The miner was helping the certified electrician splice A 300 volt D.C shuttle car cable. The miner placed the cable on employee's leg then cut the insolated jacket of the cable with A razor knife when the knife cut through the cable causing A laceration to the miners leg.

Miner was in process of assisting to make a belt splice when employee's hand slipped and was struck on the Right ring finger causing laceration and fracture of right ring finger.

Miner was using sledge hammer on coal chute inside Coal Preparation Plant when the chute was missed with hammer head. Miners hand struck chute causing fracture to Left index finger and laceration to said finger.

Employee was removing rock duster hose with belt knife, Knife slipped, cutting tip of left ring finger - resulting in five sutures.

Employee was helping the shop repairman repair A-arm on a diesel mantrip. While releasing the track jack middle finger of left hand was pinched between the jack handle and the wheel well causing a hairline fracture to finger.

Employee was cutting a piece of conveyor belt to use as throat rubber in continuous miner when utility knife slipped and cut his left hand, requiring three stitches.

While using a hammer to hit another hammer, a sliver of metal from one of the hammers struck and embedded in the employee's left forearm.

While loosening a nut with a wrench, the nut broke loose suddenly and the employee hit his left thumb against a sharp piece of metal causing a laceration. The employee received 9 sutures.

While splicing a continuous miner cable, the utility knife the employee was using slipped and cut him on the right thumb causing him to receive sutures.

While prying on a cat pad, the pry bar slipped and the employee hit the back of his hand on a jagged piece of metal causing a laceration. The employee received 1 suture to the middle finger on his left hand.

While watching another employee beat on a piece of metal, a small piece of metal sheared off cutting the employee on the chin. The employee received 6 sutures.

While trying to move a large rock with a chain, the chain broke, picked up a board and tossed it through the air. The board struck the employee's index finger on his right hand causing a laceration. The employee received 5 sutures.

While cutting a piece of mining belt that was laid across the employee's lap the knife came through the belt and cut the employee on the left leg. The employee received 2 staples.

Employee was at the #4 entry cutting a roll of tape with a belt knife. The knife slipped cutting his left thumb and index finger causing lacerations. He received 6 sutures in the left index finger.

While watching someone else hammer on a bearing, a piece of metal flew off of the bearing and struck the employee on the arm.

While opening a glue box with a utility knife, the knife slipped and cut the employee on the left wrist. The employee received 4 sutures.

Employee was in the process of making a splice in a piece of mine belt. As he was cutting the belt with a utility knife the knife slipped causing a small laceration to the left knee. He received 4 sutures.

Employee was cutting a piece of rubber off a tire of a supply trailer with a utility knife when the knife slipped causing a laceration to his right arm. He received 6 sutures.

While tightening a conveyor chain on a shuttle car the socket slipped off of the nut and employee fell face first into the bumper on the shuttle car. Employee busted his bottom lip and received 7 sutures.

While cutting a suction hose for a roof bolter, employee cut his index finger on his left hand. Employee received 3 sutures.

Employee was using a pair of cutters to cut the metal bands on a bundle of nine (9) foot roof bolts and one of the metal bands cut his left wrist causing a small laceration. He received five (5) sutures.

While changing the lights in the men's shower employee was standing on a ladder when he felt himself trying to fall and when he reached to catch himself he stabbed his left hand with a utility knife he had in his right hand. Employee received 6 sutures.

While trying to put a bolt in the rubrail on the miner employee was using a long screw driver to pry the rubrail in order to line up the holes when the rub rail slipped and fell on the employees left foot.

While changing a stationary roller employee was prying on the roller with a pry bar when the roller broke loose causing the roller to fall and hit the end of the pry bar. The pry bar flipped upward striking the employee on the left side of the chin. Employee sustained a fractured jaw and 10 sutures.

Employee had just finished cutting a piece of brattice cloth and was putting the belt knife back in his pocket, the blade was accidently left partially open and cut his finger. Employee received 3 sutures to his index finger on his left hand.

While making a belt splice on the #2 unit belt located in the #5 entry employee raised hammer, striking the mine roof dislodging a piece of rock which struck employee on right thumb area. Employee received 6 sutures.

The injured employee and another employee had removed a cat pad from a continuous miner road chain. They were pulling the road chain back together with a come-a-long when the hook end of the come-a-long dislodged striking the injured employee in the nose. He sustained a fractured nose and received 4 sutures.

Employee was using an axe to break the metal bands that hold brattice block together and one of the bands, after being struck, recoiled back striking the employee on the right side of his chin causing a leceration. The laceration required 4 sutures by a physician.

Employee was attempting to break apart a feeder conveyor chain by striking it with a hammer when a piece of the hammer broke off striking him on the chin. The piece of metal lodged up under the skin requiring it to be removed by a physician. He received 4 sutures.

Employee was hitting on a plastic (pump line) pipe, with a hammer, attempting to insert one section of pipe into another section of pipe when a piece of the pipe he was hammering on broke causing his left hand to strike the pipe cutting his index finger. He received 4 sutures.

EE WAS HELPING ANOTHER EE CHANGE OUT BITS ON THECONTINUOUS MINER. THE OTHER EE STRUCK A BIT WITHA HAMMER AND A PIECE OF METAL CAME OFF THE HAMMER STRIKING EE IN THE EYE LID.

WHILE CHANGING BITS ON THE CONTINUOUS MINER HE HIT THE BIT WRENCH WITH A HAMMER CAUSING A PIECE OF METAL TO COME OFF OF THE WRENCH STRIING HIM IN THE LEFT CHEEK RESULTING IN 2 SUTURES.

EE WAS USING WEDGES TO BREAK ONE SID OF FORM. THE FORM BROKE & DROPPED. EE HAD FINGER BETWEEN FORM & CONCRETE, MASHING FINGER.

EE WAS TRAMMING A BELT SPLICE WHEN HIS KNIFE SLIPPED CUTTING HIS RIGHT LEG.

EE WAS TIGHTENING ROPES ON A BELTLINE WHEN GRIPLOCK CAME LOOSE HITTING HIM IN THE MOUTH.
