Was Changing support rollers on 57101 Clinker Pan conveyor. Was using standard steel 3lb sledgehammer to drive the rollers into the bracket on the pan. The hammer glanced off the end of the roller and struck the heat-treated flange of the roller causing a shard of steel to break off and lodge in leg. The employee was taken to clinic and released back to work with no restrictions.

Miner was cleaning the tower using a bar and a sledgehammer. Miner hit the bar with a hammer; a piece of metal broke off and flew into the miner's eye.

Miner cut their hand and required stiches.

EE was trying to clear out a port on the tower with a bar. EE hit the material twice and on the third time the material fell from the wall of the riser, the momentum of them pushing caused their finger to get pinched between the tab on the bar and the port on the riser.

Miner was using a bar to clean buildup inside of preheat tower. A large chunk of material broke free hitting the edge of the bar causing the end that the miner was holding to lurch up pinching EE's index finger against the metal frame of the tower.

Swinging sledge hammer to open slide gate.

Team was attempting to seat the elevator take up sprockets back into the chain. The take up was stuck and the employee attempted to free it by striking it with a sledgehammer which accidently struck the hardened tooth and a piece broke free penetrating their left thigh. Employee was taken to Concentra and redirected to Urgent Care.

Employee was using a box knife for non-task related reasons. As employee was trying to cut away a zipper out of jacket with the box knife, it slipped and cut finger.

While work on #2 Coal Mill using chain hoist to pull motors back in place to tighten belts EE picked up the chain hoist and pulled on it. The chain flew toward EE's right foot and the chain hook struck right behind the steal toe and injured EE's foot little toe.

While cleaning probe, employee tried to hold the probe in position but other employee hit injured employee's ring finger with the sledge hammer.

Employee was using a digging bar to clear a chute that was plugged. His middle finger was caught between the digging bar and a piece of metal part of the chute and caused my finger to break open in two places.

I went to cut back the North side skirting with the scissors and was not able to do it. I needed my left hand to cut the skirting. The pain in my right wrist was too bad.

This is a re-occurance of a previous injury. Employee had been released to return to full duty. Employee was tightening a bolt with a wrench and felt a sharp pain in his left elbow. When he went to lift the blade, he felt a severe sharp pain in the left elbow. Employee has been on restricted duty since July 3, 2015, but has been at work doing light duty.

Employee sprained his left wrist while using a sledge hammer for a starvation on #22 gyp feeder. He was put on restrictions 10/6/14.

Employee was painting handrails. Shortly after painting pain began and has worsened in the last two weeks.

EE was loosening bolts using a 15/16 socket and breaker bar. Employee pulled in his shoulder trying to break loose some bolts. Date of Accident: 2/19/11 Employee went to the doctor on 2/21/11 and he did not indicate that was a work related injury Employee's supervisor notified on 2/22/11 Employee went to our worker's comp clinic on 2/23/11 for evaluation.

Ee was cutting a rubber hose with a utility knife when the blade slipped, cutting his hand. Injury required sutures.

Employee was moving the lids of a screw conveyor into place with a pry bar. After relocating several of these, the employee experienced pain in his right hip. Late that evening, he made a visit to the local hospital where he was told to rest. He is awaiting a complete diagnosis and has missed 4 days of scheduled work as of 6/10/2008.

Employee struck elbow on the concrete.

Employee was striking a chute to dislodge stuck material. The material (kiln dust) released suddenly and made contact with employee?s right leg. The hot dust caused first and second degree burns to his right ankle and lower right calf.

The employee was connecting iron and struck the knee on a steel beam and then again on a spud wrench.

WHILE PERFORMING MAINTENANCE ON A CRUSHER, EMPLOYEE WAS USING A 4'PRY BAR TO ROTATE THE CRUSHER HAMMER. AS THE HAMMER TURNED OVER, EMPLOYEE'S HAND WAS CAUGHT BETWEEN THE PRY BAR AND A CRANE HOOK.

EMPLOYEE WAS STANDING ON THE GROUND NEXT TO AN ELEVATED WORK PLATFORM. A SECOND EMPLOYEE WORKING ON THE PLATFORM WAS USING A HAMMER TO AID A HYDRAULIC PRESS IN MOVING A LARGE COAL MILL PLATE. THE SECOND EMPLOYEE SWUNG THE HAMMER, WHICH GLANCED OFF OF THE PLATE AND HIT THE INJURED EMPLOYEE ON HIS HARDHAT.INITIALLY NOT REPORTABLE.RECENTLY EMPLOYEE RECEIVED MEDICAL TREATMENT FOR NECK.

EE WAS PERFORMING AN INSPECTION IN THE COAL RECLAIM A BELT CONVEYOR AREA. A PIECE OF COAL WAS LODGED IN BETWEEN THE CONVEYOR BELT AND DRIVE-OVER HOPPER SKIRTING. EE WAS HITTING THE UNDER SIDE OF THE CONVEYOR BELT AND A RETURN IDLER, PULLINGHIS HAND IN BETWEEN THE BELT AND IDLER.

EMPLOYEE WAS ATTEMPTING TO UNPLUG A BUCKET ELEVATOR FEED CHUTE USING A STEEL BAR. EMPLOYEE'S FINGER WAS CAUGHT AND PINCHED BETWEEN THE BAR AND THE ELEVATOR HOUSING.

EE WAS REMOVING (CUTTING) TAPE OFF OF A MOTOR ELECTRICAL WIRE. DURING THIS ACTIVITY, EE TRIED TO GRAB A RADIO MICROPHONE THAT BECAME DETACHED FROM HIS SHIRT AND CUT HIMSELF WITH THE KNIFE HE W AS USING.

EE WAS CLEANING A BUILD-UP OF MATERIAL IN A PREHEATER USING AIRLANCE/PRYBAR WITH A THRUSTING MOTION. EE EXPERIENCED PAIN IN HIS RIGHT ELBOW AFTER THE TASK WAS FINISHED.

EMPLOYEE WAS CUTTING PLASTIC SHEETING ON THE FLOOR. HE PUT A METAL STRAIGHT EDGE ON THE PLASTIC FOR A GUIDE AND WHEN HE STARTED TO CUT TH EPLASTIC, THE UTILITY KNIFE CAUGHT ON THE METAL AND T HE KNIFE SLIPPED ACROSS HIS FINGER. FOUR STITCHES WERE REQIURED.

EE WAS ATTEMPTING TO PUNCH OUT AN END PLATE OF ASILO VALVE WHEN THE PIECE OF WOOD HE WAS USING TO SOFTEN THE BLOW SLIPPED & HE STRUCK HIS FINGERWITH THE HAMMER

EE HAD JUST FINISHED LUNCH WHILE MONITORING OPERATIONS IN THE CONTROL ROOM AND WAS CLEANING THE KNIFE HE HAD USED DURING LUNCH. IT CUT THROUGH THE PAPER TOWEL HE USED TO WIPE THE BLADE AND CUT HIS THUMB.

EMPLOYEE SAYS HE WAS REMOVING A BEARING FROM A SHAFT WHILE IN THE REPAIR SHOP. THE HAMMER HE WAS USING GLANCED OFF THE SHAFT WHEN HE STRUCK IT AND HE HIT HIS OWN LEFT KNEE.

EMPLOYEE WAS ATTEMPTING TO DISLODGE MATERIAL IN A SILO BY HITTING THE CONE WITH A STEEL BAR. THE BAR BOUNCED BACK, STRIKING THE EMPLOYEE ON THE LIP AND LACERATION ON THE INSIDE. SUTURES WERE R EQUIRED.

EMPLOYEE HAD SET AND PRESSURIZED A HYDRAUULIC RAM BETWEEN TWO COAL MILL TABLES FOR DISASSEMBLY. EMPLOYEE'S HELPR APPLIIED MORE PRESSURE TO THE RAM WHILE THE INJURED STRUCK THE TABLE WITH A HA MMER. THE HYDRAULIC RAM POPPED OUT OF POSITION AND A 1 1/2" X4" ROUND SPACER USED WITH THE RAM HIT THE INJURED ON THE CHIN.

EMPLOYEE WAS PRYING A PIECE OF STEEL [13" X 13" X 2 1/4"] FOR THE ROCK BREAKER WHEN THE PRY BAR SLIPPED, CAUSING THE STEEL TO PINCH THE TIP OF THE LEFT THUMB.
