Employee was working on a Excavator bucket changing out teeth. Employee used a sledge hammer to strike the tooth in attempt to loosen. After multiple hammer strikes, the bucket metal tooth broke causing a metal fragment to enter EE's LH Side Groin and penetrate the skin. A surgical visit is required to remove the object.

A cable holding a chain come-a-long failed and the chain hit an employee in the head. The employee received a laceration to the right side of his head that required 5 stitches to close the wound.
Employee was using a punch and hammer to create a bolt hole in a rubber skirt liner. The punch in his left hand tilted and he was not able to stop swinging the sledge hammer in his right hand and he smashed his left hand. The left index finger was bruised and the left middle finger had the skin split open for the length of two knuckles.
EE was removing wear shoe from Eagle Screw. EE was bent over screw to catch bolt as it was hammered out of shoe and shoe broke during hammering. Sliver from shoe punctured EE's t-shirt and skin.
EMPLOYEE WAS USING A STEEL BAR TRYING TO DISLODGE A LARGE ROCK THAT WAS JAMMED IN A CHUTE
EE WAS LOOSENING NUT ON 2 1/2" BOLT. SLEDGE HAMMER DEFLECTED OFF WRENCH AND EE STRUCK ANOTHER BOLT WITH FINGER OF LEFT HAND.
