EE was using a 24" pipe wrench and mashed finger in between the wrench's handle and another pipe lacerating and fracturing the tip of EE's right ring finger through EE's glove. The laceration required stitches thus making this MSHA reportable.

Employee was cutting a few feet off a hose to replace a section on a waterline that was leaking. While doing so employee went to cut the piece and that's when they cut their middle finger on the tip of the left hand.

The injured employee along with two others were locking down large threaded nuts on the coal mill roller journal assembly, using a large open end wrench and a 10lb sledge hammer. The injured EE was holding and adjusted the wrench when the hammer hit EE's middle finger.

Employee was trying to position a sprinkler head on a conveyor using zip tie straps. Seeing that EE had positioned the sprinkler head incorrectly, EE was going to use a utility knife to cut the zip tie. Sometime during this task, EE cut self.

Employee was attempting to remove a coal mill roller from its journal using a 5 pound sledge hammer. Not being successful, they then began using a 10 pound sledge hammer. On about the 5th swing, they lost movement to their right shoulder.

Employee was trimming the rail rubber with a utility knife so the wire mash screen could clear and go underneath the lip. The knife slipped and cut their left hand.

Employee was trying to remove a nylon coupling from a hydraulic pump with a small pry bar. Bar slipped from employees grip hitting EE in the lower lip causing a laceration requiring 7 sutures. Employee returned to work the following day with only restriction was to keep injured area clean.

The injured and another employee was cleaning out a lime slurry tank. The steel rod that they where using got stuck inside the tank. As they where pulling the rod out lime slurry started to flow. They where closing the gate valve, lime splashed into injured employee's eyes.

Employee was cutting a piece of rubber with a utility knife. Employee accidently cut hand.

The employee cut a 3/4 inch hose to length. Then he was making a vertical cut downward splitting the hose lengthwise. Holding the hose in his hand he cut through the hose his glove and the palm of his hand

Employee was using a parts puller to pull a coupling hub off a shaft, he said it was tight. He was using an adjustable wrench to turn the treaded shaft. He was using repetitive motion to remove the hub. After removing the hub, he turned and felt a sharp pain to his neck.

The employee was using a hammer and punch to remove the cut bolts from the connecting ring on the ID fan stack. A piece of the punch chipped off the top cutting his hand. At the time of the injury, the employee didn't realize that a piece of the chipped punch was in his hand!

The employee was using a sledge hammer to clear dust from a hopper. He believes he strained himself swinging the sledge hammer.

EE was changing screens with fellow ee. EE was prying up screen with pry bar, the screen cloth broke loose and hit employees finger causing a fracture.

EE was attempting to remove the anti-theft plastic band from a new socket wrench set. He began using a pair of pliers, which didn't work, so he used a utility knife instead to cut the plastic tabs. When he exerted pressure to the tab, part of the tab broke off allowing the force of the utility knife blade to hit his inner thigh, giving him a 1 inch laceration.

EE was unclogging a chute using a crowbar through a hole in the chute. When the rocks broke loose, the weight of the rock forced the crowbar down and the EE's hand got pinched between the crowbar and the chute. This caused a laceration to his thumb requiring stitches.

Employee was using a ball pin hammer and the concrete floor to straighten out a bent retaining collar ring made of metal, that is a part of the assembly of a Dodge speed drive reducer. Employee believed he struck the edge of the collar sending the collar directly toward him striking him above his right eye. The employee was wearing his safety glasses.

EE was helping maintenance pull cages out of the cloth bags that are a part of our dust collector system. He was pulling on this particular cage, when the cage became stuck. EE slid a screw driver insided the top of the cage to give himself some leverage. EE was pulling down w/his legs. Heard a pop and sharp pain to his lower right abdomen.

Inj EE and 2 others, were attempting to remove a harden sleave from a shaft on kiln cooler system. Inj EE was heating sleave with torch, the other EE's were prying on the sleave with a crow bar. The pry bar slipped and struck inj EE on the bridge of his nose.

EE was pulling on a torque wrench, he was torquing the head bolts down on a crusher head. When he pulled on the wrench he felt a twinge to his left upper chest muscle.

EE'S WERE TIGHTENING NUTS ON THE LONG THRUST ROLLER BOLTS. HE WAS PULLING UP WITH A PIPE WRENCH . THE PIPE WRENCH SLIPPED OFF OF THE NUT, CAUSING THE WRENCH TO STRIKE HIM AT HIS LEFT TEMPLE, GIVING A LACEERATION 1 INCH.

A PIECE OF BELT WAS HANGING FROM A CONVEYOR BELT, EE USED A UTILITY KNIFE TO CUT THE BELT FREE. AS HE PULLED THE KNIFE TOWARDS HIM, THE KNIFE CUT THROUGH SUDDENLY AND THE EXERTED FORCE CARRI ED THE KNIFE THROUGH TO CAUSE THE CUT.

EE WAS CUTTING A PIECE OF CONVEYOR BELT WITH A UTILITY KNIFE. AS HE WAS PULLING THE KNIFE TOWARD HIM HE ACCIDENTLY CUT HIMSELF. HE HAS A LACERATION ABOVE HIS LEFT ANKLE, 1" LONG HE REC'D 5 STITCHES & A TETANUS SHOT

EE WAS ATTEMPTING TO BREAK LOOSE A LARGE PIECE OF MOLTEN ROCK WHICH WE CALL KLINKERS, FROM THE BACK OF #5 COOLER. AS HE BROKE THE ROCK LOOSE WITH A PIPE, THE ROCK ROLLED AND PINCHED HIS LEFT H AND BETWEEN THE MOLTEN ROCK AND THE IBEAM, SMASHING HIS LEFT HAND, RING FINGER.

EE WAS DOWN AT THE LOWER END (SHIPPING) DRIVING A GROUND ROD (ELECTRICAL)INTO THE GROUND. AS HE ALMOST WAS COMPLETE WITH HIS TASK, HE MISSED THE ROD WITH THE SLEDGE HAMMER AND STRAINED HIS LEF T LUMBAR AREA.

EE WAS LETTING DOWN A DOOR OPERATED BY A MANUAL BOAT WINCH & THOUGHT HE HAD THE LOCKING LATCH ENGAGED BUT IT WASN'T THE HANDLE FLEW AROUND & HIT HIS RT THUMB & HAND. EE WAS DIAGNOSED ON 3/9/99 BORKE & DISLOCATED THUMB Y

EE WAS USING A 10 LB. SLEDGE HAMMER TO DRIVE THE SHAFT OUT OF A DRUM PULLEY, HIS LEFT FOREARM BEGAN HURTING & SWELLING THAT AFTERNOON. HE DIDN'T GO TO THE DR. UNTIL 8-12-98. (STRETCHED AND INF LAMED TENDONS.)

THE ELECTRICIAN WAS RELOCATING A RAMSEY SCALE BOX, HE HAD 1 SCREW STARTED IN THE TOP HOLE THEN KNELT DOWN TO START A SCREW INTO THE BOTTOM HOLE WHEN THE BOX FELL LESS THANT 12" ONTO THE TOP OF HIS HEAD.

EE WAS USING A HAMMER TO FIX THE METAL TIP ON HIS BELT BUCKLE WHEN HE HIT HIS INDEX FINGER TIP.

EMPLOYEE WAS CLEANING OUT A BUILDUP OF MATERIAL WITH A SHOVEL WHEN HIS RIGHT MIDDLE FINGER HIT A SHARP PIECE OF METAL. 5 STITCHES.

EMP'S WERE PULLING UP BOXES OF DIFFUSERS WITH A BLOCK AND TACKLE. EMP WAS REACHING OVER OT GRAB THE BOX WHEN THE PIPE WIHT THE BLOCK AND TACKLE BROKE LOOSE AT THE BOTTOM, STRIKING EMP ACROSS T HE BACK WITH HIS CHEST AGAINST THE HANDRAIL.

EMPLOYEE WAS PULLING A KILN GUN BACKWARDS WHEN HE RAN INTO AN ANGLE IRON SUPPORT PINCHING HIS RIGHT HAND BETWEEN THE GUN & ANGLE IRON. HE RECEIVED 3 STITCHES IN THE PALM OF HIS RIGHT HAND.

EMPLOYEE WAS USING A STEEL BAR TO BEAT ON #4 PREHEATER GRATES & HE HIT HIS LEFT ELBOW ON THE PLATFORM HANDRAIL, DIDN'T GO TO THE DOCTOR UNTIL 3 1/2 WKS. LATER & HAD OUTPATIENT SURGERY ON 11/18 -92 MISSING 1 DAY NOW MAKING THE REPORTABLE.

EMPLOYEE WAS USING A 2LB HAMMER AND CHISEL TO SEPERATE A COAL MILL BOWL FROM THE STEM WHEN HE HIT HIS LEFT THUMB WITH THE HAMMER.DIST PHALANX,UNDISPLACED.CRACKED THE BONE IN THE TIP OF LEFT TH UMB.

EMPLOYEE WAS STANDING IN FRONT OF #5 KILN TRUNION ROLLER END CAP. ANOTHER EMPLOYEE WAS BREAKING THE BOLTS LOOSE ON THE END CAP WHEN ONE OF THE BOLTS BROKE & HIT I,NAME IN THE FOREHEAD, JUST OB OVE LEFT EYE.

EMPLOYEE WAS PUNCHING OUT CRUSHER CHUTE WHEN HE BUMPED HIS LEFT ELBOW ON THE CHUTE CAUSING IT OT SWELL. A PRESCRIPTION MEDICINE WAS FILLED A TAKEN.

CUT INDEX FINGER WORKING ON WATER PIPE ON BACK OF PICKUP TRUCK, PIPE BROKE LOOSE & CUT KNUCKLE ON TAIL GATE OF TRUCK.
