Taking water line apart with a hammer. Swung hammer to remove line and missed causing right elbow to strike belt framing. Soft tissue and ligament damage.

Making a splice in #4 belt. Knife slipped and cut left index finger. Bandaged on unit and went to Dr. for stitches.
Employees were attempting to jack up one side of tailpiece with a railroad jack to prevent #3 belt from contacting the tailpiece. The jack slipped out (kicked out) and off of a piece of wood that the jack was sitting on, striking injured in the left knee causing pain and swelling in the knee.
EE WAS USING A PIPE WRENCH TO TIGHTEN UP A UNION ON THE MINER PUMP. THE PIPE WRENCH SLIPPED & HIT EE IN THE RT. EYE AND BROKE HIS SAFETY GLASSES.
EE WAS HELPING TO ALIGN A 75 HP MOTOR ON A COAL FEEDER. CO-WORKER WAS PRYING THE MOTOR UP WITH A PRY BAR AND EE PLACED HIS LEFT HAND UNDER THE MOTOR. THE PRY BAR SLIPPED LOOSE, MASHING EE'S LE FT SMALL FINGER UNDER IT.
