A mechanic was attempting to remove a zip tie on a plug while using a utility knife when it made contact with their left middle finger, lacerating it.

Employee was replacing GET bucket teeth, one tooth retainer would not move. Employee cut off that tooth retainer, then struck the tooth with a 3 pound hammer. A piece of metal came off the tooth and struck them in the forearm.

EE was dumping barrel with barrel dumping device when chain came off dumping mechanism pulley, causing the EE to pull with no resistance suddenly causing pain in left shoulder, back between shoulders.

Employee was attempting to remove a water filter housing using a strap wrench at our water treatment plant. The employee was pulling on handle of strap wrench when it slipped hitting them in the mouth loosening a front tooth #8.

EE was cutting thick rubber with a knife using force and slipped cutting left wrist sutures required.

On 10/25/11 EE was using a pipe wrench to break a joint during a trip. His right hand was gripping around the wrench that was backed up against the tower. When the rotation was engaged to break the joint the wrench jaws slipped and the handle of the wrench slipped forward and pinched the edge of EE's right index finger between the pipe wrench and the drill mast.

During relining the ball mill a fellow employee was hammering a liner bolt in, injured employee was standing off to the side and behind him when a small piece of metal came off the hammer and struck him in the neck.

Employee was inserting a hoist pin, swing with a sledge hammer, felt twinge in back.

Replacing lip shroud on shovel, using hammer to remove bolt, was struck by flying metal.

Employee was removing a hub from a shaft when a hub puller bolt broke and the arm of the puller hit the employee in the chin, requiring stitches

Employee got hand smashed between jack stand and the frame of a pickup

Was scaling rock box in crusher and got dirt in eye

Baring tire down to install lock ring, bar slipped off wheel and hit right cheek.

The mine guys strapped a motor with the wire part of the come-along around the rafter and as they moved the motor the wire kinked and broke dropping the motor; it fell and hit the side of my body and bounced landing on my hand on the railing of the crane smashing my hand and removing two fingers on my right hand.

WORKING AN STEMMING HOLES. EE BENT OVR TO SHOVEL AND FELT HIS BACK GIVE UP.

RIGGING CAME APART THAT WAS SUPPORTING 100 TON HYDRAULIC PRESS & 3 TONE CHAIN COMEALONG. WHEN RELEASED A 3" PIPE STRUCK EE'S HARD HAT & HEAD CAUSIG A LACERATION

THE EMPLOYEE STATED THAT HE WAS UNHOOKING A WELDER OFF OF THE BACK OF A PICKUP WITH A HANDY MAN ACK. THE EMPLOYEE STATED THAT WHEN HE WAS LOWERING THE TONGUE WITH THE JACK, IT (THE HANDLE) SP RANG P AND HIT HIM IN THE MOUTH. THE EMPLOYEE WAS TAKEN TO THE WESTERN MONTANA CLINIC IN CHALLIS WEHRE X-RAYS WERE TAKEN OF HIS JAW. THE X-RAYS WERE NEGATIVE BUT THE EMPLOEYE HAD TO GO TO TH

THE EE STATED THAT HE WAS RAISING THE HEADER LINE AT THE TAILINGS DAM USING A HYDRAULIC PORTA POWER WHEN A HYDRAULIC HOSE BROKE LOOSE SPRAYING OIL IN HIS LEFT EYE.

THE EE STATED THAT HE WAS TIGHTENING A LOWER BALL STUD ON HAUL TRUCK T-53. THE EE STATED THAT AS HE WAS TIGHTENING THE BALL STUD, HE FELT HIS RIGHT SHOULDER POP. THE EE REQUIRED SURGERY ON HIS SHOULDER 12/26/96.

THE EMPLOYEE STATED THAT HE WAS INSTALLING ATOOTH ADAPTER ONT EH SHOVEL BUCKE.T AS HE WAS POUNDIND ON THE END OF THE ADAPTER WITH A HAMMER, A PIECE OF METAL CHIPPED OFF AND HIT HIM IN THE FO REHEAD (LEFT OF CENTER).

EMPLOYEE STATED THAT HE WAS USING A COME-A-LONG TO DROP THE BELLY PAN ON A DOZER WHEN THE RATCHET HANDLE ON THE COME-A-LONG SLIPPED HITTING HM IN THE LOWER RIGHT SIDE OF HIS LIP. ALSO CHIPPIN G HIS FRONT TOOTH.

THE EMPLOYEE STATED THAT HE WAS INSTALLING A TOOTH ADAPTER ON THE BUCKET OF A DART LOADER WHEN THE ADAPTER JAMMED. HE WAS STRIKING THE ENDOF THE ADAPTER WITH A HAMMER WHEN A PIECE OF STE EL BROKE OFF FROM THE HAMMER LACERATING HIS THIGH OF RIGHT LEG. THE EMPLOYEE WAS TREATED AT THE LOCAL MEDICAL FACILITY FOR LACERATION, IMBEDDED METAL LEFT IN THIGH. HE WAS RELEASED FOR LIGHT

EMPLOYEE STATED THAT HE WAS CUTTING A ONE AND A HALF INCH RUBBER HOSE WITH A TILITY KNIFE WHEN THE KNIFE SLIPPED RESULTING IN A CUT FINGER. EMPLOYEE WAS TAKEN FOR MEDICAL TREATMENT. HE RECEI VED FIVE STICHES AND WAS RELEASED TO COME BACK TO WORK THE SAME DAY.

EE WAS USING A HANDYMAN JACK TO CHANGE A PICKUP TIRE. THE JACK BECAME STUCK WHILE JACKING THE PICKUP DOWN SO THE EE WAS SPRAYING THE JACK WITH WD-40 WITH ONE HAND WHILE THE OTHER HAND WAS ON T HE JACK. THE JACK LET GO CAUSING THE HANDLE TO STRIKE THE EE IN RT SIDE OF HIS JAW.
