PRYING UP ON COMBINATION WRENCH TO REMOVE BOLT- BOX END OF WRENCH SLIPPED OFF BOLT AND OPEN END OF WRENCH STRUCK MOUTH WHEN HANDS FLEW UP TOWA RDS FACE.
WAS REPAIRING JUMBO DRILL STEEL CHUCK TENDER. WHILE ATTEMPTING TO PRY IT OPEN, IT OPENED SUDDENLY UNDER PRESSURE AND PINCHED FINGER AGAINST FRAME.
WORKER WAS LOWERING A PUMP INTO A SUMP WITH A SMALL HAND OPERATED WINCH. HIS HAND SLIPPED AND WINCH BEGAN TO FREE-WHEEL DOWN . HE REACHED OUT TO GRAB THE WINCH HANDLE AND MISSED. HANDLE CAME A ROUND STRIKING HAND HARD.
WORKER WAS CLEANING LIFTERS OR A DRILL FACE WITHA FIN HOE AND FELT PAIN IN LOWER BACK.
