Employee was bolting 7 right punch when he felt something hit his elbow which resulted in a laceration requiring stitches.

EE was installing corner bolt, bolt wrench came out of head, struck him in upper right eye and forehead area. Injuries resulted in medical treatment with intravenous antibiotic treatment. This report is being generated under protest to terminate citation #9006078.

Employee was installing a bolt when the head of the bolt slipped off the drill pod and struck on the left hand.

EE was removing a canopy from a shuttle car. He was sitting under the canopy & when he removed a bolt, the canopy dislodged & struck him on the right shoulder. EE was taken to the hospital & was diagnosed with a compression fracture of the No. 12 vertebrae. He had a follow up visit with his family physician on 6/8/2010 and returned to work on 6/9/2010.

EE was welding on section feeder without a hood. Injury resulted in application of prescription medicines. This report is being generated under protest to terminate citation #9006077.

While installing an o-ring in a 777 rock truck tire, the hydraulic jack employee was using flew out striking employee in hand.

EE was grinding on c/m bit lug, a peice of metal bounced off under his safety glasses into his right eye.

EE was cutting a piece of Conduit with an angle grinder. The fringing wheel was pinched resulting in the grinder jumping out of ee's hand. The grinding wheel struck the ee above the right knee requiring 6 stitches.

Drill pot caught wrench and caused it to hit the employee's upper right leg and groin area.

Employee was grinding a piece of metal in a vise when the grinder kicked out and struck him in the mouth requiring stitches.

The roof bolter operator was guiding his drill steels into the drill head and inadvertently pinned his right index finger between the mine roof and the roof bolter head breaking right index finger.

EE was tramming roofbolter with his right hand on top of the canopy, when his fingers were mashed between the canopy and the mine top, resulting in an avulsed finger tip on his right middle finger and required stitches on his right index finger.

EE was installing roof bolt, bolt wrench swung around striking him on the right hand. The injury resulted in the application of a finger splint. This report is being generated under protest to terminate citation #9006064.

Employee was installing 6' roof bolts with a Fletcher roof bolting machine when he bent a 6' bolt. He went to straighten the bolt and accidently hit rotation handle spun the bolt striking his right hand.

Employee was aligning the bolt in the drill pod and accidently activated the rotation lever. His glove caught on the roof bolt and twisted his hand. He was taken to Pikeville Medical Center and x-rays revealed a fracture to his left middle finger.

Two employees were drilling hole in floor with hydraulic drill. The drill stalled and the handle of the drill hit the injured employee in the left knee.

Tramming drill, he ran over a ridge in the road; drill dropped hard after crossing ridge straining his back and neck.

Had hand under metal cutting with torch. When ball released metal dropped and punctured hand (7 stitches) bolt was holding 2 pieces of metal together.

Moving excavator to lower level, when it started sliding to the left side. It came in contact with loose soil and sunk up, letting the machine roll over on its top.

Building loader pad, rock pushed from above hit his dozer jarring operator causing back strain.

Struck by a rock while grading road, throwing operator into armrest and steering wheel causing back pain.

EE was installing a roof bolt on rib area. He slipped on loose coal, while trying to recover his balance, he grabbed the lower boom lever and pinned his foot. X-rays revealed 2 broken toes.

Dozer topped short incline; when front of dozer came down on other side it jarred operator.

Drilling hole when finger got caught between drill pot & pinner steel amputating tip of left ring finger, fracturing middle finger & lacerating indes finger.

While grinding on a shuttle car bed, a piece of dust or metal entered the left eye.

EE was drilling a hole for a roof bolt installation. Drill steels stuck together. While trying to get them apart, the bottom one came loose and struck him in the face. (nose)

Ee was setting up to put bolt in. As he was lowering the drill pot his foot slipped on loose coal and he dropped drill pot on his rt. foot. He was taken to hospital x-ray showed no fractures. He went to another dr these x-rays also found no fractures. He went to a 3rd dr. These x-rays show a hairline fracture on 2/28/07.

Employee was putting bolt up fast feed pot, he was down on left knee sitting on gob when his knee slid and got caught under pot.

While drilling a hole to insert roof bolts, a piece of stone became incircled around the auger. The rotation caused the stone to come apart and a piece struck him in the upper lip.

EE was helping install a cutting edge onto a bulldozer blade, when the lifting eye that was being used to lift up the edge broke. The cutting edge landed on his foot.

Employee was inserting a 6'bolt when it bent. He attempted to pull the bolt out & accidentally hit the rotation lever causing the bent bolt to strike his right hand & wrist. EE is on restricted duty.

EE was attempting to clean mud from drill pot using a lenght of wire. As he rotated the drill pot the wire wrapped around his left thumb causing a severe laceration and avulsion.

Sitting in the deck chair of the roof bolter and tramming it, he got his head caught between the canopy and coal rib. No information on individual received until 10/5/05.

EE was kneeling with his back to the bolting machine, extending the line curtain. The bolter trammed forward running over his foot - The operator alerted him that the machine was going to move, EE thought his foot was clear. No lost time - on restricted days.

EE was installing a cable bolt, positioning the bolt plate so it would not hit T-bar. He got his right index finger pinched between the plate and the roof.

He was tramming the continuous miner, when his head came in contact with a roof bolt plate and lacerated his left ear.

EE WAS HELPING TAKE A ROOF BOLTER TO THE SECTION AND CUT FING ON THE SHARP EDGE OF THE T-BAR

EE was in process of installing 6' bolt, as he picked up the bolt bent pinching hand between plate & bolt - Cause- safety awareness and improper hand positioning. Protective equip. not a factor, regulations not a factor.

EE was putting the drill steel against the top to drill when a piece of draw rock fell striking him on the left hand.

EE WAS OPERATING THE CONTINUOUS MINER WHEN A PIECE OF DRAW ROCK FELL, STRIKING EE ON THE MOUTH AND CHIN.

EE WAS TRYING TO STRAIGHTEN THE ROOF BOLT BACK OUT AND IT KICKED BACK, LETTING THE ROOF BOLT PLATE STRIKE HIS WRIST.

He was operating a highwall drill, when the side glass busted and a piece of glass flew out and went into his eye.

While operating the continuous miner a rib rolled off and struck him on the right shoulder and right side of his back.

While he was tramming the continuous miner the miner cable caught his feet and leg knocking him down and twisting his right knee.

EMPLOYEE INFORMED HIS SUPERVISOR AT QUIT TIME THAT HE HAD PAIN IN HIS LOWER BACK & LEFT LEG. THE PAIN STARTED SOMETIME AT THE BEGINNING OF THE SHIFT & AS THE NIGHT WENT ON IT GOT WORSE. HE STARTED MISSING WORK BECAUSE OF THIS ON 9/2/04 & IS STILL OFF WORK.

Ee stated that he was lifting the hood on a dozer with a spreader chain and the crane on his truck. the hood swayed and when he tried to steady it with his left hand his hand became pinned between the hood and radiator on the dozer

Employee of Beaver Creek Clearing was cutting a tree, when another tree struck him from behind.

EE, DRILL OPERATOR, SET THE JACK PAD OF THE DRILL ON A ROCK. THE PAD SLIPPED OFF THE ROCK, JARRING EE. (ON 11-17-03 EE CALLED THE MINE CLERK, & STATED THAT THE OCCURRENCE AGGRAVATED A PREVIOUS INJURY.)
