Injured was lowering a belly pan down on a 475 Komatsu dozer when his pry bar slipped and hit him in the cheek. Had 3 stitches.
EE was helping install a Cylinder Yoke Bushing in a D11R. The bushing did not want to move, so they hooked up a battering ram from the machanics boom to force it in. The ram hit the back of EE's hand and took skin off up his arm. EE is off work for fear of infection.
Injured was using a hammer changing bits on an auger head and hit his finger causing a cut which required 3 stitches.
Injured was using a sledge to drive a pin in and a sliver of metal came off and went into his calf muscle. Doctor removed it. No stitches required.
Employee was using a bar to unplug a feeder chute. The bar kicked sideways and pinched his fingers between bar and chute. Employee was taken to a medical facility.
Was removing a transmission filter on a cat 777. Filter was stuck and he put extreme pressure on it. When it let loose he felt a pull in his back. This happened on Monday July 2nd. And he was in pain and went to doctor on Aug. 6th and then reported it to us.

EE WAS REPLACING A LOADER TOOTH AND WHILE STRIKING THE STEEL PIN WITH A HAMMER, A SLIVER OF STEEL STRUCK HIM IN THE LIP AREA.
