Employee was core drilling. The rig shifted slightly while drilling, which caused the barrel to catch the mud pan as it was being extracted. The mud pan struck the employee on the left knee.

Mostrando los 11Two employees and a contractor crane operator back a crane into position. One employee was in front and the other was in the back. The crane operator was backing and was not aware the employee in the back had placed themselves behind the crane and the employee was pinned between the hand rail and the crane injuring their leg.
The EE was operating a 14 ton manitex boom truck in a narrow road between B-Tanks preparing to move an empty MCC panel. EE was unable to extend the main outriggers due to width of road. EE didn't like the position and began to pull boom back in and the truck begin tilting, so the injured worker jumped off the boom truck onto the ground.
Boom truck was not setup properly due to limitations caused by concrete barriers. After the boom truck flipped on its side due to outriggers not being deployed an all stop was not called to access the hazards presented by the equipment failure. After attempting to safely stabilize the hoisted load, consideration was not made for a safe clear escape path.
EE was replacing a pneumatic actuator that controls a knife gate. EE had trouble inserting a pin that links the actuator arm to the knife. The pin became stuck when inserted about halfway. EE tried to rotate the connecting arm to align the pin. When EE rotated the arm, it activated trapping EE's left thumb between the pin and the knife. EE received a cut that required stitches.
Miner was grinding on a roller bracket in control room. Grinder kicked back and cut the top of hand under the thumb. Miner is back at work with restrictions, no days missed. EE will have outpatient surgery on 6/15/21.
EMPLOYEE CAUGHT RIGHT HAND MIDDLE, RING AND PINKY FINGERS IN AIR ACTUATED KNIFE GATE VALVE ALMOST SEVERING THEM REQUIRING SURGERY.
College Summer Work employee was cutting a 2x8 board with a skill saw when the board dropped - EE reached to catch board striking saw cutting EE's left hand index, ring and pinky fingers. EE was taken to McDuffie ER then transferred to the Doctor's Hospital in Augusta, GA where surgery was performed to repair nerve and tendon damage.
Employee was welding in the shop. When employee squatted down employee's right leg hit an open welding rod can. The can cut employee's leg. Employee was taken to ER and received 6 stitches and is back at work with no restrictions.
THIS IS A CONTRACTOR EMPLOYEE WORKING ON OUR MINING PROPERTY WHO WAS PRESSURE WASHING PIPE WHEN EE STEPPED INTO A HOLE LOSING BALANCE. EE LOST CONTROL OF PRESSURE WASHER WAND AND IT HIT LEFT LEFG SHIN CAUSING 2ND DEGREE BURNS.
EMPLOYEE WAS WELDING AND OTHER CONTRACTORS WERE ABOVE EMPLOYEE, GRINDING. EMPLOYEE WAS WEARING PPE BUT A PIECE OF RUST BECAME EMBEDDED IN THE RIGHT EYE.
