Employee was carrying old scraper parts, another miner was cutting a lid from a bucket using a knife. The knife slipped as employee walked by, cutting employee's leg. Stitches were required to close the cut.

Mostrando los 36Employee was using an index bar to pry with while removing a stab jack from a piece of equipment & felt burning sensation in both arms. Later, medical examination was sought & showed that employee needed surgery for the repair of the condition, surgery on 9/23/2019, therefore, employee is on days away from work. Late reporting due to the late medical evaluation.
Employee was removing wires from a communication box, the box was sitting on a table. The employee used a razor knife to cut a couple of wires, the knife slipped and cut employees wrist area. The wound required stitches and employee was placed on restricted duty.
Welder was standing on a platform, driving a wedge setting the sidewall on a dragline bucket in the bucket shop. The employee lost balance and put left hand on the dog while swinging the 6 lb. Sledge. The sledge glanced off the wedge and struck the employee's left ring finger.
Employee was removing approximately 600 feet of 4" plastic pipe. They came to an area where the pipe crossed some cables and decided to break a coupler using a hammer. While striking the coupler, the coupler broke and allowed the pipe to swing up and strike employee in the face breaking some teeth.
Employee was attempting to straighten a cable hanger clamp using a pair of channel lock pliers. As EE was performing this task, the channel lock pliers slipped and struck EE in the mouth breaking a tooth.
Employee was removing a fuel filter from a haul truck with a chain wrench when the wrench slipped and struck the employee on the right side of his jaw chipping his tooth.
Employee was using a scaling bar to push on the longwall monorail system. As he was prying, he slipped and hit his head on the rib causing him to become unconscious momentarily.
Employee was using a roof bolt to pry a cable tray with. As he was prying, the roof bolt slipped and struck employee on the side of the head. Late reporting due to further medical evaluation (8-14-2015) which placed him on days away from the work place.
Employee was kneeling, cutting a 4 inch rubber hose with a box knife. When he stood up he punctured his right thigh above the knee with the open box knife blade.
The employee opened a 5 gallon bucket of Foray fire extinguisher chemical with a box cutter knife. The bucket turned and so did the knife causing the knife blade to lacerate his left ring finger. Six sutures were used to repair the wound.
Employee was trying to get the deck bushing turned with pry bar supporting it. Bar slipped out and bushing came down and smashed right hand and fingers middle 4th and little.
Contractor was using a chain hoist to lift mesh up, the chain hook dislodged and hit him in the side of the face. The individual could have been knocked unconscious for a few seconds.
Two contractors were replacing screw jacks, in the process of handling one of the jacks, it slipped from one contractor's grasp and hit him on the foot. He has been placed on restricted duty.
Employee was helping in the setting of screw jacks in the beltline entry. The tube portion of the jack was set up right and they were placing the screw portion onto the tube portion. Employee's thumb was caught between the two pieces. He has been placed on restricted duty.
Employee was changing wheel unit on shuttle car, he was using an Allen wrench to remove bolts. In removing the bolts he felt pain in his shoulder and the pain increased as he removed the next bolt after Allen wrench slipped. This has been a chronic situation with employee, this is being deemed an illness.
Employee was removing a grate over a sump in order to clean the sump. Employee used a 1/4 inch round stock hook approx. 3 feet long with a 1 inch 90 degree bend on the end of the hook. The grate slipped off the hook and struck the employees right foot just behind the steel toe of his boot resulting in a fracture to his Medial Metatarsal of his right foot.
The employee was pulling a belt back together with a come-a-long. When the employee released the come-a-long the come-a-long spun rapidly and struck the employee in the jaw.
While opening cable jacket on shuttle car cable, the knife slipped and cut employee's finger. The laceration required stitches to close.
Employee was lowering mesh on the longwall face with the use of a hand winch. The employee released the winch, the handle of the winch struck the employee in the hand, this action fractured a bone on the top of his hand. He has been placed on restricted duty.
Employee was digging a ditch using a pick to loosen the material. The employee swung the pick and felt a pop in his middle/lower back.
Employee was using a razor knife to open a jacket on a electrical cable, he was cutting away from himself but the knife slipped cutting his thumb. Stitches were used to close the cut.
While attempting to free up the gearing of a hoist with the use of a screw driver, the screw driver slipped allowing employees finger to come in contact with one of the gears of the hoist. Employees finger was cut, requiring stitches.
Employee alleges that while swinging a sledge hammer on a piece of rock, he felt a pop in his elbow. After extensive medical evaluation, the employee has been placed on restrictive duty. The difference in date of injury and reporting is due to medical evaluation and investigation conducted to determine whether the alleged accident was indeed an industrial related injury.
Employee was cutting a section of belt with a knife. The employee slipped, causing the knife to dislodge from the belt, striking the employee above the right knee. The injury required stitches.
While cutting on a plastic bucket with a razor knife, the knife slipped and cut the left thumb of the employee. Stitches were required.
Removing old belt roller. Roller was hard to get out so he was tapping it with a hammer. Employee tried to grab roller when another employee hit his finger with a hammer causing a laceration to his fingers which required stitches.
Employee was attempting to cut a zip tie loose with a utility knife. In doing so, the knife slipped cutting his left lower leg (calf). The laceration required six stitches.
Employee was crossing a locked out belt conveyor and caught cap lamp and pouch on a 4" pipe and cause the handle end of a utility knife to hit him in the lower left rib. The incident caused a fractured rib.
At the time of the incident employee was using an air torque wrench to remove sprocket segments. As he was removing one of the bolts holding a segment in place the wrench slipped from his grip. When he attempted to catch the wrench he accidently squeezed the wrench trigger, activating the wrench and causing the torque arm to swing into and smash the left ring finger.
The employee was cutting an envelope open with a pocket knife when he cut through the envelope and caused a laceration to the inside of his right wrist. This injury required three (3) stitches.
THE EE WAS HOLDING THE BOTTOM OF A GROUND ROD DRIVING TOOL TO HELP ANOTHER EE, WHO WAS HOLDING THE TOP, DRIVE THE GROUND ROD IN. THE EE'S LEFT HAND SLIPPED OFF THE BOTTOM OF THE TOOL DURING IT S UPWARD MOTION. THE TOOL CAME OFF THE ROD. DURING THE DOWNWARD MOTION, THE EE'S LEFT INDEX FINGER WAS LACERATED WHEN CAUGHT BETWEEN THE TOOL AND THE ROD. THIS RESULTED IN THE FINGER REQUIRIN
THE EE WAS HOLDING A 5 1/2" SOCKET. ANOTHER EE WAS HOLDING A 4# SLEDGE HAMMER TO HIT THE SOCKET TO SEAT A BUSHING. THE HAMMER DEFLECTED OFF A NEARBY PIECE OF METAL STRIKING THE EE'S RIGHT THUM B. THIS RESULTED IN EE'S THUMB BEING FRACTURED. ALTHOUGH THE ACTUAL EVENT TOOK PLACE ON 5-5-02, IT WAS NOT DETERMINED THAT A FRACTURE EXISTED UNTIL 5-6-02 WHEN THE EE OBTAINED XRAYS.
THE EE WAS UTILIZING A HAMMER TO REMOVE A BACKUP WRENCH. AS HE STRUCK THE WRENCH IT FLEW UP AND STRUCK THE EE IN THE MOUTH. THIS RESULTED IN A LACERATION TO THE UPPER LIP.
THE EE WAS ATTEMPTING TO LOOSEN A BOLT. THE HAND TOOL BEING USED SLIPPED AND THE EE'S LEFT KNUCKLE STRUCK AGAINST AN ENGINE CASTING. THIS RESULTED IN A LACERATION TO THE LEFT KNUCKLE, WHICH RE QUIRED SUTURES.
THE EE WAS HOLDING A SLUGGER WRENCH ON A NUT. ANOTHER EE WAS USING A 10# SLEDGE HAMMER TO HIT THE WRENCH . THE HAMMER DEFLECTED OFF A PAD EYE AND STRUCK THE EE'S LEFT INDEX FINGER. THIS RESULT ED IN THE EE'S FINGER BEING FRACTURED.
