While on mine property, a security guard working for Guardco Security LLC shot and fatally injured a trespasser. This accident is being investigated by WV State Police.

EE was putting a new ball stud in a sterring jack, While striking it with a hammer a piece of metal flew off and hit him in the left eye. EE was taken to PMC Hospital immediately after the accident, and then was transported to Hospital in Morgantown.

While employee was changing bits in the cutter head, the hammer he was using hit the chisel and slipped off and struck the back of his hand. He was holding the chisel in his left hand, the hammer was in his right.

Bar slipped and hit his neck cutting him on the right side below his ear.

Employee was using a pry bar to dislodge some material in a hopper when the bar slipped, causing a quick jerk to his back. Diagnosis is pending.

Was lifting up mantrip to change tire when it fell on his hand.

Was prying on a rock in box at #2 head drive. When it popped loose, pry bar hit him in the chest.

EE's were attempting to drive a wedge in powerhead with hammers. EE slipped and as he caught himself, left hand slid across powerhead, causing hammer to strike hand.

EE was using a hammer and chisel to remove u-bolts from the leaf spring of the mantrip. The contact surface of the chisel slipped off the head of the hammer causing the chisel to strike him in the mouth chipping off part of a tooth.

Mechanic was driving in a pin and a piece of metal flew off and lodged in upper left arm. Object was removed by an ER physician.

Was prying metal with pry bar, when it slipped and jarred his body.

EE was working on radiator using a pry bar. The pry bar slipped hitting the ee in the nose.

Employee was trying to get a fan hub off of a fan motor with a hammer and punch. When he was striking punch with hammer a piece of hub came off and struck him in the hand.

Mechanic was cutting back rubber on 777 transmission harness to put in new plug. He was using a utility knife when it cut through the rubber and sliced into his left pinky finger.

Cutting a piece of plastic, tripped over a rock and cut index finger and middle finger on right hand.

Breaking bolts loose on starter and felt something pop in hand.

Hammering pin into excavator bucket, chip flew off and hit him in the arm.

While electrician was attempting to repair a bent guard on a scoop, his hammer slipped causing him to mash his finger.

Removing a pin in a roller frame. He hit the pin with a hammer. He slipped causing the hammer to miss the pin and cut his finger.

Working on tail piece on back of miner, was prying on motor with pry bar, bar slipped and caught finger between bar and motor.

EE was attempting to knock a bit out of a SHM cutter head with a ball peen hammer. As he missed, he struck himself on the left thumb. He had some light swelling and discoloration. We applied ice and advised medical attention.

Cutting lid off a box, struck hand with the box cutter.

EE was pulling on a wrench and felt something pop in his left elbow.

Employee was loosening a small nut with a crescent wrench. He felt a pop in his right arm, and then pain.

Employee was cutting teeth from loader bucket with torch and chipping hammer. When he struck the tooth with the hammer, a piece of metal chipped off striking him in the groin.

Driving bar pin out of D11R roller frame using slate bar & hammer, pin came out from roller; frame shifted causing bar to kick down hitting employee in arm & hand.

Employee was attempting to put an air conditioner belt on an L-1 loader. Pry bar slipped hitting him in the mouth/upper lip. He said he was holding the pry bar down with his elbow while he was trying to get the belt onto the machine.

Cutting a piece of metal off a tape line with a knife, knife slipped & cut left forearm. (6 stitches)

Found a airline leaking on a slip joint. Pulled the line off. To trim the end off while cutting the line the knife slipped and cut his forearm. We took him to the er room where they put 5 stitches in the cut.

Working on undercarriage, trying to get top roller off using a hammer, a piece of metal came off the roller and was embedded in the right little finger.

PUTTING A LINE-UP PIN IN AN INGERSOLL RAND DM45 DRILL TRACK USING A SLEDGE HAMMER. HE HIT THE PIN AND A PIECE OF METAL HIT HIM IN HIS LOWER RIGHT ARM AND EMBEDDED.

REMOVING AND REPLACING TILT CYLINDER, BEARING WERE BAD IN TILT LINK AND EMPLOYEE WAS USING A HAMMER TO REPLACE BEARINGS. WHILE USING HAMMER, A PIECE OF METAL CAME OFF OF THE BEARINGS & EMBEDDE D INTO HIS FACE.
