The employee was using a 10" X 1.5" brass bar to bump a bearing race into a final drive hub. The employee unexpectedly missed the race, causing little finger to get pinched between the bar and race. The employee was wearing gloves at the time of incident. The employee fractured the tip of little finger.

Additional backup support was needed to break loose a drill stem connection that was very tight. The pipe wrench used as backup support slipped and pushed EE's hand into an expanded metal guard that had an exposed edge. The exposed metal cut through the glove and cut the bottom side of employee's right ring finger.

Cutting fabric to install in stilling basin. Fabric slipped, forearm contacted blade causing puncture laceration requiring two stitches. Treated at hospital, refused "post-accident" drug test. Was reminded of co. drug policy and consequences of refusal. EE still refused. Possibility drug use may have contributed to accident, unable to confirm due to refusal to testing.

Employee was installing a new OEM pin in the equalizer bar on a Caterpillar D10R dozer roller frame. The employee was using a slide hammer to perform the task. While installing, a small piece of the pin became detached and struck the employee above his knee.

Employee was removing c-frame bolts on the lower segment. While swinging a 20lb. sledge hammer, employee made contact with the bolt with the hammer. Upon contacting the bolt with the hammer a pain shot in his upper right arm.

Employee was on the ground on the right side of the dragline preparing to install a cover plate on the propel. Another crew were working on the "A" leg platform when a chipping hammer fell from the platform and struck the employee on the arm.

Employee was using 20lb hammer to knock out hoist drum bolts and hit hammer handle on bull gear with pinky finger between handle and gear.

Employee using impact wrench to remove bottom roller. Loader cribbed with 1' of clearance. He loosened what he believed was the next to the last bolt, the roller dropped. Impact wrench pinched employee's finger against track. It was found that employee had completely loosened the other bolt, but it never fell so he thought it was holding.

EE was struck in the chin by a sliver of metal from a socket wedge being driven out by a sledge hammer appx. 10 ft away. Sliver was removed and chin stitched.

EE was installing the master pin in a set of rails on a truckhoe.This requires driving the pin in with a sledge hammer,as the employee was doing this,a small piece of metal chipped off the end of the pin,hitting the EE on the side of his right leg.EE was taken to the ER requirinf 4 stiches. All proper PPE was being used.

Two mechanics were removing a U-joint from a haul truck and were striking the U-joint with a sledgehammer to remove it from the dragline. One mechanic was swinging the sledgehammer and struck the U-joint when a piece of metal flew off & struck the 2nd mechanic in the right thumb.

Injured and a co worker were driving out a pain in the steering boss of a motorgrader. the co worker was helding a track maul against the pin and the welder was striking the maul with a sledge hammer when a piece of metal flew off and struck the welder's right thigh

THE INJURED EE WAS REMOVING BOLTS FROM A P&H LOADER PROPEL TRANSMISSION. HE WAS USING A 3/4" RATCHET WITH AN 18' HANDLE PLUS AN ADDITIONAL 18" CHEATER PIPE FOR ADDED LEVERAGE. THE SQUARE DRIV E ON THE RATCHET BROKE OFF ALLOWING THE INJURED TO LUNGE FORWARD CAUSING A LACERATION APPROX 2 1/2" LONG TO HIS RIGHT HAND.
