Mostrando los 7Maintenance Mechanic was attempting to loosen a 1" water pipe union. The employee was pushing on a pipe wrench with left arm when employee felt a pain in shoulder. The employee did not wish seek medical attention at that time. On January 4, 2018, the employee was seen by a doctor, and was placed on restricted duty.
Employee was using a 5 lb sledge hammer and poke bar to clean material build up in the tower. While striking the poke bar with the hammer, the hammer head slipped off of the head of the poke bar and the employees right point finger was crushed between the handle of the hammer and the head of the poke bar. The finger tip was fractured and stitches were used to close the wound.
A process attendant received minor irritation to the eyes when hydrated lime inadvertently flushed out of a port when a cap fell off. The employee was attempting to unblock a chute by using a threaded pipe when the cap was accidently hit and came off. Prescribed eye drops were given to the employee for the prevention of infection.
Laborer while walking around a corner in the shop bumped into an repairman while ee was trimming a poster with a utility knife. The accidental contact caused the left hand of the laborer to become lacerated by the index finger requiring 5 stitches.
EMPLOYEE WAS REMOVING A PIECE OF ANGLE IRON FROM UNDERNEATH A SCREW CONVEYOR BY USING A PRY BAR. WHEN THE STEEL WAS REMOVED, THE SCREW DROPPED ON THE PRYBAR FORCING HIS FINGER INTO THE SCREW.
EE WAS WORKING CLEANING OUT SCREEN CONVEYOR WHILE ANOTHER CREW OF EES WERE CHANGING A CONVEYOR BELT 3-4' ABOVE. BELT ABOVE WAS CUT & CONVEYOR FELL ONTO EE.
