Miner was using an impact wrench to remove the bolts from the tail bearing. When they removed the last bolt, the bearing jumped up, trapping a hand between the bearing and the take-up guard.

Mostrando los 4Employee was utilizing a grinder to cut a bearing sleeve. This task was necessary to replace the bearing on the head pulley of the C1 belt. Midway through the task, the grinder unexpectedly kicked back towards the employee, causing the grinder's disk to make contact with the employee's left forearm resulting in a laceration.
Injured employee (IE) attempted to provide maintenance on the PU 6 pump belt. As other employee was performing that task, IE removed the keeper that was in place and opened the guard gate to inspect the tension of the belts. The motor immediately began rotating as IE placed hand on the belt. This caused IE's left hand to pull into the belt, causing injury to IE's fingers.
Employee was using a cutting torch, cutting 4 inch square tubing when the flame of the cutting torch penetrated through employee's left welding glove.
