Employee was changing out 3/4" bolts on flight for upper dust screw conveyor. EE was using the open end on two ratchet wrenches to tighten the bolt and nut. While tightening towards self with left hand and away from self with the right, the wrench in EE's left hand slipped off the bolt head. Employee lost balance, falling forward onto wrist rolling it over.

Mostrando los 9Employee sprained right wrist while pulling scale from #2 kiln. On 12/29/08, employee went back to doctor due to pain in wrist. On 1/15/09, we received a notice that employee would be getting therapy 3 times per week for 3 weeks. This is the reason the injury became reportable.
The employee was trying to break up a piece of scale from the rotary kiln with a pry bar. When he went to shove the bar into the scale he felt a pain in his right elbow. He is enrolled in physical therapy three times a week for three weeks.
While poking pocket on #3 Cooler, employee's hand slipped off rod pinching it between pipe & rod cutting right thumb. Taken to ER & they put in 3 stitches.
Employee was shoveling lime in kiln basement and some of the lime dust got into left boot burning top of foot.
EMPLOYEE WAS USING A LARGE MOTAR HOE TO PULL KILN COATING/SCALE FROM #3 KILN DISCHARGE HOOD. CONTINUED PULLING PUT STRESS ON RIGHT WRIST.
EE WAS LOWERING SPOUT IN PLACE WHEN COME A LONG HANDLE HIT HIS THUMB AND TORE OFF FINGERNAIL AND SMASHING THUMB. EE HAD TO HAVE STITCHES.
CHEST, ARM, NECK PAINS- THOUGHT MAYBE HEART ATTACK- TRANSPORTED TO LOCAL HOSPITAL. VIA EMS TESTSFOR HEART NEGATIVE- POSSIBLE MUSCLE STRAIN FROM SWINGING SLEDGE.
SHOVELING KILN WASTE, AIRBORNE LIME DUST ENTEREDRIGHT EYE. WAS WEARING SAFETY GLASSES WITH SIDE SHIELDS. SUPERVISOR REMOVED 1 SPECT OF DUST FROMEYE AND FLUSHED WITH EYEWASH. EMPLOYEE FINISHED SHIFT AND CONSULTED EMERGENCY ROOM PHYSICIAN THESAME DAY BECAUSE EYE WAS IRRITATED. PHYSICIAN REMOVED 2 MORE SPECKS OF DUST AND ADVISED 24 HOURSOF REST.
