Deer Creek Mine Coal

On 2/26/15 employee reported an injury to his neck that he claims happened on 12/17/14, when he struck a door frame with his head. He has missed several days between the reporting date & the alleged accident date. His first day missed following the date reported was 3/2/15. Investigations continue to determine if an accident actually occurred and if there is a legitimate injury.

EE injured his back on 12-11-14 as he was loading a ¾ chain onto a piece of underground diesel equipment. He had been scheduled to be seen by a physician on 12-17-14 for a separate non-related incident. During this visit he was taken off of work until follow up diagnostics could be performed. EE first day of work missed was 12-17-2014.

Employee was in the process of re-setting a small set of ventilation doors on the Mill Fork compressor station. As he let go of the inby door, it shut on his left index finger causing a laceration and fracture to the finger. He returned to work on his next scheduled shift.

EE was pulling rock props down the longwall face and lifting them over the cable trays into the panline. As he was placing the rock props in the panline he felt pain in his lower back. EE continued to work until 11/06/2014 when he went off for surgery. His first day missed due to this incident was 11/06/2014.

On 8-18-14 employee and a co-worker was lifting a portable pump onto a flat-bed truck. As they were lifting employee felt a pain in his right shoulder. Employee was seen by a physician and was scheduled for an MRI. The MRI revealed that he has injured ligaments in his shoulder that are going to require surgery to repair. His first day missed from this injury was 8-22-14.

Employee was watching the V-Belts in operation on a motor that operates the shaker for the Main West bin. After de-energizing the power he felt the V-Belts to determine their tension. Immediately after making contact with the V-Belt the shaft rotated and his finger was caught between the V-Belt and the pulley. The shaft then immediately returned releasing his finger.

Employee was walking to the tailgate to reset a faulted shield during the mining process. When he reached 125 shield a bounce occurred. The bounce threw material toward the shield employee was standing on. The material hit the bounce guard which resulted in employee being knocked over striking his ribs against the shield. This caused a small laceration to his spleen.

Employee was attempting to cut some holes in a plastic barrel using a reciprocating saw. As he was cutting one of the holes the saw kicked back. The kick back of the saw resulted in his pointer finger on his right hand contacting the saw blade causing a laceration that required five sutures to close.

EE was at his desk in his office and started to feel ill. As the signs and symptoms progressed and with his knowledge of medicine as a certified Paramedic he felt that he was experiencing a possible heart attack. EE initiated the emergency response system where he was taken to a hospital and it was confirmed that he did in fact have a heart attack.

On 2/26/14, employee filled out an incident report for straining his right shoulder. He continued to work until 7/30/14 at which time he had surgery to correct his shoulder problem, after receiving confirmation that his claim was accepted as a work related injury. The difference between the injury date, surgery date and reporting date was pending acceptance of this claim.

Employee injured his shoulder on 2/26/14. While operating a piece of equipment he ran over a cement spill that had dried. This caused him to strike the cab injuring his shoulder. Employee continued to work up until 12/30/14 where he went off for surgery to repair the shoulder. Employee subsequently retired on 12/31/14.

EE was in the process of tightening a take-up on the surface sampler belt using a wrench. As he was pulling on the wrench he felt pain in his left shoulder. EE continued to work up until 6-17-2014 when he had surgery to repair his shoulder. His first day of work missed due to this incident was 6-17-2014.

We received notification at the mine on April 3, 2014 that employee had exercised his Part 90 Miner Rights, from the miner. This was diagnosed in November of 2013 which is the date noted above for illness date. The difference between reporting date and date of illness is related to the date we were made aware of the option to exercise his Part 90 Miners Rights.

On 9-17-13 employee was attempting to lift a belt roller to release a chain and felt a pain in his right groin area. On 10-3 he was sent by his doctor and diagnosed with a hernia that would require surgery to repair it. Employee continued to work until 10-9-13, on 10-10-13 he had surgery. First day from work was 10-10-13

On 8-21-13 employee struck a piece of coal causing the steering wheel to spin straining his shoulder. On 8-26 he was seen by a Dr and released for work. On 9-4 he was taken off work pending an MRI. EE has previous neck and shoulder injuries and this case was under review by industrial to determine if it was a new or old injury. On 10-10 it was accepted as a new injury

Employee was removing a bolt from a filter housing using an 18 inch extension on a breaker-bar. As torque was applied to the breaker-bar the extension broke lacerating his palm against the broken extension. The laceration required two stitches to close.

The employee was in the process of opening a mandoor and as he was pulling the door open he strained his lower back.

On 6-18-13 EE claimed he slipped on loose coal straining his knee. He's been off work pending a medical evaluation. There was no witness to incident. There were implications this may not have been work related. On July 9 this claim was accepted as an industrial accident. Time difference between accident date and reporting date was pending investigation and acceptance.

On 6-3-13 employee filed a claim of repetitive fatigue to his right hand from using a dust hose attached to a bulk dusting system while dusting belts. Employee continued to work with this condition. Over time the condition worsened and on 10-28-13 he had surgery to correct the condition.

On 6-1-13 employee filed a claim of repetitive trauma to both hands and wrists from years of repetitive use. Employee continued to work with this condition while seeking a medical opinion about the symptoms that persisted. On 12-27-13 he had surgery to correct the condition. The first day of work missed due to this condition was 12-30-13.

Employee was in the process of replacing a top belt roller. As the roller was being slide into place it came in contact with a metal shim that flipped up and pinched his left little finger between the shim and the roller frame causing a tip fracture and a laceration that required stitches.

On 4/8/13 ee fell straining his shoulder, he continued to work until 5-9-13 at which time he was seen by a Dr. and taken off work. It was undetermined at this time if his condition was work related, on 8-29-13 it was accepted as an industrial accident, he is schedule for surgery 0n 9-12-13. The difference of the accident date and reporting date was pending acceptance.

On 1-3-13 employee was closing a metal door on a feeder chute at the mines tipple. While closing the door he strained his abdomen. On 1-18-13 he was seen by a Dr. and was diagnosed with a hernia that would require surgery to repair. On 2-12-13 he had surgery.

On 1-2-13 EE filed a claim of accumulative trauma from operating a jack hammer; he stated there was no specific event only that his elbows hurt. 2-4-13 he went to Dr. and was diagnosed with tendonitis. 4-3-13 he return to his Dr. and was taken off work. 6-17-13 this claim was accepted as an illness. Time off and reporting difference was pending a medical report.

Employee was placing a section of wire mesh on the atrs of a roof bolter and as he went to step onto the driller's platform, he missed the platform and his leg dropped approx. 2 feet below the platform. This caused him to strain his right knee. He was scheduled to see an orthopedic Dr. for an evaluation. The Dr. has now taken him off work.

A roof fall occurred just off the tailgate of the 25th West longwall face. The fall was approx. 2.5 feet thick, from 5 feet to 20 feet wide for a distance of approx. 60 feet. The fall blocked access off the tailgate entry. This fall was found at the start of the weekly examination at approx. 12:35 a.m.

EE, a section mechanic was making a splice on a miner cable and as he went to lower the cable his left hand came in contact with a razor knife which caused a laceration that required stitches.

On 9-21-2012 EE was installing cable truss bolts and as he went to install the cable in the truss harness he overextended his arm straining his shoulder. EE continued to work with this condition; on 4-25-2013 he had surgery to correct the condition. Last day he worked was 4-24-2013.

Employee was in the process of installing a bolt in the mine floor to secure a belt drive. As he was rotating the bolt, his left index finger got pinched between an air hose and the frame of the drill causing a laceration that required stitches.

Employee was in the process of rock dusting a belt line. The duster stop for a minute so he sat on a water line that was held up by chains attached to a rib bolt plate. The plate that the water line was hooked to broke and the water line fell landing on his ankle causing a fracture to his lower leg.

Employee was removing coal from the cable trough on the longwall face when a piece of rock fell off the tip of a shield and struck the back of his right hand causing a fracture.

On 8-6-12 EE was moving a stopper and felt a pain in his knee. He continued to work; on 9/10/12 he was seen by a doctor and was taken off work pending a MRI. On 9-17 he had a MRI. On 11/15/12 he had surgery. The time difference between reporting and submitting report was pending a medical report that determined that this was accepted as an industrial injury.

On 8-3-12 employee was opening a material door and suffered a torn Achilles tendon. After a medical exam it was determined that sometime prior to opening the door he had injured his tendon, this injury is being submitted as an accumulative trauma type of injury. The time difference between when the injury occurred and the reporting date was pending a medical report.

The injured was recovering a screw that had fallen to the bottom of an electrical cabinet. The cabinet was top heavy while on his hands and knees attempting to get upright the cabinet was falling forward. He tried to stop the cabinets forward movement. His left hand was caught between the cabinet and the metal decking. He developed a contusion.

On 7/20/2012 employee was in the process of cutting down some old mesh. While cutting down the mesh he was struck in the head with a loose piece of top. He was seen by a Dr. on 7/27/2012 and was released to work that day. He continued to work while waiting for industrial to accept his claim up until his surgery date of 5/6/2014.

Employee was repairing a mod valve on a duster when the valve twisted and fell smashing his right middle finger between the valve and the duster frame causing a laceration which required stitches.

A roof fall was found off the tailgate entry of the (22nd West) of the 23rd West longwall. The roof fall was approximately 16 feet from the tailgate and was approximately thirty feet in length, twenty feet in width and approximately five feet in height. The roof fall block access off the tailgate.

EE was in the process of lowering a metal cover on a miner and as he dropped it into place a cover fell smashing his left hand between the miner frame and the cover causing a laceration and a fracture to his left thumb.

Employee was setting a rock prop. The slammer used to set the locking wedge in the rock prop fell from the prop and struck the employee in the arm causing a laceration.

On 4-23-2012 employee was traveling down the longwall face when he tripped and fell injuring his shoulder. He continued to work until March 28th 2013 at which time he had surgery to repair his shoulder. First day of work missed due to this injury was March 28th.

IEE and three other employees were in the process of lifting a metal side board into place on a feeder breaker. As one of the employees' s started to lift the side board it slipped and fell striking EE on top of his left foot causing a fracture.

EE was working at the mines tipple and as he was walking to a parked truck, he slipped and as he fell he landed on his left elbow which caused a separation of his left shoulder.

EE was helping replace a section of ventilation tubing. As he was holding up the tube another miner was attempting to attach the tube hanger to the mines roof mesh using a bar. The bar slipped off the tube hanger and struck EE across the nose fracturing his nose. the fist day of work missed due to this accident was 2-27-2012.

Employee is a roof bolter, on 1-16-12 he was positioning a section of wire mesh when the mesh got hung up and he strained his shoulder. He continued to work but his shoulder never healed. Employee was seen by a doctor and it was determine he needed surgery. On 10-8-12 he had surgery to repair his shoulder.

On January 12, 2012 EE was walking and his feet got tangled up in a plastic band which caused him to fall. As he went to catch himself he injured his left shoulder. EE continued to work. On February 3rd, he went to the Dr. and was taken off work. The first day of work missed was 2-6-12.

On 10-10-2011 employee moved a shuttle car anchor. Later during the shift he started to have pain in his left shoulder. He had previous problems with his shoulder. On January 5, 2012 he had surgery on his shoulder. We received his medical report and it was determined that his injury was due to the event on 10-10-2011. The date of the report was pending a medical report.

Working on a belt drive installation. He was welding on a belt drive installation guarding when a piece of rock struck him on the right hand causing a fracture of the little finger.

Employee was repairing the mesh handler on a roof bolter when the handler shifted and caught his hand and wrist between the handler and hoses on the bolter. Employee continued to work until January 19, 2012. Employee was scheduled for surgery on his wrist. First day away from work was 1-23-2012.

On 7-18-11 employee was stepping down off a gob pile and the material shifted causing him to twist his right knee. He continued to work but his knee never healed. On 1-30-12 he had to have surgery to repair his knee.

Employee stated that on 6-23-11 he had struck the back of a mason hammer with a claw hammer and received a laceration to his thumb. In November, several months later he started to have problems with his thumb. On 11-04-11 he was examined and a piece of metal was embedded in his thumb. He believes this was from the incident on 6-23-11

Employee was removing a belt roller frame when his left thumb got caught between the roller frame and a belt rail. This caused a laceration which required stitches.

Employee was attempting to move a section of miner cable, as he reached down to lift the cable the cable was pulled by a piece of mobile equipment. This caused the cable to flip up and strike employee across his right knee knocking him down. This caused injuries to his knee that may require surgery.

On 4-4-2011 employee was walking in the mine and slipped and strained his knee. He was seen by a Dr. and released for work. His knee continued to bother him. He was seen by a Dr. again and it was determined that surgery was needed. Employee's first day off work because of the surgery was July 25, 2011.

Employee initially injured his back 9-18-08; he continued to work, still having back problems. On 3-4-11 he was working on an overcast and aggravated his back. He continued to work until 4-7-11; he then went off work pending a medical evaluation. The difference between the reporting date and the accident was pending the information from the Dr. We received it 7-11-11.

Employee was replacing a cover on a miner and as he attempted to put the cover back in place he caught his finger between the edge of the cover and the frame of the miner causing a laceration and a fracture to the end of his right ring finger.

Employee is a pumper and was loading a five hp pump in the back of a truck. He felt a strain in his right shoulder and bicep. He continued to work but his shoulder and bicep still bothered him. He went off on surgery to repair a tear in his bicep on 3-17-2011 making this MSHA reportable.

Continuous miner operator was struck on the chin by a piece of coal causing a laceration that required stitches. The stitches made this a reportable accident.

EE was working on a truck that wouldn't start; he reached in to rotate the crank shaft by pulling on the fan belt when another employee activated the start switch. His fingers were caught between the belt and the alternator pulley. The ends of his little and ring fingers on his right hand were amputated.

Employee is a bathhouse attendant and was repairing a wash down hose by putting on a screw clamp. He was holding the hose and clamp with left hand and tightening the screw with his right hand. The screwdriver slipped of the screw and punctured his left hand which required stitches to close wound.

A supervisor found a roof fall in the # 6 intake entry of the Mill Fork Mains. This entry is not a normally traveled entry. The roof fall was approximately 20 feet wide, 60 feet long, and 5 foot in height. The roof fall was immediately reported to MSHA and blocked off to prevent entry. Current plans are to re-bolt the area.

Employee was swinging an axe to knock a hole in a stopping. As the axe went past his belt his hand hit a razor knife attached to his belt which had the blade out. This caused a laceration to his hand that required stitches. Note: Mine was idle due to vacation shut down from 7-26-2010 through 8-9-2010 to reflect reporting time frame difference.

Hanging a roof bolter cable when a rib fell and struck him on the left foot causing a laceration requiring stitches and dislocation of two toes on the left foot.

On 7-6-2010 ee was recovering a water pump using a cart. The cart started to tip over and as he attempted to stop it from falling over he strained his right shoulder. EE continued to work. On 1-14-2014 he had surgery to correct this condition. The first day of work missed due to this condition was 01-14-2014.

Employee was working on the longwall face as a longwall helper. A piece of coal came from the longwall face and struck him on the face, causing a laceration that required stitches, making this accident reportable.

Employee went to kneel on a platform. As he raised his left knee it hit the platform. He kept having problems and reported the accident to management on 4-22-2010. He was taken to a medical facility where an X-ray was taken which found a fracture to his knee cap. He saw an orthopedic surgeon on 4-26-2010 and was taken off work.

Employee was in the process of disconnecting a clevis from the end of the boom of a mule and a material trailer when his hand got caught between the boom of the mule and the trailer causing a laceration to his left hand which required stitches.

The employee was using a hand drill to make a hole in a section of plastic pipe. While holding the drill, the drill bit stuck and jerked his arm and shoulder. Employee continued to work until June 2, 1010 at which time he went off work. Accident did not become reportable until June 2, 2010.

Miner was walking along the longwall face when he slipped and his foot went between the relay bar and toe of the shield and was pinched. This involved fractures of the toes on his left foot a nd also the loss of skin on the first two toes of the foot. The bones of the first two toes were still attached but toes of foot were surgically removed after accident.

The Employee was in the process of removing belt structure when a top belt roller slide along a belt rail and pinched his finger between the roller frame and the belt rail causing a fracture and a laceration to his finger.

Employee was using a porta power jack to straighten out a section of bent metal when the jack became dislodged and struck him in the face causing a laceration and fractures to his face.

Employee was attempting to remove a damaged wheel bearing with a hammer and a chisel when a piece of metal from the bearing race chipped off and struck him in the face and become imbedded in his left cheek.

Employee was walking along a belt when a piece of coal bounced off the belt and landed on top of his left foot causing a fracture to his foot.

Employee strained his shoulder lifting a DA ram up on a clevis rail on the longwall face. He continued to work performing his regular duties but his shoulder never healed. On 2-24-10 the employee had surgery to repair his rotator cuff. The difference between the accident and reporting date is because this was the first day the employee missed work for this injury.

Employee was riding in a vehicle when it hit a rib. This caused him to be thrown against the cab of the vehicle causing a laceration on his head which required stitches.

Employee was installing a rock prop support. As he stood up the support it started to fall and he tried to catch it. His finger was pinched between the support and another support that was set. This caused a laceration of his little finger and fracture to the tip of the finger.

Employee was replacing a damaged track on a ASV loader. As he was attempting to pry the track into place with a bar, the bar slipped and caused the employee to strain his abdomen which herniated.

Employee was in the process of washing out the scrubber duct on a miner and had lifted up a metal cover to spray inside of the scrubber duct. As he started to wash out the scrubber the cover fell landing on his arm causing a fracture.

Injury was operating a shuttle car and drove off a lip in a crosscut and hit his head into the cab injuring his neck. He continued to work until March 1st 2010, at which time he was taken off work pending surgery on his neck. The difference between the accident date and report date is because He had not missed any work until March 1st 2010.

Examiner found a roof fall on the 3rd North belt just outby x-cut 93 at approx. 1:30 pm. Operator did not feel this roof fall was at or above the anchorage zone but was reporatable for impeding travel in the belt line. MSHA was immediately notified and roof fall was investigated. The roof fall was approximately 20 ft wide, 35 ft long and approx. 4.5 feet in height.

Employee was in the process of cutting a section of discharge line with a saw when his finger slipped off the finger guard and got pinched between the saw housing and the blade retaining bracket causing a laceration to his little finger.

While brushing the top with the continous miner head an ignition of coal dust occurred. This was of short duration of 2-3 seconds. No individuals were injured or no equipment damaged. Section was immediately shut down and MSHA notified and an investigation was made of the ignition.

On 2-26-09 EE was struck by a piece of coal which may have aggravated a previous neck surgery. EE went off work 7-13-09 for surgery. After a review of all of the doctors reports it has been determine that the second surgery was required because of an aggravation. The difference between the accident date and reporting date was pending the final review of his surgery.

EE was working with another employee installing a belt take-up. The second employee was setting a wedge in place using a hammer and as he swung the hammer to strike the wedge, he missed the wedge and the hammer slipped from his hand and struck EE in the lip, causing a laceration to his lip.

A bounce occurred adjacent to the stageloader caused rib material to fall which knocked out rib support. The rib support and loose rib material fell into the walkway and stageloader stopping production.

Employee was lowering a section of 4/0 cable from the mine roof when the cable slipped from his hand. The insulating hook used to support the cable caught on his glove then caught his hand causing a laceration.

On 9-24-2008 a dust ignition occurred in the 5th L miner section. MSHA was immediately notified and an investigation conducted. It was found that the water sprays had been plugged on several water sprays and the bits had also been worn. The company thought this had been submitted but after a review it was found the report had not been filed. This report is to correct the error.

Employee was adjusting a belt head roller when a bolt securing a mounting plate broke and struck him in the mouth causing a laceration requiring stitches.

A diagnosis of carpal tunnel syndrome was made that may have resulted from work activities. The date noted above is the first day from work because of the surgery. The difference from the date of the report and the date of the illness was pending a medical report from the physician and the acceptance as an industrial illness.

Employee was cutting a piece of rubber to make a seal when the knife he was using slipped and the the back of his left middle finger causing a laceration which required stitches.

A diagnosis of carpel tunnel syndrome was made that may have resulted from work activities. The date noted above is the first day from work because of the surgery. The difference from the date of the report and the date of the illness was pending a medical report from the physician and the acceptance as an industrial illness.

Employee was reviewing the screen display of a SCC read out when a cutter shaft for a crusher motor fell off the head gate drive motor and smashed his finger causing a laceration to the little finger on his left hand.

Employee was attempting to loosen a bolt with a ratchet when the socket slipped off and struck the employee in the chin knocking a tooth out.

EE, a longwall propman attempted to lower a shield on the longwall face. A valve soleniod malfunctioned, which caused the pull ram to activate prematurely, tightening up a chain which broke and a link from the chain struck EE on the leg and caused a hair line fracture.

Miner was using a chuck wrench to knock a hole in a stopping. As he swung the wrench, it hit a cable and deflected the wrench and it hit his hand, causing a fracture to08 his finger.

Employee is maintenance trainee and was watching mechanic repairing a piece of equipment. He placed his hand on tire of the vehicle and hit hand was caught between the tire and frame of vehicle.

A roof fall was found in the return entry off of Mill Fork Access section at crosscut 69. This roof fall was above the anchorage zone but was not in the weekly examiner route of travel but did block travel in the entry that fell.

Employee slipped while tightening a bolt with a ratchet and strained his shoulder. This employee worked for several weeks then went off work and had surgery. Date noted above was first report of injury. Difference of date of injury and date of report was pending a medical report from physician and acceptance as in industrial injury.

A roof fall occured at an intersection where there was an intake/return overcast in 10th West at crosscut 11, # 6 entry. The roof fall was approximately 35 ft in length, 20 ft in width, and approximately 6 to 8 ft in height. The fall was at or above the anchorage zone and impeded travel in the return and intake entries. This roof fall was immediately report to MSHA.

Employee was replacing a guard along a belt drive and as the guard dropped into place his finger was pinched between the guarding and frame.

While operating the shear a piece of rock flipped off the cutting drum and struck employee in the back of the hand causing a fracture.

A roof fall occurred in the number 2 entry of the 3rd north mains between crosscut 91 and 92. the fall was approx. 5.5 feet high X 20 feet wide and 75 feet long. the area was supported with 6 foot resin bolts during initial development.

Employee was attenpting to remove a pump head from a emulsion pump and the pump head fell striking him on top of his foot causing a fracture.

Employee claimed he was stepping backwards and fell and as he reached back to catch himself he strained his shoulder. This employee had a previous condition with this shoulder. The difference between the date of injury and reporting date was pending a medical report from the Dr. and acceptance of the claim.

On 8-30-07, employee twisted his knee and was later diagnosed with a degenerative knee problem that may have been work related. On 9-26-07 he had surgery. The difference between the report date, injury date and return to work date was pending a medical report and acceptance by workers comp.

Employee was preparing to drill a hole to install a resin bolt and as he reached to raise the drillhead he hit the wrong lever and closed the steel guides on his fingers.

Employee was installing a winchter motor for a conveyor drive when the motor shifted and pinched his thumb between the motor and the motor housing.

Employee stated he was attempting to open a door, he pulled on it first then jerked on the handle and his hand slipped off straining his finger. First report of injury was received 7-18-07. Employee had surgery Aug 3rd. The difference between the reporting date, surgery date and accident date was due pending a medical report verifying injury.

An area approx. 20 feet by 25 feet by up to 8 feet high fell in the number 2 entry of the Rilda break outs. This fall was above the anchorage zone. initial roof support was 6 foot bolts. This is one of 3 entries.

Employee was drilling a hole when the drill steel came loose and struck him in the face causing a laceration to his nose.

Employee was attempting to loosen a bolt on a belt pre-cleaner with a socket and ratchet. The socket slipped off the bolt and the ratchet struck him in the mouth causing a laceration to his lip.

A roof fall occurred on the 16th West retreating longwall headgate entry, inby crosscut 34. The fall was approx. 20 feet wide by 20 feet long, approx. 2 to 4 feet high. The roof sagged down on the stage loader impeding travel. Primary support was 7 foot resin bolts with additional 10 foot cable bolts between each row of bolts. Additional supports were installed in the area.

A diagnosis of carpal tunnel syndrome was made that may have resulted from work activities. The date noted above is the first day away from work because of surgery. Difference from date of report and day away from work was pending medical report from physician and acceptance as an industrial illness.

Employee was operating a shuttle car when the door popped open then slammed shut on his finger causing a laceration.

On 3-26-07 EE was getting out of a truck and his foot got hung up, he fell to the ground injuring his back. He continued to work with back pain through this time period with no other incidents. On July 18, 2011 he had surgery which has been accepted as an industrial injury. The difference of the first day off and report date was pending a medical report.

Mine lost power. Employee was send sent to investigate and found a roof fall in the main line just inby the Rilda intake portal. The roof fall was above the anchorage zone and blocked passage of personnel. Mine called MSHA and inspector came and investigated the roof fall.

Employee was walking through an area that had been re-graded and slipped on some loose rock causing him to fall. As he reached to catch himself with his hand, he jammed his wrist, which caused a fracture to his wrist.

Employee was walking to his car in the parking lot and slipped on some ice and fell jamming his shoulder. The employee continued to work. After a followup visit with a physican it was determined that surgery was required. On 4-2-2007 surgery was performed. This was the first day away from work.

Employee stepped in a mud hole and twisted his knee as he turned around. The difference between the accident day and report day is because employee continued to work. First day away from work due to injury was 4-30-2007.

A diagnosis of carpal tunnel syndrome was made that may have resulted from work activities. The date noted above is the first day from work because of the surgery. The difference from the date of report and day away from work was pending a medical report from the physicain and acceptance as an industrial illness.

Cutting coal face with rock band. Spark from rock caused coal dust to ignite. MSHA was notified and investigated. Water sprays and pressure were as required and bits on miner were in good shape. Ventilation was good in the face and last open crosscut. Localized area affected mostly on left side of miner.

Employee was replacing damaged belt rollers railings when a top roller fell between the rails and smashed his finger causing a fracture to his left index finger.

Employee reached down to help guide a tram motor that was being pulled between a section of the stage loader and a roof support when the motor rolled pinching his finger between the motor and the stage loader which amputated the end of his finger off.

A diagnoses of carpel tunnel syndrome was made that may have resulted from work activities. The date noted above is first day from work because of surgery. Difference from date of report and day away from work was pending medical report from physician and acceptance as an industrial illness.

Employee was using a lathe to reduce the size of a roll pin to repair a hinge when his gloved got caught between a piece of emery cloth and the pin, pulling his hand into the pin. This incident caused lacerations, a fracture and 2 dislocations.

Employee was installing a resin roof bolt and as he went to raise the drill head on the bolter he hit the wrong lever and closed the steel guides on his finger.

EE stated he was rock dusting and stepped on an uneven surface and strained his knee. He finished his shift without incident. The following day he seen a doctor and was scheduled for exploratory surgery on 10-23-06. The medical report indicated a tear. The report was received 11-29-06. The difference from the accident and reporting date was due to receiving a medical report.

Employee was walking in a return when a bounce occurred and piece of coal from the rib flew out and struck him in the face causing a laceration to his face.

Employee was stepping down from a belt cross over and twisted his knee causing him to fall. The fall hyper extended his knee. The initial doctor visit released the employee to work. On 9-26-06 he return to his doctor and has been taken off work. This is why there is a difference from date of injury and date report was submitted.

As the employee was walking along the side of a bundle of wire mesh he stumbled and as swung his arms to catch his balance he struck a piece of metal banding which caused a laceration to his thumb.

A roof fall approximately 75 feet in length by 20 feet wide from 1 to 4 feet thick fell in the intake entry between the # 51 and 52 crosscuts in the 15th west section. It appears that a lateral shift of the top may have occured. The top in this area consisted of laminated shell and cap rock.

Employee was scaling down a section of loose rib to complete a stopping when a piece of rib fell and struck him in the hand and arm causing a laceration and fracture to the tip of his finger

The employee was helping carry a top belt roller and the other employee dropped his end causing the roller to pull from the injured employee's hands and striking him on top of the foot causing a laceration to the top of his foot. It should be noted that this happened on last day of work prior to a two week vacation shut down.

While installing a section of ventilation tubing a piece of rock fell through a piece of wire mesh and struck the employee on a tooth breaking the tooth.

Employee was attempting to get a crib block from the gooseneck of a trailer and felt a pop in his shoulder.

Employee was building a stopping and was wedging the top in and struck his finger with a block cauing a laceration to his finger

Employee was operating the shear and had just cut through the tailgate and had stop to discuss the grade cut with the other shear operator and the face blew out striking him with a piece of coal in the face.

Employee was hanging a water line. While hanging the line it came loose and fell striking him on the shoulder causing a bruise and a strain.

Employee was operating a truck and the brakes went out on the truck. As the employee attempted to make a turn the truck struck a rib which cause him to strike his hand against something in the truck fracturing his finger

The employee was operating the continuous miner when a rib sloughed off and struck the employee in the right leg, causing a strain to his leg.

Employee was operating the shear and had just cut through the tailgate end of the face when a bounce occurred in the floor and jarred the employee's back. This employee has had previous back problems and continued to have problems with his back and eventually required surgery. The first day away from work from this incident was April 14, 2006.

The employee was bent over in the process of setting the bottom end of a timber in place when another timber fell striking him in the head which knocked him down, this caused a strain to his lower back.

Employee was unloading a belt roller and the roller slipped from his hands and fell and landed on his foot causing a fracture to his left little toe

Employee was attempting to roll a rock out of a walk way when the rock rolled back pinching his finger between two rocks

An area approximately 20 feet wide by 45 feet long by 7 feet high fell in a return entry. The top in this area consisted of laminated shell and mud stone. The fall area was wet with water dripping from the fall area.

The employee was diagnosed with carpal tunnel syndrome which may have resulted from work. Date noted above was first day report was filed. Employee received surgery on 7-18-2006 for condition.

Employee was walking along the Longwall face and stepped on a relay bar which was under water and slipped twisting his knee. Note the time between accident and reporting, employee first day of restricted duty was Feb. 24th 2006

Employee was installing sandwich plates along the longwall pan and dropped a plate and it smashed his finger

The employee was cleaning a floor in a service pit area and became dizzy and fell straining his back, leg and shoulder. The employee had been suffering dizzy spells for several days prior to this incident. The employee is also seeking medical treatment for the dizziness.

Employee was in the process of setting rock props and as he was handed the setting hammer for setting the props he felt something pop in his shoulder. the time differaece between the accident date and the date submitted is because the employee did not loose any time until May 12 2006 from this incident

The injured employee was behind the continuos miner as the miner operator was moving the miner to the next face, the slack from the miner cable caught the employees legs and knocked him down. The time difference from the reporting date and date of injury is due to waiting for the employee to be scheduled for a MRI suggested by the attending physician.

Employee was climbing out of a scoop when his foot hit the directional control lever which caused the machine to move and pinch his hip. There were no fractures or internal injuries as a result of this injury.

Employee was bending a piece of wire mesh and fell on the mesh causing a cut to his chin.

Employee was unloading roof bolting material and his finger got caught between some roof bolts

Employee was loading bolting material on a roof bolter and as he was placing roof bolts on the machine he caught his finger between the bolts and the edge of the machine's cab.

EE stated that he placed his hand on the mine floor while moving from under a section of vent tubing and something fell and struck his hand. The difference in the accident date and reporting date is due to waiting for a medical report which indicated a very small fracture, this employee went to the doctor on his own the following day, we received the report on 07/08/2005

Employee was carrying a bucket of brake-away pins for shields and felt a sharp pain in abdomen. Employee had surgery on 7-9-2005 for this condition. Time difference between surgery and reporting date was waiting for final diagnoses medical report from doctor. Report received on 08/31/2005.

Employee has been diagnosed with carpel tunnel syndrome. The date indicated above is the date of surgery. The difference from the date of reporting and surgery date is due to this claim being accepted as an industrial illness.

Employee was driving a truck along a intake roadway and missed a turn and ran into the rib

Employee was removing bolts from a pre-cleaner on a belt when the wrench he was using slipped of the bolt and struck him in the mouth.

Employee was walking in a intake entry and stepped in a hole twisting his knee. The employee work several days after this incident before seaking medical treatment. The employee recieved medical treatment for this injury April 14th 2005.

On 2-28-05 EE reported that on 1-17-05 he felt a pain in his stomach after lifting. On 3-07-05 EE had surgery for a hernia that may have been work related. The date noted above is the first day away from work due to the surgery, the difference from the date of this report and the surgery date is due to waiting for a medical report from the attending physician.

Employee was installing a gear box in a drill carriage and pinched his fingers insided the carriage housing.

A diagnous of carpal tunnel snydrome has been made that may have resulted from work activities. Date noted above is first day employee missed work.

EE was reparing a metal cover when it fell and pinched his finger between the metal cover and the spill plate on the face

The employee was assisting in spotting a material trailer and got caught between the machine and the rib.

EE was pulling a water hose around a corner and twisted his knee. EE received medical treatment on March 9, 2005 at which time made this a reportable accident.

Employee was standing in an entry about 100 feet from a forklift that was hauling cribs when the forklift caught on a joint of pipe and caused the pipe to bow out into the entry knocking him down.

A roof fall occured in an intersection in the Mill Fork Left section. The fall measured approximately 20' x 20' x 6' in height. This roof in this area is laminated mustone and cap rock. Roof fall was in rock rooms and was not part of the working faces on this section.

Employee was changing out belt rollers and his finger was pinched by the belt roller and frame causing a fracture to the tip of his finger.

A roof fall occured in an intersection approximately 2500 feet outby the face area. the fall measured approx. 45' X 35' X 6' high. The top in this aera was a laminated mudstone and cap rock. The area inby is a wet area which may have contributed to the fall

Employee was attempting to move the miner cable when his leg got caught between the miner cable and the miner as it was backing up.

EE WAS CARRYING A PIECE OF WIRE MESH ALONG SIDE OF A ROOF BOLTER AND THE RIB ROLLED OUT STRICKING EE IN THE LOWER LEG

EMPLOYEE WAS REMOVING A DRILL HEAD OFF OF A PORTABLE DRILL AND SMASHED HIS THUMB BETWEEN THE DRILL HEAD AND DRILL TABLE.

EMPLOYEE WAS HELPING ON THE MINER TENDING CABLE WHEN A PIECE OF RIB 12"X 12"X 32" ROLLED OUT LANDING ON TOP OF HIS FOOT.

THE MINER HAD CUT APPROX. 13 FEET INTO THE LEFT SIDE OF THE #2 FACE IN FIRST LEFT SECTION. IT APPEARS THAT WHEN THE BITS ON THE MINER STRUCK AGAINST THE SANDSTONE TOP, DUST WAS IGNITED NEAR THE HEAD OF THE MINER. THE IGNITION SELF-EXTINGUISHED IMMEDIATELY.

Employee was installing a fire supressing bottle over a battery charging station and strained his abdomen.

A CHUNK OF ROCK HAD BECOME WEDGED IN A COAL CHUTE AND THE EE AS USING A SLEDGE HAMMER TO BREAK UP THE ROCK AND FELT SOMETHING PULL IN HIS SHOULDER. 1ST DAY OF WORK MISSED FROM THIS INJURY WAS 2/26/04

ON THE DATE NOTED ABOVE EE WAS CAUGHT BETWEEN A WAKER HOSE AND THE RIB AGGRAVATING AND PREVIOUS INJURY. SINCE RETURNING TO ENGERY WEST IF 1998 EE HAS HAD SEVEN BACK AND OR HIP INJRIES. IT SHOU LD ALSO HE NOTED THAT HIS BACK AND HIP WAS SERIOSLY INJURED WHEN HE WAS INVOLVED IN MINING ACCIDENT WHILE WORKING FOR ANOTHER COMPANY.

EE WAS TROUBLE SHOOTING A POWER CIRCUIT TO A CRUSHER MOTOR LOCATED AT THE MINE TIPPLE.

Employee was lifting a section of 60" belt structure and strained his shoulder. This employee did not seek medical treatment for several weeks. Initial treatment was physical therpy which did not help. Employee required surgery and this was done several months later. Employee's fisrt day of restricted duty for this injury was June 14th, 2004

EMPLOYEE WAS STEPPING OFF THE BACK END OF A ROOF BOLTER. HE STEPPED ON A PIECE OF COAL OR ROCK IN A MUD HOLE AND TWISTED HIS ANKLE.

WORKING ON WATER LINE. COUPLING BLEW APART & PIPE OR COUPLING HIT HIM ON THE LEG.

OPERATING ROOF BOLTING MACHINE. INADVERTENTLY HIT DRILL GUIDE CLAMP LEVER & CLOSED GUIDE ON FINGER CAUSING LACERATION OF THUMB.

THE EE WAS POUNDING ON A CHAIN TRYING TO SEPARATE A LINK WHEN A CHUNK METAL FLEW OFF THE EDGE OFA LINK AND STRUCK THE EE.

EE WAS HELPING CUT DOWN WIRE MESH FROM THE MINE ROOF WHEN A SECTION OF MESH STRUCK HIM IN THE FACE.

EE WAS PREPARING TO LOWER A FACE SHIELD DURING EXTRACTION AND HIS FOOT WAS PINCHED BETWEEN A BRIDAL STRAP AND THE RELAY BAR ON THE SHIELD.

THE EE WAS ADJUSTING THE DRILL HEAD ON A PAN BOLTER TO INSTALL A ROOF BOLT. THE OPERATOR HAD STEPPED OFF OF THE DRILLING PLATFORM WITH ONE FOOT & PLACED IT BETWEEN A SPRING BAR AND THE SKID P LATE FOR THE DRILL HEAD WHEN THE DRILL HEAD WAS MOVED IT PINCHED HIS FOOT BETWEEN THE TWO AREAS.

STRUCK ON FOREHEAD BY WATER MANIFOLD DURING INSTALLATION OF WATER LINE.

EE CLOSED THE DRILL STEEL GUIDES ON HIS FINGER WHILE HE WAS OPERATING A ROOF BOLTER.

EE WAS CLOSING ONE SIDE OF AN AIRLOCK DOOR AND THE OTHER DOOR CLOSED ON HIS ARM.

EE WAS REPLACING A WATER HOSE ON A TRAM MOTOR ON A MINER WHEN A METAL HEAD BLOCK FELL LANDING ON HIS ARM.

EE WAS UNLOADING A BELT ROLLER FROM A MATERIAL TRAILER. THE ROLLER GOT HUNG UP AN HE JERKED ON IT TO FREE IT UP AND STRAINED HIS SHOULDER

EMPLOYEE WAS OPENING A SET OF MATERIAL DOORS AND STRAINED HIS SHOULDER. FIRST DAY OF WORK OFF 4/16/03. TIME DIFFERENCE FROM ACCIDENT DATE AND DATE OF REPORT WAS WAITING FOR MEDICAL REPORT CONF IRMATION RECEIVED 5/5/03.

EMPLOYEE WAS ATTEMPTING TO REMOVE A SECTION OF GUARDING AROUND A BELT DRIVE AND CAUGHT HIS FINGER BETWEEN TWO SECTIONS OF GUARDING.

A DIAGNOSIS OF CARPAL TUNNEL SYNDROME HAS BEEN MADE THAT MAY HAVE RESULTED FROM WORK. DATE NOTED ABOVE WAS FIRST DAY OF WORK MISSED.

EE WAS CLEANING A BELT CUTTER & RAN THE EDGE OF HIS FINGERS ACROSS THE BLADE
