The designated person called for the cage to pick the people up from the work deck. The designated person failed to notify the hoist person the exact location of the work deck while it was on approach. Therefore causing an employee to be in line of the landing area, the bottom of the cage brushed EE and pushed IE away from landing area.

Mostrando los 11A miner was working in the #1 production shaft. While being tied off to a divider, the employee stepped back onto the workdeck and radioed the hoist operator to go up. The employee forgot to transfer the lanyard from the divider to the work deck and was subsequently pulled off the work deck by the lanyard and landed on the divider fracturing a leg.
While 2 EEs were riding work deck below chippy, a hose blew on hoist & it began to unreel. Hoist operator hit E-Stop. 1 EE on chippy work deck lost his balance & struck a divider injuring back. He was taken to a hospital to be checked out, given shot for pain, & released to full work duty. Hose blew @ 5PM, @ 5:22 evac called, @ 5:25 MSHA called, @ 6:00 all EEs out of mine.
The employee boarded the skip to ride down with the first shift. His hand was positioned on top of the closing handle as he was sliding the gate closed. When the gate slid to the closed position his right middle and ring finger were in the line of fire of the magnet sensor for the skip. A crushing injury resulted in a single fracture to both finger tips.
EE & COWORKER WERE LOADING EXPL;OSIVES INTO A DRILLED ROCK SALT FACE. COWORKER WAS OPERATING CVONTROLS TORELOCATE MANBASKET TO NEXT ROW OF HOLES. EE HAD HAND OVER TOP RAIL, FINGERS OUTSIDE OF BASKET. HANDRAIL STRUCK ROCK FACE, PINCHING EE'S FINGER.
OPERATOR ERROR CAUSED SKIP TO STOP SUDDENLY. SUDDEN STOP CAUSED INDIIVIDUAL TO STRAIN HIS BACK.
THE EE WAS RIDING THE SERVICE SKIP INTO THE MINE AFTER LOWERIG MATERIAL. THE LARGE DOOR CAME OPEN WHILE THE SKIP WAS TRAVELING IN THE SHAFT, STRIKING HIS RIGHT SHOULDER.
EE WAS ENTERING MINE USING PRODUCTION HOIST. HOIST WENT INTO ESTOP MODE WHEN D.C. MOTOR LOOP CONTACTER AUXILIARY SWITCH FAILED TO STAY CLOSED. SUDDEN STOP CAUSED EE TO FALL, BRUISING LEFTY KNE E AND ANKLE.
TWO SHAFT INSPECTORS RUNNING NEW TELEPHONE LINE BETWEEN 900FT & 1000FT LEVEL MINE INJURED INSPECTOR WAS OPERATING SIGNAL CARD WITH HAND OUTSIDE CAGE & WATCHING OTHER INSPECTOR RUN WIRE WHEN SK IP REACHED LANDING AT 1000FT LEVEL INJURED INSPECTORS HAND GOT CAUGHT IN RESTRICTED AREA OF LANDING HOOD & MANCAGE
7 OXYGEN ACETYLENE & ARGON BOTTLES WERE LOADED ON T HE AIRSHAFT CAGE TO BE TAKEN INTO THE MINE. THE BOTTLES SHIFTED & FELL OFF THE CAGE & DOWN THE SHAFT. EE WAS AT THE FOOT & INJURED HIMSELF WHILE MAKING AN EXIT FROM THE BOTTOM OF TH ESHAFT. EE WAS SITTING IN A JEEP & FELL WHILE GETTING OUT OF JEEP.
HOISTING MUCK OUT OF #1 SHAFT BOTTOM AT 1600\LEVEL WHEN SMALL PIECE OF LOOSE SALT FELL OFF SIDE OF SHAFT WALL HITTING EMPL ON HARD HAT.
