The employee bumped into the chute gate valve an activated the chute gate, which struck a scaling bar they were using to relieve muck. The end of the bar caught their left cheek and cut them.

The scaling bar was pulled from the EE's hand causing the EE's arm to get pushed back and twist. This resulted in a torn ligament in the left bicep. The EE was placed on restricted duty by the physician.
While scaling, loose material fell from the rib and rolled over the EE's ankle causing a sprain/strain of the ankle. The EE was placed on restricted duty by the physician.

In 3000 Level Grizzly, using double jack to break rocks, piece of rock flew up and broke front tooth.
In 3000-079, hit large rock with smaller rock on top; smaller rock came back after being hit with double jack hitting miner in right eye. Miner was wearing safety glasses at time of accident.
Employee was attempting to remove a plug from a pump. There was a pipe in close proximity and slightly above the pump. As he was attempting to loosen the plug the wrench slipped and his chin made contact with the pipe causing his jaws to slam together. This resulted in a chip in Tooth #8 and piece of tooth #14 was broken off.
The employee was attempting to bar a large rock that was stuck in a muck chute. As he pried on the rock he felt a sharp pain in his back. He was diagnosed with a back strain/sprain.
The employee was breaking rock on the grizzly with a double jack. When he made contact with the rock a small shard of broken rock was ejected and it struck him on the wrist between the cuff of his glove and the Kevlar arm guard that he was wearing. It caused a small puncture wound that nicked a vein causing significant bleeding. He received one suture.
Employee was using a hack saw to cut a 3" plastic pvc pipe in an area with a whiz-bang set up for ventilation. While cutting the pipe some debris was blown in behind the safety glasses and became stuck in the right eye. The debris was washed out underground.
An employee was breaking rock on the grizzly when a large shard of rock flew up and struck his glasses knocking them off of his face. At the same time a tiny fragment of rock became embedded in his eye.
The employee was breaking oversized rock on a grizzly using an 8 pound double jack. He swung and struck the rock when the handle of the double jack broke on impact. He said he felt a pop in his elbow when this occurred and he also noticed pain. The doctor diagnosed this injury as a tendon strain and placed him on restricted duty.
The employee was breaking a large rock on the grizzly with a doublejack when a shard of rock flew back and struck him on the left forearm. He was wearing Kevlar sleeves but swinging the doublejack had caused one of the sleeves to slide down which exposed a portion of his arm.
The employee used a rap wrench instead of a long handled tool to break the bands on a bundle of timber and when the band broke one end recoiled and contacted his arm causing a laceration which required sutures.
EE is a miner trainee and was observing a slushing operation. The trainer was using light signals to direct the operator. He had been warned not to get too close to the chain that was supporting a Sheave block for the slusher cable but he walked into the hazard zone just as the chain broke which caused one end of the chain to recoil and strike him causing a contusion.
The employee was pulling muck from a shaft pocket when the material hung up due to a large rock bridging the chute opening. He was using a scaling bar to dislodge the rock and when it broke loose it struck the bar and cantilevered it upward striking the employee in the mouth.

4000-259 I Drift. EE was cutting his haul back cable on slusher bucket 8' from face, pulled cable tight on top of bucket to get it into cutter on top of bucket. Held cutter w/left hand and hit cable cutter with sledge hammer in right hand. After hitting the cutter 5-6 times, missed cable cutter, hitting his left hand smashing left ringer finger. 3-sutures.
EE was cutting a wrench holder out of a piece of air hose, slipped and cut left hand. Laceration to "L" hand.
