While drilling a roof bolt hole the bolter operator bent the starter steel. When the steel was removed from the roof the bent steel rotated into the operator striking him in the neck.

Mostrando los 11The employee attempted to start drilling a roof bolt hole and started rotation instead of thrust. The roof bolter wrench being used on the starter steel was bent which caused the wrench to whip into the employee's forearm.
Cutting metal and hot slag got into his pant leg and down into left shoe burning foot.
The employee stepped out of the cab of a Wagner 20X shield hauler and then reached into the cab and operated the fork down function. The machine articulated with the employee in the hinge point.
In attempting to remove a shield from the recovery face the shield was lowered to the transport position. When the shield was lowered the side seal broke loose and fell. The side seal struck the employees right knee.
While attempting to drill a roof bolt hole the employee reached up to re-align the roof mat with the drill hole. The mat moved and pinched the employee's finger between the roof mat and the roof mesh.
The employee stood on the boom of a roof bolter to install a belt hanger. When the employee slipped and fell he reached out to catch himself and disabled the hydraulic lock out. He also activated the boom lower function. The drill mast lowered and caught the employee's foot.
The injured person (driller) screwed in the pulling plug to add another piece of drill pipe while drilling. He did not notice that it had vibrated loose prior to picking the pipe up and it fell from approximately 15' hitting him on the shoulder. He has a broken left fore-arm and cracked four ribs.
While attempting to pull a coupler off of a gearbox shaft the hydraulic puller coupler link broke. A piece of the link struck the employee.
The employee attempted to insert a truss bolt into the bolthole. He bent the bolt and when he attempted to spin the bolt the end whipped into his face.
The injured person was helping to hang ventilation tubing. As he lifted the tube, his footing slipped and he tried to catch himself by reaching for the roof bolter canopy. The canopy was being lowered at the time and his left index finger was caught between sections of the canopy mast, severing the tip of the finger.
