While installing a BFR door to the longwall stage loader a water jack became dislodged striking an employee across the lower legs. The water jack was used to keep tension to the BFR door in order to connect the ram jack pin to the door.

Reportable only due to sutures. Employee was using a grinder to cut some bolts off when the grinder slipped and cut EE's left index finger.

Employee was bolting down a pinch clamp on H-1 belt when the hammer drill slipped pinching left thumb between the drill and clamp.

Reportable due to sutures. Employee was using a drill press to drill a hole in a metal plate when the drill bit caught the plate and mashed left ring finger.

Employee was installing a roof bolt when it got stuck. The employee was spinning roof bolt trying to remove the roof bolt when it dropped out of the roof causing the roof bolt plate to hit the employee in the face area.

Employee was helping put side plate on the # 122 Raw Coal Screen when the huck bolt gun twisted and mashed his right ring finger between the huck bolt gun and the side plate.

Reportable due to sutures. Employee was removing drill steel from hole when the drill steel slipped and struck employee on left ring finger.

Reportable Only due to sutures. Employee was installing a roof bolt when his right hand got caught between the roof bolt plate and the drill steel guide.

Employee pinched his left index finger between the end of the bolt wrench and the frame of the drill head while he was roof bolting.

Employee was pushing up a roof bolt with the roof bolter when the tip of his right index finger got caught between the end of the roof bolt and the top.

Employee was helping move the miner out of the face area and as the miner was backing out of the entry it hit a slider tubing. The slider tubing fell hitting the employee on the top of the head.

Reportable only due to sutures. As the employee was using a grinder to cut a bolt out on the drive unit his hand slipped and the employee cut the back of his right hand on a sharp piece of metal.

Reportable only due to two sutures. Employee was removing a canopy post from a shuttle car when a piece of metal became embedded in left forearm.

EE was installing the balance ropes underground, there was a lot of airflow coming in which was blowing debris all around, EE received debris in eye.

Employee was cutting coal toward the longwall tailgate when a rock fell off the shear plow hitting employee on his right side and landing on his right ankle.

Employee was plugging up an air greaser when he felt a pain and pop in left elbow.

EE was operating a spader (60 lb) when the bit became stuck. As he attempted to remove it, he felt pain in his lower back.

Employee was burning ludges off conveyor on a shuttle car and when ludges became dis-lodge a flame came out and burned the left side of employee face.

Employee was attempting to unstop a dust hose on the roof bolter, when employee struck the top part of left hand with the bolt wrench.

EE was working at a seal site in B- bleeders and he had gunite/concrete in his eyes.

miner was roof bolting and his drill steel became stopped up. a co-worker was striking the steel attempting to unstop it missed and struck the miner in the face.

Drill steel hung in hole, He pulled on steel with one hand, had other hand on jaws. Co-Worker accidently hit controls and pinched employee's hand and fingers.

Miner was struck on the foot by a rock that came off the shearer.

Miner was struck on the hand by a rock thrown by the tailgate shear drum.

miner was manually pulling a shield forward top caved behind shield shoved the shield forward.

Miner was taking a tie-rod end off of tractor. Heating the tie-rod end caused the ball to come out of the socket under pressure striking the left index finger and right lower leg.

A piece of rock fell off the miner while place changing. The rock broke apart upon impact on the mine floor. One of the pieces that broke off from impact on the mine floor struck the employee on the right side of the face. He received sutures.

Employee was operating roof drill installing roof bolts. while hauling roof bolt, piece of cap rock fell. As worker was moving out of way He bumped control lever which caused head to move and pinch finger between bit wrench and roof bolter.

While roof bolting, Employee was installing bolt, Bolt missed hole, bent and caught finger on steel guide.

EE was cutting a timber. Saw jumped out of cut and ran across back of his hand.

Employee was pulling stuck drill steel out of a hole & felt pain in his shoulder. *EE began missing work 11/21/06*

WHILE SCALING THE EAST SIDE PIT WALL WITH DEMAG H285 HYDRAULIC EXCAVATOR S/N 78111 THE CAB CAME OFF AND FELL SEVENTEEN FEET TO THE GROUND.

Employee was pulling shields when a rock came off shearer drum and landed on his foot.

EE WAS TIGHTENING CST GEARBOX BOLT WITH IMPACT WRENCH. SOCKET SLIPPED OFF BOLT HEAD AND CAME LOOSE FROM WRENCH. SOCKET FLEW UP AND STRUCK EE ON THE CHIN.

Employee was kneeling on a beam, welding - he twisted and felt pain in his lower back.

Checking roof drill hydraulics with leavers and hand was pinched by drill jaws.

While drilling a hole for monorail hanger, the drill steel broke off just above the chuck letting the drill steel strike the person.

Employee started a 58 hp water pump. The rush of water into the flexible discharge hose cause it to raise up suddenly, striking and jamming his right ring finger. Employee started missing work on 4/4/05 due to surgery.

EMPLOYEE WAS IN PROCESS OF TASK TRAINING MINER UTILIZING A COBRA AIR DRILL TO INSTALL RIB BOLT. THE BOLT BROKE HOLDING DRILL SHANK & STRUCK EE IN THE FOREHEAD CAUSING A LACERATION.

A SECTION OF DRAW ROCK FELL BETWEEN BOLTS STRIKING EMPLOYEE ON LEFT SHOULDER, ARM AND BACK AREA.

A rib rolled out striking injured in both legs and lower back area.

EE WAS ROOF BOLTING WHEN A ROCK FELL, STRIKING VENTILATOR TUBING WHICH FELL AND STRUCK EE ON HIS SHOULDER.

Employee was bolting top rib. Rib rolled off striking back of leg.

EMPLOYEE STATED HE WAS REMOVING PIN FROM BOOM SWING JACK ON ROOF BOLTER. REACHED UP TO CATCH PIN AS IT WAS NEAR OUT, AS SOON AS PIN CAME THROUGH JACK EYE THE BOOM SHIFTED TRAPPING FINGERS BETWEEN EDGE OF PIN AND FRAME OF MACHINE.

Coal/ rock fell from roof striking hard hat, rt shoulder and right arm, employee received 8 stitches in right arm.

EMPLOYEE WAS IN PROCESS OF ATTEMPTING TO SET DRIVE DRILL AGAINST ROOF TO BEGIN DRILLING WHEN A PIECE OF DRAW ROCK FELL OUT STRIKING HIM ON HARD HAT & NECK. EMPLOYEE STARTED MISSING WORK ON NOV 24, 2003.
