Mostrando los 6Employee was assisting the maint. crew while changing blowbars in an HSI impact crusher. EE was using a prybar to move"scoot" the blowbars into the bucket of a skidsteer. The prybar slipped allowing his right hand to slide down the bar and between two blowbars. The blowbars fell together pinching the tip of his right ring finger.
Employee was using a 2 lb sledge and steel bar to position wedge on steadman crusher when he missed bar and struck his left hand with 2 lb sledge.
USING PICK, HE WAS REMOVING A ROCK FROM THE BOTTOM OF THE PRIMARY CRUSHER.
EE WAS REPAIRING THE DRILL WHEN HIS WRENCH SLIPPED CAUSING HIM TO HIT HIS LEFT RING FINGER ON THE DRILL. NO LOST TIME NO RESTRICTED DUTY.
EMPLOYEE WAS USING A CHIESL IN AN ATTEMPT TO PRY OPEN A STEEL COLLAR. WHEN HE STRUCK IT WITH A HAMMER, THE CHISEL FLEW UP AND STRUCK HIM. HE WAS WEARING GLASSES. HOWEVER A SMALL PIECE OF GLASS CHIPPED OFF AND LODGED IN HIS EYE. HE MISSED ONE DAY OF WORK AND RETURNED WITHOUT RESTRICTIONS.
