Mostrando los 18Employee was trying to flip a D11 cutting edge over and the crowbar slipped out of the cutting edge causing them to slip and fall on a strut that was laying on the ground.
Employee was removing a Bit and Ring on Highwall Drill, after opening the left side of the ring EE struck the right side of the ring with a Mallet. The ring came around the drill steel and struck EE in the mouth damaging a tooth requiring dental procedures and causing a laceration on EE's lower lip requiring stitch. Employee had no lost work days and returned with no restrictions.
Employee was replacing brakes on a Komatsu 730E Rock Truck using a chain ratchet. When EE moved the handle on the racket, EE's hand hit a sharp burr on the brake rotor cutting finger. EE was taken to the ER. The cut required three stitches to close the wound. EE had on jersey gloves. EE came back to work the same day and continued to work.
Employee was using a 3/4" drive ratchet to tighten a brake pack on a Rock Truck. The ratchet slipped off the bolt and hit them in the face. The resulting injury was a broken nose and six stiches to their nose.
Employee was removing a tire from the rim on a fuel truck with a metal bar. EE was prying down on the bar when EE felt a pop and resulting pain in EE's left shoulder.
While using a chain block to line up a trunion ball on a grader, the hook came undone and struck the employee in the groin area causing swelling.
Employee was changing cutting edges on a Cat 16M Grader using a line-up bar to position the bolt holes. He had his hand around the end of the line-up bar which was has a 90 degree flat, sharp edge. When his helpers bar slipped, the resulting jar caused the sharp edge of the bar to penetrate his leather glove resulting in a cut requiring 3 stitches.
Injured was attempting to open the lid of a 5 gallon bucket of oil. The spout ring broke when he lifted it. He laid his left arm on the bucket lid and attempted to use a sharp chisel to pry open the spout when the chisel slipped and struck his left arm resulting in a laceration.
Employee used a steel sledge hammer to strike a bucket tooth on a loader and a piece of metal struck him on the leg resulting in a laceration.
While changing a dozer blade on a D10 dozer, the tilt jack was suspended by a hoist. Employee laid his hand between a rock guard on the push arm and tilt jack. As the push arm moved forward the cable on the hoist slipped and the tilt jack fell catching his fingers between tilt jack and rock guard.
EE had a plastic cup holder lying on the dashboard of his drill. He was using a utility knife to cut it down. As he was putting pressure on the knife, the plastic broke causing the knife to continue downward and strike him in the leg.
Employee was prying on a chute with a pry bar. The pry bar slipped and employee smashed his right thumb against corner of chute.
Whle attempting to remove a bucket tooth by using a hammer a piece of metal struck EE in the right calf muscle.
Employee was raising screen plate and 3" pipe caught on chain hoist, causing pipe to break loose, smashing left hand & breaking it.
EE WAS USING A SLEDGE HAMMER INSIDE THE #2 SILO TO REMOVE (KNOCK OUT) A PIECE OF ANGLE IRON. WHILE STANDING ON A PLATFORM HE REACHED OUT (STRETCHING) TO SWING HAMMER WHEN HE THOUGHT HE HAD PUL LED A MUSCLE. HE CONTINUED TO WORK. HOWEVER ON 2-20-04. HE MISSED WORK TO REPORT TO DOCTOR.
INJURED EE WAS ON OPPOSITE SIDE OF BULLDOZER, ABOUT 8 FEET FROM THE PIN THAT WAS BEING STRUCK WITH HAMMER BY THE WITNESS. A SLIVER OF METAL CAME OFF THE PIN STRIKING INJURED EE IN LEFT EYE.
