Employee was using a stationary grinder to smooth off a piece of metal that had been cut. The piece of metal kicked and the employee's right thumb contacted the moving grinding wheel causing a laceration. The laceration required two stitches to close.

Employee was attempting to cut some holes in a plastic barrel using a reciprocating saw. As he was cutting one of the holes the saw kicked back. The kick back of the saw resulted in his pointer finger on his right hand contacting the saw blade causing a laceration that required five sutures to close.

Employee was in the process of installing a bolt in the mine floor to secure a belt drive. As he was rotating the bolt, his left index finger got pinched between an air hose and the frame of the drill causing a laceration that required stitches.

Employee was attempting to move a section of miner cable, as he reached down to lift the cable the cable was pulled by a piece of mobile equipment. This caused the cable to flip up and strike employee across his right knee knocking him down. This caused injuries to his knee that may require surgery.

The employee was using a hand drill to make a hole in a section of plastic pipe. While holding the drill, the drill bit stuck and jerked his arm and shoulder. Employee continued to work until June 2, 1010 at which time he went off work. Accident did not become reportable until June 2, 2010.

Miner was walking along the longwall face when he slipped and his foot went between the relay bar and toe of the shield and was pinched. This involved fractures of the toes on his left foot a nd also the loss of skin on the first two toes of the foot. The bones of the first two toes were still attached but toes of foot were surgically removed after accident.

Employee was in the process of cutting a section of discharge line with a saw when his finger slipped off the finger guard and got pinched between the saw housing and the blade retaining bracket causing a laceration to his little finger.

EE, a longwall propman attempted to lower a shield on the longwall face. A valve soleniod malfunctioned, which caused the pull ram to activate prematurely, tightening up a chain which broke and a link from the chain struck EE on the leg and caused a hair line fracture.

While operating the shear a piece of rock flipped off the cutting drum and struck employee in the back of the hand causing a fracture.

Employee was preparing to drill a hole to install a resin bolt and as he reached to raise the drillhead he hit the wrong lever and closed the steel guides on his fingers.

Employee was drilling a hole when the drill steel came loose and struck him in the face causing a laceration to his nose.

Employee was using a lathe to reduce the size of a roll pin to repair a hinge when his gloved got caught between a piece of emery cloth and the pin, pulling his hand into the pin. This incident caused lacerations, a fracture and 2 dislocations.

Employee was installing a resin roof bolt and as he went to raise the drill head on the bolter he hit the wrong lever and closed the steel guides on his finger.

The injured employee was behind the continuos miner as the miner operator was moving the miner to the next face, the slack from the miner cable caught the employees legs and knocked him down. The time difference from the reporting date and date of injury is due to waiting for the employee to be scheduled for a MRI suggested by the attending physician.

Coming out of scraper cut. Had can too low and hit a bump. This caused employee to lunge to the left. Hit head on rops causing laceration to back of scalp. Required five staples.

EMPLOYEE WAS HELPING ON THE MINER TENDING CABLE WHEN A PIECE OF RIB 12"X 12"X 32" ROLLED OUT LANDING ON TOP OF HIS FOOT.

OPERATING ROOF BOLTING MACHINE. INADVERTENTLY HIT DRILL GUIDE CLAMP LEVER & CLOSED GUIDE ON FINGER CAUSING LACERATION OF THUMB.

THE EE WAS ADJUSTING THE DRILL HEAD ON A PAN BOLTER TO INSTALL A ROOF BOLT. THE OPERATOR HAD STEPPED OFF OF THE DRILLING PLATFORM WITH ONE FOOT & PLACED IT BETWEEN A SPRING BAR AND THE SKID P LATE FOR THE DRILL HEAD WHEN THE DRILL HEAD WAS MOVED IT PINCHED HIS FOOT BETWEEN THE TWO AREAS.

EE CLOSED THE DRILL STEEL GUIDES ON HIS FINGER WHILE HE WAS OPERATING A ROOF BOLTER.

O&K HYDRAULIC SHOVEL BUCKET MADE CONTACT WITH HAULTRUCK BOX, UPPER REAR LEFT SIDE.

EMPLOYEE WAS USING A HIGH PRESSURE HOSE TO WASH FACE SHIELDS. HE TURNED THE SHUT OFF VALVE THE WRONG DIRECTION AND THE END OF THE HOSE FLIPPED UP AND HIT HIM IN THE BACK OF THE HEAD.

EE WAS ATTEMPTING TO INSTALL A ROOF BOLT, WHEN THE BOLT GOT CAUGHT AND BENT AND SMASHED HIS FINGER BETWEEN THE DRILLHEAD STEEL GUIDE AND THE BOLT.

EMPLOYEE WAS OPERATING A ROOF BOLTER ON THE LONGWALL FACE. HE WAS STRUCK BY SOME ROOF BOLTS THAT WERE IN THE LONGWALL PAN WHEN IT WAS STARTED UP.

OPERATING SHIELD. AS THE SHIELD WAS BEING PRESSURIZED TOWARDS THE ROOF IT CAUGHT ON THE SIDE SHIELD & A PIECE OF METAL SHEARED OFF. THE PIECE OF METAL HIT HIM IN THE HAND CAUSING A CUT TO HIS FINGER.

EMPLOYEE WAS INSTALLING A ROOF BOLT AND THE BOLT WRENCH POPPED OUT OF THE STEEL GUIDE AND SMASHED HIS THUMB BETWEEN THE GUIDE AND WRENCH.

EE WAS INSTALLING A RESIN ROOF BOLT, THE BOLT GOT CAUGHT IN THE HOLE, FLIPPED BACK AND STRUCK EE IN THE HAND.

EE WAS TRAMMING ROOF BOLTER ALONG FACE. THE MASKGOT STUCK ON SOME LOOSE COAL. OPERATOR ROTATED MAST TO CLEAR MATERIAL. A WATER HOSE HIT CONTROLS& CAUSED MAST TO ROTATE BEYOND DESIRED DISTANCE. MAST PINCHED EE BETWEEN MAST & BOLTER FRAME.

THE EE WAS OPERATING THE SHEARER AND WAS STRUCK BY A PIECE OF COAL ON THE SHOULDER.

EE WAS CLEANING OUT A 4" PIPE WITH AN AIR PIG. WHEN THE AIR WAS APPLIED THE PIPE SHIFTED AND THE EMPLOYEE'S HAND WAS CAUGHT BETWEEN TWO SECTIONS OF PIPE.

EMPLOYEE WAS HELPING ANOTHER EMPLOYEE REPAIR A BELT TRANSFER CHUTE. THEY WERE DOING A LOT OF CUTTING AND WELDING DURING THE SHIFT AND THE EMPLOYEE BURNED HIS EYES.

THE EMPLOYEE WAS SPOTTING SHIELDS BETWEEN 40 & 50 IN AN AREA WHERE THE FACE WAS HIGH. WHILE PULLING AND SETTING THE SHIELDS A BOUNCE OCCURED AND THREW THE BRETBY OUT OF THE PAN AND THIS STRUC K THE EMPLOYEE IN THE RIBS. THE SHEARER WAS AT THE HEADGATE.

EE WAS SETTING THE PRESSURE ON A COMPRESSOR & THE AIR TANK FAILED CAUSING CONTUSINS TO R SHOULDER & ABDOMEN

EE WAS PREPARING TO GREASE A BEARING ON A VIBRATING SCREEN. HE DISCONNECTED THE POWER POWERING FDOWN THE SCREEN. HE HAD FAILED TO BRING HIS FLASHLIGHT UP ON THE PLATFORM. AFTER WAITING FOR A P ERIOD OF TIME FOR THE DECK TO STOP, HE ATTEMPTED STO USE HIS FINGERS TO LOCATE THE FITTING BY REACHING THROUGH THE ACCESS PORT IN THE GUARD. THE ECCENTRIC SHAFT WAS STILL MOVING, TRAPPING HIS

EMPLOYEE WAS CUTTING PIPE WITH A CUT-OFF SAW. WHEN EMPLOYEE TURNED PIPE, THE BACK OF HIS RIGHT HAND STRUCK THE BLADE AND HE RECEIVED A LACERATION. MSHA REPORTABLE. LIGHT DUTY-SUTURES.

THE SHEAR WAS CUTTING THROUGH THE TAIL GATE. THE TAIL GATE DRUM OF THE SHEAR CAUGHT A TIMBER AND THREW IT INTO INJURED.

THE NEW LONGWALL FACE EXPERIENCED IT'S FIRST CAVE. THE AIR FORCED FROM BEHIND THE SHIELDS, PUSHED EE DOWN THE FACE.

RIB BLEW OUT STRIKING EMPLOYEE ON THE LEFT LEG.

A FOREIGN OBJECT GOT INTO LEFT EYE WHILE EMPLOYEE WAS GRINDING. HE WAS WEARING SAFETY GLASSES AND A WELDING HOOD AT THE TIME OF THE ACCIDENT. THE FOREIGN OBJECT WAS IMBEDDED IN THE EYE.

CHUNK OF COAL THROWN OFF THE REAR DRUM OF LONGWALL SHEARER. CHUNK HIT HIM ON BACK OF LEG.

EE WAS OPERATING THE SHEARER WHEN A BOUNCE OCCURRED THAT THREW COAL OVER THE SHEARER AND ALSO CAUSED THE BRETBY TO COME OUT OF THE CABLE TROUGH AND ONE OF THESE 2 STRUCK HIM ON THE HAND AND SM ASHED 2 FINGERS. INDEX FINGER WAS AMPUTATED AT THE 2ND JOINT.

EE WAS CUTTING OFF A SECTION OF EXHAUST PIPE AND A PIECE OF METAL GOT IN HIS EYE.

EE WAS OPERATING CONTINUOUS MINER WITH REMOTE CONTROL WHEN A SECTION OF RIB ROLLED OUT AND STRUCK EE, KNOCKED HIM DOWN AND LANDED ON HIS LEGS THE EE SUSTAINED A FRACTURED LEG.

BULLHOOK OFF OF 2029 DOZER BORKE AND CAME THROUGH 2030 DOZER BACK WINDOW STRIKING HIS RIGHT SHOULDER. MSHA REPORTABLE LOST TIME

REPAIRING ROTARY CRUSHER. DURING LIFTING OF SCREEN WITH CRANE, SCREEN SLID AND HIT HIS ARM, SHOULDER, AND HEAD.

EE WAS BERNT OVER HANGING A WASH DOWN HOSE ON FACE CONVEYOR PAN. FACE BOUNCED AND A PECE OF COAL STRUCK EE IN THE HAND.

PIECE OF COAL CAME OFF SHEAR DRUM & HIT HIS FOOTCAUSING A BRUISE.

RUNNING SHEAR BACK TO HEADGATE WHEN FACE BOUNCED, THROWING COAL AND STRIKING EE ON THE LOWER LEFT LEG AND ANKLE BREAKING BOTH THE LEG AND ANKLE.

PULLLING SHIELD TO PAN LINE. HOOKED UP CHAIN TO PULL IN SHIELD. PIN OR CHAIN BROKE & HIT EE ON THE HEAD.

LONGWALL FACE BOUNCED, THROWING COAL, STRIKING EE ON THE RIGHT SIDE OF HIS FACE. THIS LOOSENED 1 TOOTH AND BROKE ANOTHER ONE.

EE WAS OPERATING SHEARER, STOPPED CUTTING DUE TO FACE CONVEYOR PLUGGED WITH COAL. SLAB OF COAL ROLLED OVER ONTO CONVEYOR, BROKE APART & A CHUNK OF COAL FELL ONTO EE'S L FOOT CAUSING A FRACTURE BEHIND LITTLE TOE. SHEARER WAS NOT TRAMMING AT TIME OF ACCIDENT BUT WAS RUNNING.
