Employee was in the process of installing a continuous miner bit insert using a brass hammer. A piece of the insert broke off and pierced the employees hand becoming embedded. Employee taken to the ER and given two prescription medications. EE was referred to a surgeon for removal of metal.

Two employees were in the process of installing the spray bar on the west concentrate screen. Employee 1 was using a hammer to align the bolt holes of the mounting plates, at that moment employee 2 started to place a washer and nut on a bolt to secure the spray bar. Employee 2 left ring finger was struck by the hammer.

Employee was using a stapler while splicing a belt and experienced a sharp pain which radiated down to the tips of the finger. Employee was diagnosed with bursitis / tendonitis on 3/8/23.

Employee was removing motor pulley, the puller came off & hit EE in the abdomen. Emp. was evaluated & released back to work without restrictions. MRI was ordered. Emp. was placed on restrictions related to hernia to abdomen on 9/15/2021.

Emp. was welding overhead using welding hood. Used chipping hammer to remove slag while still under the welding hood. Lifted hood and used wire brush to clean up weld. A hot piece of slag entered the right eye. Emp. taken to ER and eye flushed and given antibiotic. No restrictions given.

While using a sledge hammer to drive in a I-beam employee miss and mashed his thumb between the beam and hammer handle.

Employee was hooking up air impact into airline. The employee used a wrench to tap on the air line to clear out brine, stated that he was trying to get debris to come out of the fitting before hooking up impact wrench. He tapped on rubber grommet to loosen material.. Valve was in the off position when it broke. Employee closed a blocking valve upstream to isolate air.

Emp. was using a 24" pipe wrench to turn motor sheave to move output shaft of reducer. Emp. got his rihgt hand index finger caught between the wrench and bottom half of the guard. Emp. finger was smashed and cut. He will have surgery to repair on 08/24/2011.

3 emp. were replacing a broken cable. All emp. were on their knees on the deck in front of spring box assembly. One emp. was using a 3' bar to hold the cable bolt in place. Another emp. place the wrench on the nut on top of the cable bolt to loosen it. As the emp. pulled the wrench the bar came out of the spring box and came into contact with the other emp. left knee.

Employee was prying a bent return roller hanger. Since the belt was up against the ribs he had to stand next to the structure to get the hanger off. The scaling bar slipped, causing the employee to fall forward into the belt structure. Employee's right ear hit the structure causing a laceration.

Emp. stripping away PVC coating from electrical conduit with a pocket knife in his right hand when it slipped and he cut his left thumb on his left hand.

Emp. was swinging a 2 lb pin hammer to remove a link froman MBC conveyor chain when he felt a pain in his right elbow. Emp. requested medical attention. He was taken to ER and given restrictions and pain medication.

Emp. was cutting vacuum hose with a utility knife. He was cutting with his right hand rotating from right to left around the top of the hose. Knife slipped resulting in a laceration on his left forearm. Emp. taken to doctor and received 3 stitches.

Emp. cutting a hole in ventilation tubing to regulate the air, he cut the top of the first finger on his right hand just behind the finger nail. He was using his personal knife. Emp. taken to ER where he received stitches to his finger. No restrictions given at initial visit.

One emp. was hitting an eye bolt from the side with a sledge hammer to jar it loose so it could be removed. The sledge hammer came loose from his hands and hit another emp. on the left shin. Emp. that got hit was standing approx. 12' away.

Emp. was spotting bits on miner. He was using a raw hide brass hammer and bit wrench, as he was coming down to hit the bit wrench, the outside of his hand between the knuckles and wrist came in contact with another bit. Sent to ER. Positive X-Ray for fracture, placed on restrictions.

Emp. changing bits. While leaning over and using hammer, felt pain in between shoulder blades. Severe pain began as he started to adjust the position of the roll fan. Tips of his fingers on his left hand were numb and tingling. Sent to company dr, placed on restrictions. Muscle relaxant, MRI and P.T. ordered.

Emp was loosening a fitting on a water line a miner. He was using two end wrenches and pushing them inward. He felt pain in his right shoulder. Took to company doctor and was placed on medication, restrictions, and therapy.

Emp. was assisting a co-worker removing a 2" pin from a cylinder, both emp's were using a wrench & a bar-down-bar to rotate a sheave block. Co-worker was pushing on bar while injured emp. was pushing a wrench while kneeling, the bar slipped side-ways while hitting the emp. in the right elbow.

Emp. stated he was swinging a sledge hammer doing a belt alignment, when he noticed a bad pain in lower back. Taken to E.R for X-RAY negative. given shots and RX's for injury.Went to comp. Dr. he will return to work on restricted duty.

Emp. was removing a broken torque shaft from miner. While applying pressure with the bar to remove torque shaft, emp. felt a pop in rt. elbow, while tightening the bolts, emp. felt pain in rt. elbow. Took to comp. doctor on 11-03-08 put on restrictions.

Emp. was swinging a sledge hammer to hit the discharge chute (cleaning it out) then climbed down ladder and was walking when she felt a sharp pain in chest. Went to ER. Pulled muscle - placed on restricted duty.

Emp struck top of left hand with 5 lb hammer. Seen company doctor on 7/9/08 X-Ray negative, first aid given for first week, but then swelling and pain never went away. On 7/14/08 emp was placed on restrictive duty and given a prescription pain med. Possible bruised and inflamed tendon in top of hand.

Employee was removing a stuck bit from the center drum on a miner by using a 4 lb hammer and a bit puller. As he was hammering on the bit puller his hand slipped and he caught his left index finger between the hammer and the bit puller. Went to ER for treatment. Placed on restricted duty.

Employee was hammering another employee's hammer to drive the a pin in to replace a cat track pad on a miner. His finger got smashed between the hammer handle and the tram gear box on the miner. Sent to ER for treatment. Emp has compound fracture(doctor pinned bone together) and was placed on restrictions.

Emp. was using a shop bar to hold a bull gear and the shop bar slipped and pinched his middle right finger causing a laceration. Taken to ER for treatment. No broken bones, employee placed on restricted duty.

Employee was knealing on a spare piece of rubber belt set up to cut out a 4"X 5" piece. He was using a utility razor knife and the knife cleared the rubber and stabbed his right leg above the knee cap, causing a lacertation. Took emp. to company doctor where he recieved 4 stitches and placed on restricted duty.

Emp complained of pain at wrist on 12/19/07 while removing a drain plug from a gear box with chanel locks (Allen Wrench is the correct tool to use). He didn't feel like he needed to see a doctor so no treatment was done. on 2/21/08 emp went to company doctor due to continuous pain - he was placed on restrictions.

Emp. was repairing a broken conveyor chain on a miner. Half of the chain was on the miner & the back half was on the ground. The employee put the chain back on the miner and was pulling it together. The bar slipped and wedged his hand between the chain and the plate causing a laceration. Went to company doctor received sutures & xray positive for fracture. Restrictive duty.

Employee was changing out bits on miner with a hammer and mashed his left pinky finger between the hammer and the bit wrench causing a fracture and laceration to the left pinky finger. Taken to ER for treatment

Employee was removing a broken bit from the miner. The hammer hit a bit block in front of the bit wrench and deflected, causing him to smash his left ring finger between the hammer and a bit block. Employee to ER received 4 sutures, fracture of tip of left ring finger, immobilation. Saw company doctor on 6/29/07. Returned back to work on restrictive duty on 6/29/07.

Employee was pulling on a wrench to loosen center strap screen bolt. The wrench slipped off of the bolt head and the employee hit his elbow and upper arm on the framework of the screen body. Swelling and bruising noted with pain. Evaluated by company doctor on 6/20/07 x-ray negative, place on restrictions. returned to work on restrictive duty on 6/21/07.

Injured person had just installed a chain fall on a beam. The beam clamp slipped and the chainfall struck his finger.

While operating a conveyor belt stapler, employee felt numbness and tingling in his hands. Restricted duty and further tests were prescribed.

Emp was laser aligning coupling on Lang Main Line #17 drive. While tightening the bolt for the dodge coupling guard, the wrench slipped and left elbow hit the corner of motor base adjustment bolt.

Emp was installing pipe where the roadway bridge crosses the trench. Emp was pulling on wrench when he felt a pop and pain in his right shoulder.

Emp was adjusting wiper bracket, using a sledge hammer and standing in an awkward position. He felt pain in his right shoulder while hitting wiper bracket with hammer.

EMPLOYEE WAS STANDING ON LADDER USING HAMMER TO REMOVE RUST FROM OLD BEAM SO HE COULD LAY OUT HIS HOLE PATTERN ON OLD BEAM. WHILE HAMMERING, AN OBJECT FROM ABOVE FELL AND HIT THE EMPLOYEE IN THE LEFT ARM.

EE WAS CUTTING ACROSS A 42" CONVEYOR BELT, THE BELT WAS WADDED UP SO A BELT CUTTER COULD NOT BE USED AT THIS TIME. WHEN EE GOT TO THE EDGE OF THE BELT, THE BLADE CAME THROUGH & STRUCK EE IN RT THIGH RESULTING IN A SMALL LACERATION THAT REQUIRED 1 SUTURE TO CLOSE WOUND. EE RTW NORMAL DUTIES.

EE WAS REMOVING THE INSULATION ON THE TERMIANTION END OF AN ELECTRICAL CABLE USING KNIFE BLAD OF A MULTI TOOL DEVICE. THE KNIFE BLAD SLIPPED OFF THE INSULATION & CUT EE'S LEFT THUMB. LT. THUMB LACERATIN REQUIRED 2 SUTURES. NO RESTRICTIONS.

THREE EE'S WERE PULLING A 3' X 7" METAL PLATE WITH A COME-ALONG FOR OUTSIDE FOR #4 BIN. A 2" X 12" BOARD WAS BEING USED AS A GUIDE FOR THE PLATE TO SLIDE ON. ONE EE PULLED THE COME-ALONG & THE OTHER TWO EE'S GUIDED THE PLATE ACROSS THE HAND RAIL. THE PLATE SLIPPED AND ONE EE GOT L THUMB CAUGHT BETWEEN THE BOARD & METAL PLATE. RESULTED IN LACERATION REQUIRING SUTURES & MODIFIED DUTY

EE WAS REMOVING 3/4" MOUNTING BOLT FROM BASE OF 150 HP MOTOR. A 1 1/8" COMBINATION WRENCH WAS BEING USED. AS EMPLOYEE PUSHED WITH EXCESSIVE FORCE TO REMOVE BOLT, WRENCH SLIPPED CAUSING WRENCH TO HIT EE ON CHIN RESULTING IN LACERATION TO CHIN THAT REQUIRED SUTURES.

EE WAS CUTTING BOLT HOLE NOTICES IN WIPER MATERIAL. EE WAS HOLDING BELT WITH LEFT HAND AND PUSHING KNIFE AWAY FROM BODY WHEN BLADE SLIPPED. THIS CAUSED LACERATION TO THE BACK OF THE LEFT HAND BETWEEN THE THUMB AND FOREFINGER.
