Hammering on a block, the hammer skipped on the edge and ended up hitting the employee's right hand/wrist. Employee did not seek medical attention until 01/20/2025. With this appointment employee was placed on restrictions of 10lbs push/pull, 10lbs carry/lift with right wrist. Employee came back to work after appointment with brace on wrist.
Employee strained right shoulder while operating a come-a-long in an awkward position. Diagnostic treatment until rotator cuff surgery performed on June 22, 2023.
Was tightening bolts when one of the wrenches EE was using slipped off the bolt. The bolts were at eye level and the employee was standing while conducting this work. EE was using an adjustable spud wrench and a box end wrench to tighten bolts. Due to the force and angle, the spud wrench slipped and struck them under right eye.
While rebricking the kiln, a pogo slipped off the brick it was holding causing the brick to fall on the employee's finger.
EE was hitting a stiffener ring on a duct with a 16 pound sledge hammer. The hammer glanced off the ring pinching EE's finger between the ring and the hammer handle. EE was wearing gloves.
Trimming off black expansion material with utility knife and using the knife in an improper manner cutting towards self. Cut through the glove causing a laceration on EE's left hand.
EE was operating a chain hoist while looking up when dust fell from the hoist. Some of the falling dust got around EE's safety glasses and entered EE's right eye causing irritation.
Employee was tightening bolts on expansion joint at the chiller pad. Ratchet slipped and struck employee on EE's right cheek.
Employee was swinging a sledgehammer, when finger was caught between the handle and the manhead pin, resulting in broken finger. Physician prescribed restricted duty.
Employee was swinging sledgehammer to dislodge manhead for Ball Mill, sledgehammer bounced off of manhead. As Employee sought to control the hammer and keep it from falling, they strained a shoulder. Employee did not feel medical attention was necessary until a week later, as discomfort continued. Physician prescribed lifting restrictions and physical therapy.
EE was using heavy pry bar to clean material from clay shredder, when strain occurred. EE did not feel immediately necessary to be evaluated medically. approximately 1 week later, physician diagnosed as thrombosed external hemorrhoid. Procedure performed to drain, employee prescribed restricted duty.
Employee was chipping slag with a jitter bug. a piece of slag shot up and burned the area between the bridge of the nose and the left eye.
EE was driving shim between refractory bricks, when ee struck thumb with the hammer. Physician treated thumbnail and prescribed antibiotic/anti-inflammatory medication.

Physician prescribed anti-inflammatory medication but no restrictions. **EE underwent surgery for continued discomfort on 11/11/10. One lost workday and 60 restricted workdays resulted from this surgery.
