The repair was on the K pin location of the machine. During the install, the bearing seized roughly a quarter of the way in. To continue, EE used a brass hammer to drive it in. During one of the hammer swings, EE accidentally missed. The left finger got caught between the hammer handle and the bearing causing a fracture.

Mostrando los 20A serviceman injured his right shoulder while cutting a padlock off a truck's tool bin with bolt cutters. Afterwards he swung his arm to test and relieve the pain when his shoulder popped. This caused dizziness and numbness to his arm, forcing him to kneel. After regaining strength, he rose to report it. He was seen locally and in Kayenta where a strain was diagnosed.
The injured right index finger was pinched between a pipe wrench and the hoist brake shoe of a dragline. An impact wrench was activated from the opposite side to rotate the adjustment bar of the brakes, when the injured' s hand was jerked free pinching his index finger.
The mechanic was stringing a sling through dozer blade cutting edge bolt holes with the cutting edge in tilted position. A pry bar was used to hold the cutting edge in place, slipped causing the cutting edge to drop on mechanic's right hand/fingers.
Lower back/twisted pelvis using a pin removal tool to remove the equalizer pin from the roller frame the tool followed the pin into the bore. The pin came out and equalizer bar misalignment caused the bar to shift against and side load the tool. The tool handle struck his hand and causing his body to twist putting a strain on lower back.
THE DRAGLINE CREW WAS REMOVING THE 2ND INTERMEDIATE PROPEL BEARING COVER RETAINER. THE COVER WAS COCKED & JAMMED SO THE CREW WAS ATTEMPTING TO USE A WEDGE TO STRAIGHTEN OUT THE COVER. THE INJ WAS HOLDING THE WEDGE WHILE ONE OF THE CREW ATTEMPTED TO STRIKE WEDGE WITH 16PD HAMMER. MEMBER MISSED WEDGE & STRUCK INJ RIGHT HAND CAUSING FRACTURE TO 4TH FINGER METACAPUS
THE WELDER WAS USING A WRENCH TO PUT A 2 1/4" NUT ON A BOLT WHEN HIS RIGHT LITTLE FINGER WAS PINCHED BETWEEN THE WRENCH AND NUT WHILE WORKING ON THE SEGMENT BASE BOLTS.
The injured was repairing a tractor step that had been removed and he was pounding on a bracket to straighten out the step. He pounded on his thumb with a 20 lb sledge hammer and had a laceration and fractured bone.
THE INJURY WAS PICKING UP TOOLS AFTER THE BACKHOE BUCKET REPAIR. HE PICKED UP CHIPPED HEAD SLEDGE HAMMER BY THE HEAD WITH SHARP EDGE THAT CUT HIS TIP OF LEFT LITTLE FINGER.
He tighten the nut with ratchet on conveyor belt clamp while his helper was holding the bolt with open-end wrench from the bottom side. He was standing on inclined walkway with his left foot placed across the walkway.When he was tightening the nut most of his weight was on hig right foot. He stood back up in straight position and turn to his side when his left ankle
THE TECHNICIAN WAS CLEANING AROUND THE PULLEY BY REMOVING FROZEN COAL WITH A PRY BAR WHEN HE APPARENTLY HIT A GUARD WITH HIS HAND CAUSING THE PRY BAR TO STRIKE UP & HIT HIS RIGHT CHEEK.
THE WELDER WAS HAMMERING ON A BUSHING INSTALLING IT INTO A CHAIN LINK WHEN HIS (R) INDEX FINGER WAS CAUGHT BETWEEN THE HAMMER HANDLE AND THE BUSHING.
THE MECHANIC WAS SWINGING A HAMMER TO KNOCK OUT A PIN WHEN IT GLANCED OFF EQUIPMENT FRAME AND STRUCK HIS LEFT WRIST. HE EXPERIENCED PAIN IN HIS WRIST FROM THE IMPACT AND ALSO FELT PAIN IN HIS LEFT SHOULDER.
THE MECHANIC WAS USING A HAMMER & A SOCKET AS A PUNCH TO REMOVE A SHAFT CARRIER BEARING OFF A LOWBOY TRAILER WHEN HE HIT HIS THUMB AGAINST THE BEARING HOUSING. THE HAMMER DEFLECTED OFF THE TRA ILER FRAME AS HE WAS TRYING TO HIT THE SOCKET-PUNCH.
THE INJURED STATED HE WAS HELPING DRILL OPERATOR BREAK TWO DRILL STEMS APART. THE STEEL CLAMP/BREAKOUT WRENCH WASN'T HOLDING THE DRILL STEM FIRMLY. MECHANIC PLACED 2" X 4" X 16" BOARD BETWEEN THE DRILL STEM AND BREAKOUT WRENCH TO INCREASE GRIP ON DRILL STEM. HE MOTIONED THE DRILL OPERATOR TO TIBHTEN THE DIE GRIP ONTO DRILL STEM. WHEN THE WRENCH MOVED QUICKLY IT CAUGHT HIS RT THUMB.
INJURED WAS ASSIGNED TO INSTALL SIGNS & METAL POSTS USING A 20 LB SLEDGE HAMMER TO DRIVE IN METAL POSTS INTO THE GROUND. DURING THIS PROCESS THE INJURED MISSED THE POST AND STRUCK HIS LEFT HAN D ON TOP EDGE OF SIGN POST. THE INJURED WAS HOLDING THE HANDLE TOO CLOSE TO THE HEAD OF THE HAMMER RESULTING IN A CUT TO BACK OF HAND BETWEEN INDEX FINGER AND THUMB AREA. 14 STITCHES
THE MECHANIC WAS LOOSENING ALTERNATOR BOLTS WITHTWO WRENCHES WHEN ONE WRENCH SLIPPED. THE SECONDWRENCH USED AS LEVERAGE JAMMED INTO HIS PALM CAUSING A CUT.
EMPLOYEE PREPARING TO BLEND EMULSION UNHOOKED AN SOLUTION HOSE AND SPILLED A SMALL AMOUNT OF HOT SOLUTION ON LOWER LEG AND ANKLE; HE WENT TO FIRST AID STATION FOR ANTISEPTIC AND BANDAGE. ON 9 /6/00 HE WENT TO CLINIC BECAUSE OF INFECTION.
THE INJURED CLAIMED HE AND HIS HELPER WERE INT EH PROCESS OF PUTTING THE RADIUS ROD IN UPRIGHT POSITION, SO IT CAN BE HANDLED IWTH A FORKLIFT. HE WAS USING A 1 1/2 FOOT SCREW DRIVER TO PRY W ITH WHILE HOLDING THE RADIUS ROD AT THE EYE SECTION WITH HIS LEFT HAND. THE SCREW DRIVER SLIPPED CAUSING HIS FINGERS TO GET CAUGHT BETWEEN THE RADIUS ROD AND THE CONRETE FLOOR.
THE WELDER WAS HITTING THE BOTTOM END OF 1"X3'X4' RE-INFORCEMENT METAL PLATE ON THE SIDE OF A DRAGLINE BUCKET, WHEN IT CAME LOOSE AND HIT HIM ON HIS UPPER BACK.
