Mostrando los 11Employee was checking his grinder and putting the handle to the other side and pushed the trigger and it started and came in contact with his left index.
Employee was using a Metabo Grinder. The Metabo bounced back after coming in contact with the 1 1/2" pipe being cut. Metabo blade contacted employees middle left finger.
Employee was drilling holes into a metal PCL Cabinet, drill caught twisting employees right hand.
Using a grinder, grinder bound up, kicked back & cut knuckle on right index finger
Non-compliance of company & mine site safety policies in regards to equip. & PPE. EE while wearing safety glasses, did not don face shield while using an elec. hand grinder that he had remove the handle from. While prepping a weld, 1/8" abrasive grinding wheel got caught between pipe openings, kicked back into EE's face, causing 1' cut to his face just outside of his left eye.
The employee had been welding and grinding when it was noted that her right eye was red.
Cutting a 2x4 with Skill saw by hand, not on saw horse. Grabbed 2x4 in different position, pinched the blade. Saw kicked back and lacerated hand.
Worker was cutting 2X4 lumber with a circular saw. The lumber was lying on a saw horse unsecured. Worker made the 1st cut without difficulty. The 2nd cut started and the saw got stuck in the lumber causing it to jerk back cutting his left ring and little finger. Lacerations to both fingers needed 7 stitches each to close the wounds.
Laceration to forehead (above left eye) - was helping coworker ream holes in header with an air gun, coworker lost grip on gun, gun spun around and hit employee in forehead.
Employee was using an airline to unplug a line, he uncoupled the line and Splashed materials into his face and eyes.
