Employee was working on a side auger bulk unit and had hand on a hydraulic ram. When the employee moved the ram it closed completely, catching the employee's hand between the ends giving the employee a fracture and lacerations.

Employee was using a sledge to knock a spacer out and the spacer flew and hit him in the cheek. 4 Stitches required

Employee was driving a pin into the track segment and a piece of metal came off and hit another employees arm causing a cut that required 5 stitches.

EE was using a bar on the pins on a highwall miner and his bar came up and hit him. He received 5 stitches.

EE was using a utility knife to cut a water hose. He was cutting away from his body and came to the end and turned his knife back towards himself to cut the remaining inch. He pulled back and stabbed his leg. 3 Stitches were required.

EE was helping our mechanic change a ride cylinder on a 777 Rock Truck. EE was holding a B&O and the mechanic was using a sledge to drive the pin out of the cylinder. The sledge hit the B&O and bounced off and hit EE's hand, causing a cut that required 5 stitches.

EE was using a 10Lb. sledge to knock a corner protector off a loader bucket, when a piece of steel flew off the protector and hit his right leg. A doctor had to remove it.

Injured was lowering a belly pan down on a 475 Komatsu dozer when his pry bar slipped and hit him in the cheek. Had 3 stitches.

EE was helping install a Cylinder Yoke Bushing in a D11R. The bushing did not want to move, so they hooked up a battering ram from the machanics boom to force it in. The ram hit the back of EE's hand and took skin off up his arm. EE is off work for fear of infection.

Injured was using a hammer changing bits on an auger head and hit his finger causing a cut which required 3 stitches.

Injured was using a sledge to drive a pin in and a sliver of metal came off and went into his calf muscle. Doctor removed it. No stitches required.

Employee was using a bar to unplug a feeder chute. The bar kicked sideways and pinched his fingers between bar and chute. Employee was taken to a medical facility.

Was removing a transmission filter on a cat 777. Filter was stuck and he put extreme pressure on it. When it let loose he felt a pull in his back. This happened on Monday July 2nd. And he was in pain and went to doctor on Aug. 6th and then reported it to us.

Swinging sledge on tooth of back hoe and piece of steel came off and went into his abdomen.

Employee was performing maintenance, prying on a gear in a crusher box when a piece of steel from the toothed gear chipped off and struck him in the mouth area.

Employee was helping with maintenance in the wash plant and was cutting tabs off with a utility knife when the knife slipped lacerating himself requiring stitches.

Employee was attempting to loosen a bolt holding a fuel filter and the wrench slipped and struck the employee in the facial area causing stitches in the lip and mouth area and chipping a tooth.

The employee was performing maintenance on the auger and was prying with a long bar when the bar slipped off and struck him in the eye brow causing a laceration abvove the eye.

EMPLOYEE HAD PINCHED HIS HAND BETWEEN A PIPE AND A WRENCH AND APPROXIMATELY 7 DAYS LATER GOT AN INFECTION FROM A PIECE OF STEEL LODGED UNDER THE BOTTOM OF HIS HAND.

EE WAS REPLACING A LOADER TOOTH AND WHILE STRIKING THE STEEL PIN WITH A HAMMER, A SLIVER OF STEEL STRUCK HIM IN THE LIP AREA.
