Employee was sitting on a pipe that was suspended with chain falls. He was cleaning some of his tools. The pipe shifted when someone else hit it and smashed his foot between the pipe and the floor.

EE had been pulling on a chain fall for several days and on this day, he started to complain about his arm hurting and going numb. We found out later that he had a pre-existing arm injury we were unaware of.

Rock fell from overhead belt, bouncing off the floor of backhoe the employee was operating (canopy is in place and in good working order). Rock bruised right ankle.
The employee stepped on a lump of coal causing him to trip over a pipe and fall to the ground. He tried to catch himself with his left hand, causing injury to the left wrist.

This is to report medical treatment only. While attempting to fasten a regulator door on the #1 run of mine feeder, the employees right ring finger was struck by a rock that fell from the top of the hopper to the feeder. This resulted in a laceration requiring (4) sutures and a frature of the distal phalange of the 3rd finger, right hand.
Employee was kneeling on a beam, welding - he twisted and felt pain in his lower back.
Removing a piece of chutework, the chute became lodged in the hole from which it was being removed from the plant, the ee pryed the chute to free it. Due to the forces of the rigging the chute sank into the ee's left foot when it was freed.
