Was using a railroad jack to jack up a header. When EE was letting the header down the jack caught and the bar came up and hit EE in the chin. This resulted in a small laceration and a chipped bottom tooth.

Preparing to cut miner cable off of miner. While cutting cable hand slipped cutting left index finger.

Employee was attempting to get his steels unclogged, by striking his steels with a 1 foot wrench. When he struck the steels, the wrench bounced back and hit him in the mouth. This resulted in a chipped tooth and 4 stitches in his bottom lip.

Contract Employee was in the act of splicing a belt at 3 E Header. The employee was cutting toward himself when the knife slipped cutting him across his left upper leg. Contract Employee received five stitches at the Elk Creek nurses station and returned to work.

Knocking a piece of metal off of a sub in the yard with a pry bar. The pry bar slipped mashing his left pinky finger between the pry bar and the sub. This resulted in a laceration to the pinky that required 7 stitches to repair.

EE was helping mechanic make a splice in a car cable. When he was cutting the outer jacket off with his belt knife, the knife slipped cutting his left hand resulting in a laceration that required stitches.

EE was pulling pry bar off of his scoop and caught his hand between the end of pry bar and coal rib resulting in accident. He received 3 sutures in little finger on his right hand. He was back to work on 12/10/2012.

Employee was changing a buffer on a belt wiper. He was using a pry bar to get it out & when it came loose it hit him in the mouth, chipping three teeth.

Employee was cutting belt. He was pulling belt knife towards himself, when knife slipped and came across his right leg above his knee. The laceration took three sutures to close up. Employee came in early to get started on belt.

EE was cutting plastic glue bucket while doing so the belt knife slipped crossing his left hand cutting it. Gloves were worn at the time of the accident.

Employee was splicing open a scoop charger cable, he was pulling the cut jacket with his left hand while cutting with his right hand. The knife blade hung in the jacket and caused him to lacerate his left hand.

Injured employee was attempting to get a broken bit out of the bit block, with a screw driver and hammer. Employee struck his left ring finger with the hammer causing a smashing injury, resulting in a fracture to his left ring finger.

Employee was aligning belt roller with a hammer, when he struck his left hand against a timber in the belt entry. Employee reported this incident on 11/9, sought medical treatment on 11/11. Due to a previous un-work related injury to his hand it was not determined to be work related until 11/18/2011

Injured employee was cutting #9 left, in the 11 seam # 1 unit, when a rock fell on the miner. Employee backed miner out and was using a miner cable strap to pull the rock off the miner. The rock swung around striking employee just above his boot on the left leg.

The injured employee was using a hammer to drive a spad nail into the coal rib when he inadvertently hit his left thumb. This resulted in a fracture to his left thumb.

The injured employee was cutting a string off of a plastic bag and cut the palm of his left hand. This resulted in 5 sutures.

Employee was in the process of adjusting a bottom belt roller which was positioned in close relation to a timber, as he went to strike the roller hanger with his hammer his right index finger was caught between the hammer handle and the timber. This resulted in a laceration to his right index finger and he recieved 3 sutures.

Employee cut his middle and ring finger with a belt knife.

Employee was cutting a curtain, when he cut his left thumb with a belt knife.

Employee was splicing a car cable when he cut his left index finger causing him to recieve 5 stitches.

Employee was tearing a curtain down, another employee was helping him and he was cutting the curtain down and he inadvertently cut injured's hand.

Employee was in the process of cutting bands from 8 ft cable bolts, when a band he had previously cut flipped over and cut his forearm, causing a laceration.

Employee was hit on the chin by a come-a-long while working on a pinner.

Employee hit his knee with a ratchet.

While plugging up a cathead, the wrench slipped off, catching the Employee's finger between the wrench and the sub.

Employee was cutting belt when he cut his thumb.

Employee was helping split a pinner cable when the knife slipped and cut his left hand just below his thumb.

Employee was in the process of hanging a ventilation curtain when he smashed his finger with a hammer

Employee was cutting a bit bucket and accidently cut his right leg.

Employee was shoveling on a tailpiece when he felt his knee pop.

Hanger holding a come-a-long broke causing the come-a-long to strick both hands resulting in lacerations.

Employee was in the process of hanging a ventilation curtain when he attempted to drive a nail in a board and the nail flew back and struck him in the tooth and lip and resulted in a chipped tooth.

While digging with a pick/axe under concrete pad, employee struck protruding piece of rebar between finger and pick causing laceration to finger.

Employee was in the process of trying to open a set of equipment doors. The employee was pulling on the rubber seal of the door with a pair of channel lock pliers when he slipped and landed on his self rescuer, thus resulting in a contusion to his side around his left rib area.

Employee was trimming a piece of curtain and cut his arm with the belt knife

EMPLOYEE WAS SPLICING A MINER CABLE WHEN THE BELT KNIFE SLIPPED AND CUT HIS LIFT THUMB REQUIRING 6 SUTURES.

Employee was cutting a piece of rubber belt and his knife slipped and cut his left hand.

EE WAS WORKING ON A CABLE REEL ON SHUTTLE CAR, & A SCREWDRIVER HE WAS WORKING WITH SLIPPED & HIT EE OVER THE EYE CAUSING A LACERATION.

EE WAS WORKING ON A SHUTTLE CAR, AND HIS WRENCH SLIPPED HITTING HIM IN THE UPPER LIP.

EMPLOYEES WERE REPLACING THE 1/4 " STEEL HOIST CABLE ON THE PARTS ELEVATOR WENCH AT THE VOLUNTEER SHOP. INJURED WAS USING A SET OF CABLE CUTTERS AND A BALL-PEINE HAMMER TO CUT THE CABLE TO THE RIGHT LENGTH. AS EMPLOYEE STRUCK THE CABLE CUTTER WITH THE HAMMER, THE BALL PART OF THE HAMMER CAME LOOSE AND STRUCK HIM IN THE RIGHT FOREHEAD CAUSING A SMALL LACERATION. THIS REQUIRED TWO SU

EMPLOYEE WAS IN THE PROCESS OF OPENING AN OLD SPLICE INT HE MINER CABLE AND AS HE WAS CUTTING THE TAPE OFF WITH A BELT KNIFE HE CUT HIS LEFT HAND.
