Service Technician was standing along side his service truck. At a distance of aprox. 30 feet another crew was installing a track on a Hitachi EX600-6, using the prescribed method when a piece of metal flew across the 30 feet and struck the employee in the neck.

Mostrando los 18EE was removing a bolt that was holding the oil cooler adapter plate. Bolt was extremely tight and he was using both hands. While pulling, felt pain in right elbow.
Two employees changing bell crank on a loader. One was striking a bearing with a hammer and two metal fragments hit the other standing to the side. One fragment embedded in the right cheek and had to be removed.
Securing 2 hydraulic pumps to the bed of parts truck with chains and binder. Strained low back.
Employee was using a hacksaw to cut a piece of 3/8" all thread being held by a vise. Employee was using left hand on top of right hand holding the hacksaw. When the saw cut through the all thread, his left hand struck the vise. He continued to work but the hand later swelled. Upon examination, he was found to have a minor fracture to the left hand.
Injured employee was driving a pin out a bed hoist cylinder on a rock truck with a slate bar. When the pin came out, the bar pinned against the dump bed catching his right middle finger causing a laceration requiring 8 sutures to close.
Employee stated that he was tightening a bolt with a wrench; the wrench slipped & hit him on the bottom lip.
EE was using knife to make cut out. Knife slipped resulting in 16 stitches to EE left forearm.
Employee was welding under the frame of a shovel and a foriegn object struck his eye while chipping slag.
Employee was using hammer to drive block under truck bed. He hit his hand against truck breaking 2 middle fingers. Dr visit on 1-24-06 - restricted to light duty and treated with splint.
Employee was installing a bearing on a dozer blade. He attempted to hit a plate and pin over the bearing with a sixteen pound sledgehammer. As he swung the sledgehammer, the hammer struck the top ear of the blade. This caused the handle end of the sledgehammer to bounce back and strike the employee in the left thigh causing a severe contusion.
PUNCTURE WOUND, METAL CAME OFF HAMMER AND ENTERED LEFT HAND,.
EE WAS REMOVING THE DOOR SHIM PIN FROM A SHOVEL BUCKET. HE HAD A NYLON STRAP ATTACHED TO THE PIN& TO THE BOOM ON HIS SERVICE TRUCK, PUT TENSION ON THE STRAP BUT THE PIN DIDN'T PULL FREE. THEN HE REMOVED HIS WELDING HELMET BUT DIDN'T PUT ON HIS HARD HAT. HE STRUCK THE PIN WITH A PRY BAR & IT RELEASED STRIKING HIS HEAD & CAUSING A LACERATION THAT REQUIRED 8 SUTURES TO CLOSE.
EE WAS REPAIRING A BEARING ON REAR AXE ON TRUCK #147, HE WAS PUTTING RACE ON AND SLIPPED AND HIT HIS LEFT MIDDLE FINGER WITH HAMMER.
MECHANIC WAS DRIVING AN 1 1/2 FUEL HOSE ONTO A 1 1/2"COMBINATION NIPPLE. THE HOSE SLIPPED OFF & THE MECHANIC STRUCK HIS LEFT INDEX FINGER WITH THE HAMMER. THE FINGER RESTED THE HOSE NIPPLE WHEN STRUCK.
EMPLOYEE WAS ON THE LOWERED DRILL MAST TO REPLACE AN 0-RING ON A BOSS FITTING. HE WAS ATTEMPTING TO LOOSEN THE FITTING WITH THE USE OF A CRESCENT WRENCH AND 1 1/8"COMBO WRENCH. HE COULD NOT GE T THE FITTING TO BREAK LOOSE AND HE ATTEMPTED TO STRIKE THE CRESENT WRENCH WITH THE COMBO WRENCH. THE COMBO WRENCH STRUCK THE LOWER PORTION OF HIS LEFT THUMB CAUSING LACERATION.SUTURES RECVD.
EMPLOYEE WAS STANDING ON TOP OF THE RIGHT HAND ROLLER FRAME OF CAT D-11 DOZER. HE WAS DRIVING A PIN INTO THE HARD BAR TO SWING THE FRAME. AS HE HIT THE PIN, A SMALL PIECE OF METAL CAME OFF OF THE PIN AND STRUCK HIM IN THE CALF OF HIS RIGHT LEG. THE METAL HAD TO BE REMOVED BY A PHYSICIAN.
EE WAS INSTALLING AN INSERT BEARING ON THE ROD END OF A STEERING CYLINDER OF A CAT 988F LOADER. HE WAS USING A PIN TO HAMMER THE BEARING INTO THE ROD END. HE STRUCK THE PIN WITH A 20 LB SLEDGE HAMMER. A METAL FRAGMENT CAME OFF THE PIN AND STRUCK HIM IN THE LEFT THIGH. HE WAS EXAMINED BY SUPERVISOR. A SMALL LACERATION WAS OBSERVED WITH SOME REDDENING.
