Whilst hammering a bushing head into a conveyor pully the operator hit EE's middle finger of EE's left hand with the hammer when it slipped off the bushing. The operator visiting urgent care the following day and was found to have a small hairline fracture in the upper part of EE's finger. EE was deemed fit to return to work

Operator was removing a guard on the crusher plant when the final bolt slipped and the corner of the guard fell against EE's lip cutting it
