EE was aligning strap with suction flex when left thumb got caught and crushed between the strap and suction flex as it was being lifted.

Mostrando los 19Employee was arc gouging bulkhead weld seam off Matrix pump sled. While cutting the last of the weld seam the wall shifted pinching a finger between the floor and head of the gouging torch. Employee received laceration to right second finger. Employee was transported to Nutrien site first aid office and then ER and returned to work on 08/22/25.
EE began experiencing pain in right shoulder while working on the control cards on the #5 dragline. EE states the repetitive movement & the work to get the cards in and out of the slots caused the problem with the shoulder as the work was difficult & repetitive. EE states decreased range of motion due to pain. Denies previous problems with shoulder & is unable to work due to this.
Mine conditions were favorable with no adverse weather conditions.
On 11/17/23 while taking fitting off a hydraulic power ram, as it came off, the fitting flew off striking EE on the face in upper left eye area, along with hydraulic fluid, just above safety glasses. EE reported to clinic with pain, swelling, and 3 small lacerations to area above left eye. EE was referred to LCMC for evaluation and treatment. EE is off work until further notice.
Mechanic (M1) was tasked with working with an automated welding machine. During welding activities M1 began experiencing pain/itching in both of their eyes. M1 was diagnosed with flash burns and received prescription medication for both eyes.
Operator struck a dead tree with bucket. Top of dead tree struck Excavator, bounced and broke top glass. Part of tree top entered cab but did not touch operator. Operator closed eyes and did not see what happened. Operator stated may have lost consciousness/doesn't remember. Two medical professional examined operator, no sign of head trauma or loss of consciousness.
EE was operating a D8 bulldozer on pebble pile and backed off a drop-off of the pile. EE states: jarred neck & has tightness & soreness on right side of neck. Initially report only but presented to clinic via phone having increased pain from neck down to leg. EE was given restrictions and placed on modified duty.
Crew was in process of replacing dump cables at #2 Dragline. Employee was cutting U-bolt that holds the end of cable to the spool, when cable was cut free employee was in the line of fire & struck in hard hat & right side of face causing facial fracture, 2 chipped teeth and mild concussion.
EE was pushing product from holding bin bypass discharge up to top of rock pile when dozer fell in open draw hole while turning the corner with full blade of product. EE reported to clinic 9/29 with back & wrist pain, follow up visits with onsite Dr. EE sent to neck surgeon on 12/15, physical therapy was prescribed for neck pain.
Shift began on 2/3 at 6 pm. Employee was looping a chain under a shaft on bolt head in an attempt to roll the shaft to remove bolts. While holding the hoist control in right hand & holding the chain in left hand EE hoisted up & chain slipped off bolt head & struck EE on left index finger cutting it open. EE was sent to LCMC for evaluation, wound cleaned & bandaged.
Operator was digging into an Ore stockpile loading a screen plant. At approximately 2:26 a.m. the Ore pile peak shifted from East to West and slid down striking a 336 Excavator and a Screen Plant. During the slide the Ore entered the front glass of the Excavator covering the legs, waist and lower chest of the operator.
Two Maint. employees were in the process of removing a wedge from drag cable socket at #3 D/L with the use of hydraulic ram. When employees began to apply pressure to the ram, a coupling on hydraulic cylinder failed & struck one employee in left forearm. Employee was transported to Medical Center for evaluation, EE had surgery and was placed out of work.
Employee was placing rope over a tooth on the bucket of an excavator. While employee was positioning rope, the excavator operator started to curl the excavator bucket. This movement caused two fingers on the employee's right hand to be caught between the bucket tooth and the rope.
HEO operating D10 bulldozer on mill rock pile. Suspended air seat had leaked down. While backing tractor, felt abrupt jarring of back as EE traveled over change of elevation. EE reported to clinic on 4/20/20, treated on site, later had x-rays & MRI. Referral visit to Ortho 5/8/20 with restrictions placed out of work.
Employee was cleaning out the log washer discharge chute with a cleaning rod while it was in operation. The rodding tool impacted a washer flight and started being pulled into the machine. This caused the employees left thumb to be pinched between the handrail on the secondary flume level and the rod.
While pushing out a pin in the boom dipper with a 200 ton hydraulic jack one of the spacers came out and struck the employee in the head.
Employees were attempting to close a pneumatic knife gate valve that was stuck in open position, using 2 hydraulic jacks to apply pressure on either side of valve shaft. While pressure was applied, 4 bolts at base of actuator failed causing the actuator to strike one employee on lower right side of abdomen. The employee was transported to clinic & taken to local medical facility.
Crane was being used to remove pump impeller, placing it in bed of truck. Rigging failed resulting in impeller falling into bed of truck. The impeller rolled off bed of truck and struck employee in right leg and foot. Employee transported to local medical facility.
