Employee was trying to cut straps on cable bolts. EE was cutting away from self and couldn't cut the bands. EE turned the knife around and was using it like a saw. when the band finally cut, the blade contacted EE's gloved hand causing a laceration in the palm of EE's thumb. This required 6 stitches.

Employee was using a multiplier to remove lug bolts from a scoop. The socket shattered causing EE's hand to slip and hit EE's arm. This caused a laceration that required stitches.

Employee was helping remove a flight from a miner chain. Employee was over the chain. Another employee was using a hammer to drive the pin out. A piece of metal broke off the pin, went under the safety glasses and embedded in the Employee's eye.

Employee was on EE's knees cutting out inserts on a miner. As EE was cutting, another employee was using a hammer to drive the others in. They were about 3 feet apart. A piece of insert broke, went above EE's glasses and into EE's eye.

Employee was using a slate bar to pull a rock off of the miner (the continuous miner was located in a heading). When the rock slid off the side of the miner, they did not allow enough room to move out of the way, the rock landed on their left leg, this resulted in a fracture.

Employee was using a sledge hammer to remove belt clamps. When they swung the hammer, the handle came in contact with the clamp and caused a laceration on their finger that required stitches.

Contract Employee was wedging a brattice with a pole ax. They had both hands on the handle driving the wedge. The handle of the ax came in contact with the block, mashing their index finger. This caused a fracture to the end of the finger.

EE was cutting a zip tie with the utility knife. EE had left hand resting on the machine. When EE cut the zip tie the blade slipped and cut the palm of the left hand.

Employee was hitting 2 hammers together to remove a stuck bit on a miner. A piece of the hammer chipped and embedded itself into the employee's wrist. The piece of metal was not removed until 2/24/2023 on which date it became reportable.

Employee was in the process of setting a tailpiece. The chain slipped causing the tailpiece to set down on a crib causing the crib to roll over. The crib landed on the employee's foot causing a fracture on the end of one of employee's toes.

Employee was using a sledge hammer to drive a head jack pin back in on the miner that had worked out. As employee was striking the pin, a piece of metal broke from the edge of the pin, flew under employee's safety glasses piercing lower eye lid and embedding in eye.

EE was working on taking batteries off a scoop, the battery pin was stuck. EE was using a bar to try to pry the pin loose. The bar slipped and struck EE in the jaw causing a broken jaw.

EE was working on 7 section and was using utility knife and cut right finger. EE saw nurse, did not need stitches but nurse put a butterfly strip on cut.

Employee was helping make a belt splice. As EE was running EE's hand along the splice, making sure splice was braided, another employee struck EE's hand with a hammer. Resulting in injury to EE's left middle finger.

Employee was in the warehouse before EE's shift started. EE was cutting a zip tie, and the knife slipped cutting EE's right thumb. The laceration required 6 stitches. EE returned next shift to regular job duty.

Employee was working as an electrician on putting planetary on continuous miner. He had slate bar under planetary when it fell on slate bar smashing his left thumb between slate bar, and miner causing a laceration that required 20 sutures to close.

While in the process of using a jack and chain to adjust belt structure, jack slipped causing employee's right middle finger to be mashed between the chain and belt structure.

While in the process of using a utility knife to cut rope hangers from roof bolt cable, knife slipped causing a laceration to employee's right hand that required sutures to close.

While in the process of trimming a belt splice using a utility knife, the knife slipped causing a laceration to his left hand that required sutures.

Employee was using a pry bar to remove loose material from the top of the continuous miner, the bar slipped causing his right ring finger to be mashed between the bar and the miner. Employee finished his shift and went for medical evaluation where x-rays were taken. X-rays showed a fracture to the metacarpal in the tip of the injured finger.

EE parked the shuttle car that he was operating, as he was using a pick type hammer to pull come loose material from the rib, his right forearm was caught between the material that he was scaling and the shuttle car. EE continue to work until 10/15/2012 when he was seen by a local physician and placed off work to see a specialist.

Employee was in the process of making a belt splice using a Flexco hammer, when a sliver of metal struck his right index finger, resulting in a laceration that required sutures to close.

While in the process of using a utility knife to cut belt, knife slipped resulting in a laceration to employee's right leg.

Cutting zip tie off self rescuer. Utility knife slip cutting right hand. The cut required 12 sutures to close wound.

He was cutting a zip tie off leaky feeder line. The knife slipped cutting his left hand. 5 sutures were required to close wound.

Cutting zip tie that held up fire suppression hose. Metal whiz wheel cut his left finger causing 8 sutures to close wound.

Making a splice, cut left thumb with utility knife. The laceration required 3 sutures to close left thumb.

Employee was using a utility knife to cut life line, knife struck employee's right leg above his knee, resulting in a laceration.

Injured employee was cutting a strap on his knee pad when his utility knife slipped and cut his right leg.

Injured employee was cutting a piece of belt, when the utility knife slipped, striking him in the right leg. This resulted in a laceration.

Two employees were in the process of using a bow saw to cut timber, when the bow saw became fouled, injured employee removed his right hand from saw and the other employee pulled the saw, resulting in a laceration to injured employee's left index finger.

Injured Employee was trimming the edges of a splice when the utility knife slipped, causing a laceration to his right leg.

Injured employee was using a hammer to make a conveyor belt splice, when a foreign object struck knuckle on employee's right hand.

Injured employee was using a screwdriver to replace a suction hose on a Fletcher roof bolter. While in the process of applying pressure to screwdriver, a foreign object struck employee's right eye, resulting in scratches to the eye.

Injured employee was hitting his drill steel to unstop it with a wrench, when he felt pain in his right forearm.

Employee was performing work on a track switch; employee had placed a lifting jack between the two track rails and was applying pressure to the lifting jack bar, when lifting jack slipped, causing the bar to strike employee's head, resulting in a laceration, requiring glue to close.

Employee was using a bolt cutter to remove his pad lock from his battery light cord. While in the process of cutting the lock the handle of bolt cutter struck employee's left eyebrow resulting in a laceration that required 5 sutures to close.

Employee was in the process of making a splice in a 14/3 cable, when the knife he was using slipped resulting in a laceration to his left thumb requiring 5 sutures to close.

Injured EE was securing the fish plates on two pieces of rail. The chain ratchet that was hoisting the rail came loose, causin gthe rail to fall and strike EE on the left thigh. EE went for X-rays, report is pending. Possible fracture to the patella.

Two EE's were in the process of tearing dwon a brattice. While removing a cap wedge, the hammer struck a roof plate, causing a small piece of metal to stirke the injured EE in the left eye. Injured EE was seen by the local ER physician and received medical attention. The physician gave him a full release to return to work.

Empoyee was in the process of making a permanent splice in a trailing cable, the utitily knife he was using in the process came into contact with his finger, resulting in a laceration that required sutures to close.

Injured employee was using a utility knife to cut the straps on a large bag of rock dust. While in the process of cutting the strap, the knife struck injured employee's right forearm, resulting in laceration that required 15 sutures to close.

Clawbar slipped and smashed finger on rail.

Injured employee was assisting another employee who was using a pole axe to cut a belt pin when a piece of metal struck injured employee's left eye. Injured employee was seen by the ER physician and then was recommended to an eye specialist for further treatment.

Injured EE was lifting belt structure with a chain ratchet; hook that supported the chain broke, causing the structure to strike the EE in the left forearm, resulting in a hairline fracture.

Employee was using a 10 ton lifting jack to lift a derailed track motor. While in the process of applying pressure to the jack bar, employee's hands slipped, resulting in the jack bar striking injured employee's lower jaw.

Employee was changing the brake pads on a rubber tired 4 wheel mantrip when the wrench he was using slipped causing his right hand to be cut by the brake rotor. Seven stitches were required to close the laceration.

Injured EE was positioned next to a fellow EE that was using a sledge hammer to strike a feeder flight link. A piece of metal came off the sledge hammer striking the injured employee in the left thumb. The injured employee was seen by local emergency room physician to remove the piece of metal. No sutures were required.

Injured employee and a fellow employee were engaged in horse play. As a result of horse play, injured employee received a laceration to his right forearm requiring several sutures to close. Horse play is a violation of company policy. Both employees have been terminated.

Two employees were in the process of making a belt splice. The injured employee placed his hand on top of the nail plunger to move it, when the other employee swung the hammer striking injured employee's right hand, resulting in a fracture to the index finger.
