On 8/1/13 the employee reported to the mine operator that while the employee was loading vent tube into a Ram car he felt a pain in his lower back. The extent of the injury is pending further investigation.
While arriving for work the employee attempted to walk down the walkway from the parking lot to the bathhouse. Slick soled shoes caused the employee to slip and fall.
The employee stated that while packing a roll of curtain down the #4 lower b slope he slipped on the floor and fell. The employee stated that he suffers back disk degeneration.
While attempting to hang a length of water pipe from the mine roof the ladder used by the employee tipped over. The employee fell and landed on the ladder.
On 1/3/14 the employee elected to have his left little finger removed. The finger was injured on 5/28/13 when the employee removed the U-bolts from the axle of a welder trailer and his hand was pinched between the trailer frame and the axle hub. The ligament damage to the finger has not healed properly
On 9/16/13 the employee had surgery to reduce a hernia that may have been started on 3/14/13 when the employee was lifting bags of Tek-Seal onto the pump for mixing.
On 3/15/13 the employee failed to report to work as a result of injuries that may have occurred on 3/10/13. The employee claims to have suffered a lower back strain while packing a crib block to load it into a pickup truck.
While drilling a roof bolt hole the bolter operator bent the starter steel. When the steel was removed from the roof the bent steel rotated into the operator striking him in the neck.
The dozer operator on shift was pushing coal to a stockpile reclaim feeder when the dozer became stuck on the slope into the feeder. Poor coal pushing practices created the unstable slope.
The employee was traveling outside in a mantrip. The mantrip traveled into a hole in the roadway. The employee was bounced off of the seat and onto his lunch bucket.
The employee attempted to start drilling a roof bolt hole and started rotation instead of thrust. The roof bolter wrench being used on the starter steel was bent which caused the wrench to whip into the employee's forearm.

While the employee was attempting to cut a piece of conveyor belt with a utility knife the knife slipped and cut the employees leg.
On 3/21/12 the employee underwent surgery for a torn ACL that appears to have occurred on 1/12/12 when the employee stepped down off of an idle conveyor belt to the mine floor and twisted his knee
When the employee attempted to hang a ventilation tube the end of the #9 wire hanger flipped up under his safety glasses and scratched the left eye.
While attempting to drill a roof bolt hole the employee reached up to re-align the roof mat with the drill hole. The mat moved and pinched the employee's finger between the roof mat and the roof mesh.
A small methane ignition occurred in the face of the #5 entry of the 2nd West Mains when a 5-foot mechanically anchored roof bolt was installed in rock at the top of the rib and methane in a roof crack was ignited by friction or sparks.

The employee stood on the boom of a roof bolter to install a belt hanger. When the employee slipped and fell he reached out to catch himself and disabled the hydraulic lock out. He also activated the boom lower function. The drill mast lowered and caught the employee's foot.
While attempting to check the oil in an engine the employee did not secure the engine hood. The hood fell closed and struck the employee in the head. The employee struck his forehead on the machine frame.
The injured person (driller) screwed in the pulling plug to add another piece of drill pipe while drilling. He did not notice that it had vibrated loose prior to picking the pipe up and it fell from approximately 15' hitting him on the shoulder. He has a broken left fore-arm and cracked four ribs.

The employee attempted to help lift a belt line starter controller box.
EE shot me with a homemade blow dart. It was made out of a 4 ft drill steel, ear plug & 9 wire 3-4 ". The dart penetrated approx 2" into my left buttocks. The wire was greasy & dirty and left swelling and bruising. I do have pictures of the wound and kept the bloody underwear with the puncture hole.

The employee was riding in a mantrip traveling into the mine. The mantrip hit a pothole in the road. The employee bumped his head on the roof of the trip. Upon investigation the incident appears to have avergatted an existing condition.
The employee was attempting to install a drive line in a Ramcar. In an attempt to align the u-joints the machine was moved. The movement of the driveline lacerated and fractured the employee's left ring finger.
Audiogram testing and medical evualation of the employee indicates a reportable threshold in the employee's hearing without a medical cause.
As the employee was attempting to hang a ventilation tube, a slicken sided piece of roof rock 7-8-inches in diameter and 4-inches thick, fell and struck the employees hard hat. The hard hat cut the employees forehead.
On 9/3/10 the employee required knee surgery from an incident which occured on 7/23/10. The employee stated that he was carring two cans of oil and stepped on a piece of coal. the action twisted his knee resulting in torn knee ligament
