Shop mechanic was removing a u-joint from a 980 loader when the wrench slipped and hit EE in the mouth. The bottom half of EE's front tooth was broken off. EE received orthodontia work recently making it a reportable incident.

EE WAS REMOVING METAL BANDING ON HAND RAIL OF TUNNEL CONVEYOR WITH A CRESENT WRENCH. THE WRENCH SLIPPED AND THE BANDING CUT HIS LEFT HAND INDEX FINGER REQUIRING 4 STITCHES AND A TETNUS SHOT. A PROPER CUTTING TOOL AND GLOVES WOULD HAVE PREVENTED THE INJURY.
