Employee was cutting green dust line with a utility knife and the blade got stuck. When they forcibly removed the knife from the line, they stabbed themselves in the leg.
A contract employee was cutting a piece of mine belt with a utility knife and the knife slipped, resulting in a small laceration just above the right knee.

Employee was trying to loosen a 3/4" fitting on a locomotive using two wrenches. One of the wrenches slipped and EE's hand contacted the metal stop for the engine cover lid, causing a laceration.
Employee was helping pull broken cat pads back around the crawler frame of the longwall tailpiece. A 2-ton come-a-long was being used to pull the cats. The come-a-long chain, still securely fastened on both ends, became bound. When it came loose the chain struck the employee on the wrist, causing injury.
Employee was using a hammer to beat rust off a bolt and some rust got behind employee's safety glasses and into right eye.
Employees right foot slipped while cutting conveyor belt causing body position to shift resulting in a laceration to the left forearm resulting in 8 stitches. Employee was released back to work with no restrictions.

Employee was prying on a damaged belt roller with a slate bar to free it from the belt structure cradle. The slate bar slipped pinching EE's left thumb between bar and bed rail. The employee was wearing EE's gloves and all other required PPE. Employee is a fully trained belt person.
Employee was trying to free up the scoop winch hook from the winch frame by striking it with a D-ring. EE set down the D-ring and the hook released, falling and striking the employee in the right middle, ring, and pinky fingers.
Employee was working in an awkward (close quarters) position, removing the horizontal pins from lw shield relay bars. EE was using a 2lb hammer. The hammer glanced off the pin head and EE's momentum caused EE to strike right elbow on the relay bar in the area of the "funny bone".
Employee was using a slate bar to move track rail closer to rib. As employee re-positioned the bar for better leverage, employee hit a finger on the eyelet used to attach the safety rope on the rail car. Employee became a lost time on 2-17-2020. Employee was properly trained and wearing all required PPE.
Employee was in the engine compartment working to replace the head gasket on #8 mantrip. EE was prying on the pulley with a pry bar. The pry bar slipped causing employee to fall down into the engine compartment, striking elbow against the battery box. Employee is trained and was wearing all PPE.
While pulling feeder cable slack, employee #1 and employee #2 miscommunicated and both went to cut a "roy" rope to release tension, at the same time. Employee #1's right middle finger was cut by the knife blade of employee #2. Both employees were wearing their PPE and were fully trained.
Experienced mechanic #1 was holding the torque multiplier in position as another experienced mechanic #2 torqued the bolts. The torque multiplier slipped and mechanic #1's right pinky finger was pinched between multiplier and shearer drum. Both employee's were wearing their PPE and were fully trained.
A trained miner, wearing all PPE, with 1 year experience as a trackman, was replacing a track frog switch on 106 back switch. EE was using a track jack to spread the rail in order to install the frog. When ee hit the frog with the sledge hammer, finger was pinched between the sledge hammer handle and the track jack, causing a 1 inch laceration on right index finger.
Employee was holding up the pan line to shield "snake head" coupler with a pick hammer in right hand. The relay bar was out (close to the snake head) and ee was using left hand to feel for alignment, for the relay bar to couple with the snake head. The pick hammer slipped, allowing left ring finger to be pinched between the end of the relay bar and snake head.
Employee was cutting a coupler out of green discharge line. The line was wet, employee's gloves were wet. The knife slipped and cut employee's left thumb due to bad hand placement.

Two employees were in the process of putting the cover back on the tail gate sprocket. They had two bolts to put in yet. 2ndemployee was using the hammer trying to get the bolt holes lined up. Injured employee turned to ask where the bolts were when 2nd hit injured employee's finger with the sledge hammer.
Employee was helping another mechanic remove bolts from the cable handler jack on a Joy loading machine. The adjustable wrench the other mechanic was using slipped off the bolt head and struck the injured employee in the mouth, breaking one of the employee's front teeth.

Employee was using the pointed end of a pick hammer to punch a drain hole in a 5 gal. hydraulic oil can. Employee's left hand was on top of the can supporting it. As employee took a horizontal swing at the can employee slipped causing the hammer to glance off the can, striking employee's left hand.

Employee was using a shovel to push a large rock off the longwall shear. The shovel slipped and employee fell against the shear. Employee stuck hand out to catch fall and it contacted the pointy end of a bit fork. The bit fork penetrated glove causing a puncture wound to left palm area near thumb.
Employee was pounding the keeper staple out of a longwall ram jack hose with a hammer and chisel. When the staple let loose, stored hydraulic pressure propelled the hose upward smashing employee's left index finger between the hose fitting & the chisel.
Employee was cutting green "rock dust" line with a utility knife and the knife slipped slicing his left index finger.
While in the process of re-railing a supply car the car shifted and struck employee in the left lower leg, fracturing his tibia just above the ankle.

EMPLOYEE WAS WORKING TO PUT A DERAILED CAR BACK ON TRACK. HE HAD RAISED THE CAR WITH A TRACK JACK. AS HE ATTEMPTED TO LOWER THE CAR TO THE TRACK THE WEIGHT OF THE CAR CAUSED THE JACK HANDLE TO "SPRING" UPWARDS INJURING HIS RIGHT SHOULDER AND ARM. HE WORKED HIS REGULAR JOB UNTIL 01/16/2014. HIS FIRST REGULAR SHIFT MISSED WAS 1/19/2015.
The employee was using a utility knife to cut the end of a rock dust hose. While finishing the cut, the knife slipped off the hose and cut his left palm causing a laceration which resulted in 11 sutures.
